    Notes: We describe the computer simulation concerning the acceleration of the H− ion beams in the range of 150–450 eV. The method of H− ion production by bombarding the accelerated cluster ion beams with a small glancing incident angle on the cesiated metal surface is proposed. The computation of the spatial distribution and H− ion trajectory in the multiple stage ionizer is carried out by the iterative numerical method of Laplace equation. The numerical simulation indicates nearly 100% of the H− ion beams from the cesiated metal surface will be accelerated and appear in the output.

    Notes: Abstract Congenital spinal cord astrocytomas have only rarely been reported. We report two additional cases and a review of the literature on these infrequent tumors. The congenital cord astrocytomas can have obscure initial presentations, including fever and irritability, leading to delays in diagnosis. Magnetic resonance imaging is the diagnostic procedure of choice, allowing noninvasive delineation of the lesion. The congenital tumors are similar in location and pathology to astrocytomas of the cord in older children. As in older patients, initial treatment should include attempted surgical extirpation, delaying or avoiding radiation therapy with its attendant risks. Reported follow-ups of the congenital astrocytomas suggest they will behave in a fashion similar to spinal cord astrocytomas in older children.

    Notes: Abstract We studied nine cases of focal cortical dysplasia (FCD) by MRI, with surface-rendered 3D reconstructions. One case was also examined using single-voxel proton MR spectroscopy (MRS). The histological features were reviewed and correlated with the MRI findings. The gyri affected by FCD were enlarged and the signal of the cortex was slightly increased on T1-weighted images. The gray-white junction was indistinct. Signal from the subcortical white matter was decreased on T1- and increased on T2-weighted images in most cases. Contrast enhancement was seen in two cases. Proton MRS showed a spectrum identical to that of normal brain.

    Notes: Abstract Five children developed symptoms attributable to syringohydromyelia several years following neonatal surgery for lumbosacral meningomyelocele. Metrizamide injection followed by computed tomography (CT) was performed in all five patients. In two patients, there was direct opacification of both the subarachnoid space and the syringohydromyelia. In a further two patients, delayed CT demonstrated late opacification of the central cavity from contrast introduced into the subarachnoid space. In the fifth patient via a suboccipital puncture, there was fortuitous filling of a caudally displaced fourth ventricle communicating with a central cavity within the cord, but there was no visualization of the subarachnoid space. Surgery with decompression and plugging of the obex halted the progression of the disease in all the patients.

    Notes: Abstract MR angiography using 3D and 2D time-of-flight techniques were used to evaluate pediatric neurological disorders. MRA (arteriography) and MRV (venography) were abnormal in 63 and 45 cases, respectively. Conventional cerebral angiography was performed in 30 cases. These techniques were compared with MRI and conventional angiography. In addition, the value of MR angiography for surgical planning was subjectively evaluated. Our results showed that intracranial vessels were invariably better showed on MR angiography than on MRI. MRA and MRV were most useful in evaluating vascular distortions related to congenital brain malformations and intracranial tumors. MRA was valuable in detecting arterial narrowing but overestimated the degree of stenosis compared with conventional angiography. MRV was the technique of choice for evaluation of dural sinus and cerebral venous thrombosis and compression. MRA played little to no role in preoperative planning of vascular malformations and aneurysms. It did not appear to be accurate in assessing tumor vascularity or lesions in small arteries and arteritis.

    Notes: Abstract The entire length of distal tubing from a ventriculoperitoneal shunt was found to have migrated into the subgaleal space, and resulted in a shunt obstruction. Upward migration of distal shunt catheters has rarely been reported, but probably involves patient motion that creates a “windlass” effect. Cephaled migration requires a potential space (subgaleal or ventricular) and no resistance to movement of the tubing. This complication can be prevented by securing the shunt near the site of motion.

    Notes: Abstract Simple meningoceles are infrequent forms of dysraphism and are often benign. They have been associated with other spinal anomalies. The uncommon cervical meningocele may have a higher propensity to be associated with other spinal anomalies. Four patients with cervical meningocele are presented with radiographic evaluation and clinical course. Multiple abnormalities were documented radiographically and operatively, including hydrocephalus, Chiari malformation, hydromyelia, lipomeningomyelocele, tethered cord, thickened filum terminale, diastematomyelia, Klippel-Feil syndrome, and thoracic hemivertebrae. Prior to the development of any late neurological abnormality from associated spinal anomalies, magnetic resonance imaging is recommended early in a child born with a simple meningocele.
