    Notes: Summary A serum concentration profile study on midazolam in children was done. Fifty six children aged 3–10 years took part. The routes investigated were intravenous, intramuscular, rectal and oral at 0.15 mg·kg−1, and the oral at 0.45 mg·kg−1 and 1 mg·kg−1. Serum concentration levels for 5 h were studied using gas liquid chromatography. The volume of distribution, Vss, was 1.29 l·kg−1, the elimination half-life 1.17 h and the serum clearance 9.11 ml·kg−1·min−1. Peak serum concentrations for the intramuscular, rectal and oral routes were at 15 min, 30 min and 53 min respectively. Bioavailability was 87%, 18%, 27% respectively at a dose of 0.15 mg·kg−1. The oral route bioavailability halved to 15% at the two higher doses. Bioequivalence was present between the 0.15 mg·kg−1 intramuscular dose and the 0.45 mg·kg−1 oral dose from 45 to 120 min.

    Notes: Peri-operative plasma glucose levels were studied in one hundred children under 5 years of age and under 20 kg. All underwent procedures of less than 30 minutes. Of 32 cases under the third percentile for weight, ten developed low plasma glucose during the pre-operative andlor postoperative phase. This occurred despite receiving 5% dextrose by mouth 4 hours before induction. Even in children of normal weight there was a low incidence of peri-operative hypoglycaemia. Routine intravenous glucose-containing fluids are thus recommended during the peri-operative period.

    Notes: A constant pressure differential valve for the control of tracheal tube cuff pressure was tested under clinical conditions. Fifty-one patients underwent controlled ventilation and 20 patients were allowed to breathe spontaneously. Nitrous oxide 66% with oxygen 33% and halothane were used via a circle system. With controlled respiration at afresh gas flow of 3–10 l.min −1, the expiratory cuff pressures of 10.1–16 cmH2sO and the inspiratory cuff pressures of 23.4–32.4 cmH2O were below venous and arterial mucosal capillary perfusion pressures respectively. Cuff pressures were unaltered with time. Methylene blue instilled into the larynx did not appear in the trachea. Fifty-two control patients had the same incidence of sore throat (40%) and hoarseness (30%) at 24 h. With spontaneous ventilation, fresh gas flows of 5–15 l.min−1 maintained the cuff pressure above 10 cmH2O. We conclude that this valve prevents excessive tracheal cuff pressure while maintaining the airway seal.

    Notes: Summary We compared the cost-effectiveness of general anaesthetic agents in adult and paediatric day surgery populations. We randomly assigned 1063 adult and 322 paediatric elective patients to one of four (adult) or two (paediatric) anaesthesia groups. Total costs were calculated from individual patient resource use to 7 days post discharge. Incremental cost-effectiveness ratios were expressed as cost per episode of postoperative nausea and vomiting (PONV) avoided. In adults, variable secondary care costs were higher for propofol induction and propofol maintenance (propofol/propofol; p 〈 0.01) than other groups and lower in propofol induction and isoflurane maintenance (propofol/isoflurane; p 〈 0.01). In both studies, predischarge PONV was higher if sevoflurane/sevoflurane (p 〈 0.01) was used compared with use of propofol for induction. In both studies, there was no difference in postdischarge outcomes at Day 7. Sevoflurane/sevoflurane was more costly with higher PONV rates in both studies. In adults, the cost per extra episode of PONV avoided was £296 (propofol/propofol vs. propofol/ sevoflurane) and £333 (propofol/sevoflurane vs. propofol/isoflurane).

    Notes: SANS measurements were performed for the ternary microemulsion D2O-octane-[n-dodecylpenta(oxyethylene glycol) monoether (C12E5)] at several compositions: (1) as a function of C12E5 wt fraction γ at constant oil/water wt ratio α (α = 0.4, so the oil-to-water volume ratio is one); and (2) as a function of α (at γ = 0.07 wt fraction) through the main isotropic channel. A formalism recently developed by Teubner & Strey [J. Chem. Phys. (1987), 87, 3195–3200] was used to describe the scattering data. Some problems with the fitting of the data are discussed. The values obtained for d, the domain size, from these fits agree rather well with the conduit sizes measured by electron microscopy.

    Notes: The ir spectra of lathyritic rat skin collagen and calf skin gelatin solutions at a variety of temperatures were obtained using Fourier transform ir spectroscopy and a 9-reflection, 2-pass ZnSe prism sample cell. The spectra were then deconvolved (based on Kauppinnen's method) and the behavior of the amide I band at ∼ 1650 cm-1 observed in detail. Throughout the temperature range studied (4-50°C), three component absorption peaks within the amide I band (at 1633, 1643, and 1660 cm-1) are common to the spectra irrespective of the degree of triple helix content of the sample. Changes in the relative intensities of these component peaks are, however, conformationally dependent. During denaturation of the triple helix, the dominant 1660-cm-1 component in the native collagen spectrum diminishes and the 1633-cm-1 peak becomes relatively intensified. The inherently strong basicity of the carbonyl group of the proline residues together with the frequent occurrence of this imino acid in the X position of the Gly-X-Y triplet of collagen largely accounts for the -30-cm-1 shift of the amide I band during denaturation. Temperature and conformationally dependent changes in the fine structure of the amide I band from dilute solutions of collagen can be monitored in a reproducible and quantitative fashion.
