Summary In 19 patients after cholecystectomy alkaline phosphatase activity and bilirubin concentration in serum and bile (gall-bladder) were measured. — In another 10 patients with t-tube in the ductus choledochus, the daily volume, alkaline phosphatase activity and bilirubin concentration of the bile were measured over a longer period (9–66 days). In serum, parallel measurements of alkaline phosphatase activity and bilirubin concentration were carried out. — The alkaline phosphatase activity in the bile was measured after gel-filtration. Results and statistical evaluation: 1. There is no correlation between the levels of alkaline phosphatase activity in serum and bile of the gall-bladder resp. ductus choledochus. 2. There is no correlation between the levels of alkaline phosphatase activity and of bilirubin concentration in the bile of the gall-bladder resp. ductus choledochus. 3. There is no correlation between the daily volume of bile drawn from t-tube and its alkaline phosphatase activity. Conclusion: The levels of alkaline phosphatase activity in the bile of the gall-bladder as well as the ductus choledochus are not the result of an active excretion of serum alkaline phosphatases by the liver, but the result of an active production and sccretion of isoenzymes by liver-cells and endothelian cells of bilecapillaries. There is sufficient reason to believe that serum alkaline phosphatases arenot excreted unaltered, but have to becatabolized by the liver.

Zusammenfassung Von 19 cholecystektomierten Patienten wurden AP-Aktivität und Bilirubinkonzentration in Serum und Blasengalle bestimmt. — Bei 10 Patienten mit liegendem T-Drain im Choledochus wurden täglich Gallenvolumen, seine AP-Aktivität und Bilirubinkonzentrationen über 9–66 Tage p.o. gemessen. Parallele Bestimmungen der AP-Aktivität und Bilirubinkonzentration im Serum erfolgten alle 1–3 Tage. Die Aktivitätsbestimmung der AP in der Galle wurde nach Gelfiltration durchgeführt. Die Befunde und ihre statistische Auswertung ergaben: 1. Die Höhen der AP-Enzymspiegel in Serum und Blasen- und Choledochusgalle stellen verschiedene und voneinander völlig unabhängige Größen dar. 2. Die Höhen der AP-Aktivitäten und der Bilirubinkonzentrationen in Blasen- und Choledochusgalle sind voneinander unabhängig. Die AP beeinflußt nicht den Ausscheidungsmodus des Bilirubins (und umgekehrt). 3. Es besteht keine Beziehung zwischen der Höhe des Gallenvolumens und seiner AP-Aktivität pro ml. Ein Vergleich zwischen 8 Patienten mit normalen oder pathologischen Laborbefunden, aber intakten intra- und extrahepatischen Gallenwegen und 2 Patienten mit komplettem Verschluß des rechten D. hepaticus wird durchgeführt. Die Ergebnisse werden diskutiert. Es drängt sich der Schluß auf, daß die vorgefundenen AP-Enzymspiegel in der Gallenflüssigkeit nicht der Ausscheidung von Serumphosphatasen durch die Leber, sondern nur einer aktiven Produktion und Sekretion von Isoenzymen aus den Leberzellen und Epithelien der Gallenkanälchen entstammen.
