    Notizen: Persistent hypereosinophilia, cardiac involvement and a recurrent erythematous-papular pruritic eruption histologically characterized by eosinophilic spongiosis are described in a 77-year-old man. This condition, suggestive of “idiopathic” hypereosinophilic syndrome, represented the prodromic phase of atypical bullous pemphigoid, which manifested 7 months later.

    Notizen: A 35-year-old Italian doctor came to us for treatment of a scaly, moderately itchy patch on the shaft of his penis (Fig. 1). FHe had first noticed the patch about 20 days before and mentioned that it was spreading slowly. The lesion was oval, a little over 3 cm in diameter, lightly hypochromic, and covered with moderate, tine scaling. The upper part of the border was indefinite, while the lower part was sharply marginated. There were no lesions on the groins or elsewhere. Direct microscopic examination of a potassium hydroxide preparation from the scales showed the presence of septate hyphae. Culture in Sabouraud's agar allowed the growth of colonies that were identified as Epidermophyton floccosum. given their macroscopic and microscopic morphology (Fig. 2). Direct microscopic examination and culture of some small scales taken from the groins and the scrotum were negative.

    Notizen: Abstract: Steroids are effective in the autoimmune bullous disease pemphigus; however, treatment may be difficult to sustain because of severe side effects. Cyclosporine A acts mainly on helper/inducer T lymphocytes and has few side effects at few doses. We report three patients with pemphigus erythematosus who had a relapse while receiving the maintenance dose of steroid therapy. All patients who were treated with both cyclosporine A (5 mg/kg/d) and prednisone (1 mg/ kg/d) responded remarkably well to combined therapy. After clearing, prednisone was discontinued and cyclosporine A was reduced to 2 to 3 mg/kg/d. With this treatment, all patients have been virtually free of symptoms, have remained well, and have had normal laboratory values.

    Notizen: : Plasminogen activators are serine proteinases which transform the serum zymogen, plasminogen, into plasmin, a broad-spectrum protease with fibrinolytic effect. Two main plasminogen activators have been described in humans: urokinase (UK; molecular weight, 55,000) and tissue-type plasminogen activator (tPA; molecular weight, 74,000). Thirteen subjects were studied who had alopecia areata (AA), nine in the active phase and four in remission. There were alterations in the perivascular and peribulbar fibrinolytic activity in the nine subjects in the active phase of disease, suggesting a possible role of plasminogen activators in AA. A modified Todd's autohistographic method was used to evaluate cutaneous fibrinolytic activity (which depended on the activity of plasminogen activators) in the 13 AA subjects and five volunteer controls. Cutaneous fibrinolytic activity was reduced in perivascular areas, but increased in peribulbar areas, in the nine subjects in the active phase of disease. Tests with monoclonal antibodies directed against the catalytic sites of tPA and UK showed that the perivascular fibrinolytic activity was tPA dependent, and the peribulbar fibrinolytic activity was UK dependent.

    Notizen: A 44-year-old white woman was admitted to our hospital for the investigation of many roundish, pale-colored, nodular lesions of different sizes, ranging from a few millimeters to some centimeters, symmetrically grouped on the shoulders (〈link href="#f1"〉Fig. 1), trunk, upper limbs, and abdomen. The lesions first appeared at 20 years of age on the upper chest around the clavicular region, and thereafter gradually increased on the trunk and upper limbs; they were completely asymptomatic, appearing on normal skin without a previous history of inflammation and/or trauma. No scarring or atrophy was evident. The smaller lesions exhibited a miliaria-like shape, being evident in particular on stretching of the skin; the larger ones showed a firm-elastic density, no adhesion to surrounding tissue, and deep localization in the skin. No hypopigmented macules or sebaceous adenoma-like lesions were present. Skin biopsy of a nodular lesion from the right shoulder revealed a normal epidermis and a dermis filled with bundles of dense and coarse collagen fibers (〈link href="#f2"〉Fig. 2). Elastic fibers appeared thin and decreased in number under Verhoeff–van Gieson stain (〈link href="#f3"〉Fig. 3). A diagnosis of cutaneous collagenoma was proposed. Routine laboratory investigations were within the normal range. He had a 3-month history of numerous, asymptomatic, skin-colored nodules, ranging from 0.5 to 2 cm, on the upper back. Skin biopsy of a nodular lesion showed the same features as observed in the mother's biopsy. Routine blood and serum tests and X-ray examinations were within normal limits. Electrocardiogram revealed an early R-wave transition, and echocardiogram showed a left ventricular dilation with thickened aorta; aortic steno-insufficiency was diagnosed under cardiologic examination. The diagnosis of familial collagenoma was confirmed, and similar cutaneous lesions were found in the mother's grandmother and aunt.
