    Notes: In the present study we evaluated the allergenicity and immunogenicity of a whey partially hydrolysate formula (PHF) in a guinea pig model. Nine groups of 10 guinea pigs received either a conventional milk formula (CMF) or PHF for 37 days. After intravenous (i.v.) challenge with either α-lactoglobulin (β-L) or ultracentrifuged (u)CMF, animals fed CMF showed, respectively, 80% and 100®0 fatal reactions, whereas animals fed PHF showed no reactions when i.v. challenged both with (3-L and uPHF. Eighty percent fatal reactions, and 10%, respectively, of severe and mild reactions have been observed in the CMF-fed, casein-challenge group. In contrast, only one (10%) mild reaction occurred in the PHF-fed, casein-challenged group. When challenged with u-pasteurized cow's milk (uPCM), CMF-fed animals showed 86% fatal reactions and 14% mild reactions, whereas PHF-fed ones presented 70% no reactions, 20%, mild reactions, and one (10%) fatal reaction. Animals fed CMF showed a significantly higher level of specific IgG against cow's milk antigens than PHF-fed ones, which, however, presented higher levels of antibodies than a water-supplemented control group. The present results confirm that PHF is less sensitizing than CMF in a guinea pig model.

    Notes: Background:  Different types of textile are used for the preparation of covers for bed encasement. The aim of the present study was to evaluate different fabrics employed for mattress covers regarding their efficacy in blocking Der p 1 and Fel d 1 as well as their air permeability.Methods:  Eleven different commercially available fabrics manufactured for allergen avoidance have been tested and compared with regular cotton. Dust samples titered for Der p 1 and Fel d 1 were pulled through the different fabrics using a modified Fussnecker dust trap and collected by a filter located downstream. Airflow through the dust trap was controlled by a vacuum pump operating for 5 min and measured at the beginning (T0) and at the end (T1) of the test.Results:  All the tightly woven and laminated materials were able to control mite allergen permeability allowing air passage but they significantly differed in Fel d 1 permeability. Laminated tissues and laminated tissue not tissued were effective in controlling both the allergens but they did not allow air permeability.Conclusions:  Detailed knowledge about the actual properties of the products for bed encasement needs to be considered in order to optimize allergen avoidance, disease control and sleep comfort.

    Notes: Twenty-three Dermatophagoides pteronyssinus (Dpt)-sensitive asthmatic children aged 7–14 years entered a double-blind, placebo-controlled trial of standardized immunotherapy (IT) (Alpare) while resident at high altitude. Dpt sensitivity was evaluated by skin prick tests at different allergen concentrations at the enrollment and after 6 and 12 months of treatment. Bronchial hyperreactivity was evaluated at the same time points, and on each occasion, histamine challenge and, the following day, Dpt bronchial challenge were performed. All patients, irrespective of active treatment, improved clinically and in lung function with increased PC20 and Dpt-PD20. Alpare-treated patients had a significantly decreased sensitivity on Dpt skin testing (P ≤0.009) and felt that their asthma had improved (P ≤0.001) compared with placebo-treated subjects, but there was no difference between the treatment groups in lung function or bronchial challenge response. IT neither increased nor decreased bronchial histamine sensitivity. Our results indicate that Dpt IT benefits asthmatic children, but improvement by allergen avoidance at high altitude is even greater.

    Notes: Cysteinyl leukotrienes (cys-LTs), LTB4 and 8-isoprostane are increased in the exhaled breath condensate (EBC) from asthmatic patients. The aim of this study was to investigate whether the measurement of cys-LTs, LTB4 and 8-isoprostane in EBC can reflect the level of airway inflammation assessed by induced sputum in asthmatic children sensitized to house dust mite (HDM) during natural avoidance of HDM allergens. Twelve children were evaluated at the time of admission (T0) and after 3 months of stay (T1) at the Istituto Pio XII (Misurina, Italian Dolomites 1756  m). Sputum eosinophil percentage and measurement of cys-LTs, LTB4 and 8-isoprostanes in the breath condensate at T0 and T1 were evaluated. Eosinophil percentage in induced sputum was 8.5 ± 1.1% at T0 and 3.5 ± 0.4% at T1 (p = 0.011). Neutrophil percentage in sputum was 1.1 ± 0.5% at T0 and 1.5 ± 1.0% at T1 (ns). Cys-LTs mean level was 14.24 ± 4.53 pg/ml at T0 and 4.65 ± 0.68 pg/ml at T1 (p = 0.0125). LTB4 level was 2.36 ± 0.19 pg/ml at T0 and 2.41 ± 0.23 pg/ml at T1 (ns). 8-Isoprostane level reduced from 17.47 ± 3.18 pg/ml at T0 to 7.36 ± 3.26 pg/ml at T1 (p = 0.003). This study show that exhaled cys-LTs and 8-isoprostane, as well as eosinophil percentage in induced sputum, are reduced after allergen avoidance in asthmatic children suggesting a potential application of EBC for the non-invasive evaluation of airway inflammation in asthma in allergic asthmatic children.

