    Notes: Abstract Breast milk was analysed in 9 under-nourished Nigerian mothers and 23 well-nourished mothers who served as controls. Milk from the under-nourished mothers contained adequate amounts of lactose and total triglycerides, but had significantly lower bile salt-stimulated lipase activity (BSSL); their mean BSSL activity was only about 50% of the activity in milk from the control group. Total milk protein was also significantly lower than for the controls (1.45 vs. 1.09 g/dl, respectively;P〈0.01). Our findings may have nutritional implications for breast-fed infants of under-nourished nursing mothers.

    Notes: Abstract The lateral chest radiographs of 200 Nigerian newborn infants, whose gestational age was estimated by the Dubowitz examination, were analysed for the pattern of ossification of sternal segments. The length of each visible centre was also measured. The manubrium was ossified in all infants over 35 weeks. All infants 37 weeks and above demonstrated ossification of the first and second mesosternal segments. Ten infants had ossification of the xiphisternum. The average number of ossified segments see in this study is more than previously reported amongst Caucasian infants. The length of each segment correlated better with birth weight than gestational age. Our findings indicate that any infant with two ossified sternal segments, including the manubrium, is at least 30 weeks and those with 3 and 4 segments 34 and 37 weeks gestation, respectively. There is no difference in the pattern of sternal ossification between the sexes.

    Notes: Abstract Nine cases of neonatal necrotising enterocolitis with a combination of gastric distension, paucity of bowel gas and bowel wall oedema are presented. It is postulated that the gastric distension may either be a result of a temporary obstruction at the pyloric canal from oedema or a direct effect of bacterial toxins on ischaemic gastric mucosa. Six (66%) of the 9 infants were term and it is suggested that any newborn infant presenting with isolated gastric distension with bloody stools should be treated as a case of necrotising enterocolitis.

    Notes: Abstract Twenty-seven Nigerian infants with osteomyelitis during the first 28 days of life are presented to highlight the severity of the clinical manifestations and the radiological features of this infection. The clinical signs include limitation of movement of the extremities involved and localized swelling. Severe constitutional changes such as fever (〉39°C) and abdominal distension were common. This contrasts with results from North America and Europe which emphasize the paucity of clinical signs despite involvement of multiple sites. Multiple site involvement was encountered in only eight cases. Staphylococcus aureus, proteus mirabilis and candida albicans were the main pathogens isolated. The long bones were more frequently affected. Aggressive bone destruction was a constant radiological finding peculiar to staphylococcal osteomyelitis while the formation of sequestrum a rare occurrence in the newborn, was present in six patients. The mortality rate was 7.4% and the morbidity rate was equally low.

    Notes: Abstract The presence of the coracoid and proximal humeral head epiphysis in 201 Nigerian newborn infants was related to gestational age. The appearance of the humeral head of ossification centre was delayed, while the coracoid epiphysis appeared earlier than those of Caucasian newborn infants. There was no difference in the pattern of epiphyseal ossification between the sexes.

    Notes: Abstract The total bone width (T) and medullary width (M) of the humerus and femur of 216 and 138 Nigerian newborn infants, respectively, were measured in order to determine the normal standards for cortical bone mass in the newborn. The cortical width (C), cortical area (CA), and percentage cortical area (PCA) were calculated for each bone and correlated with gestational age and birth weight. In both the femur and humerus, the values of the cortical measurements were higher in males. The strongest correlation coefficients were obtained between T(0.84), C(0.79), and CA(0.84) and birth weight in the humerus. The correlation with gestational age was, however, similar in both bones. The values of humeral cortical width (C) obtained in this study is less than had been reported in North American white newborn infants. Cortical measurements of the humerus, which is invariably included in the newborn chest radiograph, is a more reliable method of evaluating the status of bone mineralisation than the femur.

    Notes: Summary A longitudinal study of normal hematological values of the newborn infant was undertaken in an effort to provide baseline data for assessing the African Neonate with hematological problems. There were 402 neonates, consisting of 304 full-term, 51 preterm and 47 post-term infants. The Hematocrit (Hct), Hemoglobin (Hb), Red Blood Cell Count (RBC), Reticulocyte count (Retic) and Nucleated Red Blood Cell Count (NRBC) were serially determined. The red cell indices, Mean corpuscular Hemoglobin (MCH), Mean Corpuscular volume (MCV) and Mean corpuscular Hemoglobin Concentration (MCHC) were calculated for each neonate. Our results showed that African neonates have lower hematological values than their North American and European counterparts. This was neither a reflection of an intrauterine anemia nor was it due to variables resulting from the timing of cord blood sampling. On the first day of life, the mean Hct was 45.4%; mean Hb was 15.46 gm/dl; and the RBC was 4.02×106 cells/mm2. The Retics, NRBC and other red blood cell indices do not differ from those of neonates reported from other parts of the world. We suggest therefore that the low hematological values of the African neonate may be intrinsic.
