    Notes: Summary The results of 16 cases of temporal bone resection were evaluated. The resections, 12 of which were total and 4 partial, had mostly been performed for squamous cell carcinoma invading the temporal bone. Five patients in whom the intervention was considered radical and not followed by irradiation, were alive and well 9 years or more later. The significance of postoperative radiotherapy was demonstrated in 3 patients who were alive and in good health 9 years or more later, although histological examination of resection margins showed residual tumour. Eight patients died 10 months to 5 1/3 years after operation, 6 of whom from recurrence, one from radiation necrosis of the temporal lobe of the brain, and one from cerebrovascular occlusion. Histologically, squamous cell carcinoma was found significantly more among the patients who died from recurrence than among those still living.

    Notes: The aim of this study is to evaluate the functional and cell biological applicability of a two-ply nerve guide constructed of a PLLA/PCL (i.e. poly-l-lactide and poly-ε-caprolactone) copolymer. To do so, we performed a cytotoxicity test, a subcutaneous biodegradation test and an in situ implantation study in the sciatic nerve of the rat. The nerve guide copolymer was found to be non-toxic, according to ISO/EN standards, and it showed a mild foreign body reaction and complete fibrous encapsulation after implantation. Onset of biodegradation of the inner layer was seen after one month of implantation. After 18 months of implantation complete fragmentation was observed, as well as a secondary inflammatory response characterized by foreign body giant cell activity and phagocytosis of polymer debris. Recovery of both motor and sensory nerve function was observed in all nerve guides.

    Notes: Abstract P(DLLA-ε-CL) nerve guides can be used perfectly for short nerve gaps in rats, and are even better than short autologous nerve grafts. The tube dimensions, such as the internal diameter and wall thickness, are very important for the final outcome of peripheral nerve regeneration, as well as the recovery of nerve function. Before using biodegradable nerve guides in patients, it will be necessary to control the swelling the biomaterial during degradation. © 1998 Kluwer Academic Publishers

    Notes: Summary A retrospective study of 11 patients with facial paralysis was undertaken. Correction of lagophthalmos was accomplished by inserting a dental gold weight into the upper eyelid. All weights were assessed and adjusted to fit the patient's individual need. The primary objective was to achieve adequate eyelid closure and corneal protection. The advantages and disadvantages we experienced with this procedure are discussed.

    Notes: Abstract  The posterior interosseous nerve (PIN) paralysis is characterized by weakness of the innervated muscles: the supinator muscle, the extensor muscles to the wrist, fingers and thumb (except the extensor carpi radialis longus) and the abductor pollicis longus muscle. Exploration of the nerve is recommended if there are no signs of spontaneous recovery after a period of observation of three to nine months. Another form of posterior interosseous nerve syndrome (PINS), also called the radial tunnel syndrome, presents with proximal forearm pain only. It is recommended to treat these patients conservatively at first. Only if conservative therapy does not relieve the symptoms of pain the PIN should be explored. A retrospective follow-up study of 14 patients with PINS was carried out, of which 12 patients had paresis or paralysis and two patients a pain syndrome. All, but one, were operated on. The results at follow-up will be discussed. It is recommended that the period of observation should be six to eight months for patients with PIN paresis, however, for patients with a full paralysis it is not possible to make any recommendation from this series. In contrast to the results reported in the literature, both our patients with pain as the only symptom obtained poor results at follow-up. For this group of patients the importance of the conservative treatment is emphasized.

    Notes: Abstract  Eleven patients (mean age 54 years, range 46–67 years) with osteoarthritis limited to the trapeziometacarpal-1 joint underwent a distal trapezium resection. In nine cases the palmaris longus tendon and in two cases a Swanson condylar implant was used as interposition material. The results were very disappointing. Only one patient was satisfied. Nine patients were reoperated upon (mean interval period 43 months, range 3–110) because of pain. In two cases, the cause was dislocation of the Swanson condylar implant. In the other seven cases, prominent bone on the trapezium, ulnar osteophytes, scaphotrapezial osteoarthritis, sclerosis of the trapezium remnant, and impingement were causal factors. In future, the selection of patients for this operation will be performed with even greater care. Despite these poor results, a definitive conclusion about the indication for this procedure can only be made after evaluation of an improved operative procedure.

    Notes: Abstract  The anterior interosseous nerve syndrome (AINS) is characterized by weakness of the flexor pollicis longus and the flexor digitorum profundus of the index finger. The orthopedic literature suggests that this syndrome is caused by mechanical compression of the nerve (an entrapment neuropathy) and that decompression should take place after an observational period of 6 to 12 weeks. The neurological literature suggests that AINS is a form of neuralgic amyotrophy and that it is safe to treat patients with AINS nonoperatively to obtain good results. With this controversy in mind, a retrospective follow-up study of 11 patients with this syndrome was conducted. Eight patients were operated on and three patients were treated conservatively. The question whether a patient should be operated on or not, cannot easily be answered. It is recommended that spontaneous recovery should be awaited in patients having other neurological symptoms in combination with AINS. In patients with symptoms caused by AINS only, the observation period should be 8 to 12 months.