    Notes: Variations of serum and nasal specific IgE to Dermatophagoides pteronyssinus (Der p) during alternate periods of antigen avoidance exposure have been evaluated with an open design in a group of allergic children with asthma and rhinitis at the residential house Istituto Pio XII (Misurina, BL, Italy), at 1756 m. in the Italian Dolomites. A method based on direct incubation of allergen coupled substrate on the nasal mucosa has been employed to evaluate the levels of nasal IgE. Serum specific IgE decreased respectively from (median) 117-89.3 kU/l (P 〈 0.001) during an initial period of 3 months of allergen avoidance and from 88.2 to 78.4 kU/l (P 〈 0.0002) during a subsequent period of allergen avoidance. No significant increase in serum specific IgE was, in contrast, observed during two periods, 22 and 9 days, of antigen exposure, changing respectively from 89.3 to 88.2 and from 78.4 to 89 1 kU/l. In contrast, nasal IgE has been significantly influenced by the alternate periods of antigen exposure-avoidance, showing a decrease from 19.75 to 4.01 kU/l (P 〈 0.0001) after the initial period of avoidance, followed by an increase to 9.95 kU/l (P 〈 0.0001) after 22 days of exposure. A significant decrease to a value of 2.37 kU/l (P 〈 0.0001) was also observed during the subsequent period of avoidance, followed again by an increase to 7.87 kU/l (P 〈 0.002) after 9 days of exposure. The evaluation of the kinetics of changes in nasal specific IgE revealed a significant decrease (P 〈 0.01) as soon as antigen avoidance was implemented for 3 days. Nasal specific TgE, therefore, appears to be a more sensitive index of antigen exposure avoidance than serum IgE levels.

    Notes: A cohort of 12 asthmatic children was followed over several months, during which they moved back and forth from an allergen-free to an allergen-rich environment at high and low altitude, respectively. The children were treated with non-steroidal anti-asthmatic drugs as clinically needed. Histamine PC20-FEV1 was unaltered during the study period, whereas serum levels of eosinophil cationic protein (ECP) and eosinophil protein X (EPX) showed significant changes when the children were exposed to the offending allergens. The total IgE significantly increased during exposure. The serum levels of myeloperoxidase (MPO) as well as of chemotactic factors for both neutrophils and eosinophils were unaltered during allergen exposure. We conclude that the serum markers of eosinophil activity ECP and EPX are sensitive indices of allergen exposure in asthmatic atopic children.

    Notes: Background We investigated the effects of prolonged allergen avoidance in 18 house dust mite-sensitized asthmatic children during a prolonged residential period at a high altitude, allergen-free environment.Methods Evaluations of residual volume (RV) and exhaled nitric oxide (eNO) were performed (i) at admission to the residential house in September, (ii) in December after 3 months of stay, (iii) in January after 15 days at home, exposed to allergens, and (iv) in June after 9 months of stay.Results During the study period RV showed a significant decrease in December (from 117.5 ± 7.7% to 96.5 ± 3.2%) (P 〈 0.02) and a following increase in January (126.2 ± 17.2%), after allergen re-exposure (P 〈 0.03). RV decreased again in June at the end of the study period (91.1 ± 6.0%) (P = 0.001). FEV1, FEF25−75 and VC values did not present significant variations. ENO showed a significant decrease in December after 3 months at high altitude (from 21.3 ± 3.9 p.p.b. to 11.9 ± 1.7 p.p.b.) (P = 0.03), but no further significant change. No correlation was found between lung volumes and eNO, probably reflecting different aspects of asthma.Conclusions Results suggest that RV may be more sensitive than other respiratory function parameters in identifying children with air trapping, being influenced significantly as the inflammatory indices by effective allergen avoidance/exposure regimen.
