    Notes: Summary A pituitary adenoma in a 67-year-old man was characterized by abundant mitochondria and identified as an oncocytoma, which clinically and histologically appeared as a chromophobe adenoma. In addition to the numerous mitochondria within the neoplastic cells, structurally abnormal mitochondria were also present. Compared with other pituitary oncocytomas reported in the literature, abnormally structured mitochondria appear rare among the mitochondrial population of pituitary oncocytomas.

    Notes: Summary The most well known brain tumour models will be discussed. It is a series of animal experiments with an induced tumour and with heterologous and homologous transplantation. This work will deal especially with technical problems, which have not so far been satisfactorily resolved. With the aid of a stereotaxic operation method, the exactness of the technique of transplantation could be improved. The tumour contamination problem of the injection pathway as well as of the cerebral spinal fluid compartment is satisfactorily resolved. The authors have at their disposal a standardized cerebral glioma model in the rat in order to study the interstitial chemotherapy of brain tumours.

    Notes: Summary The pathogenesis of posttraumatic subdural hygroma still remains largely unknown. One of the suggested pathological mechanisms is the traumatic development of an arachnoid tear and the subsequent efflux of CSF into the subdural space. We performed a multifactorial analysis of the hygroma fluid obtained at operation in comparison to the simultaneously taken plasma and lumbar CSF. The results of the protein analysis support the CSF origin of the subdural hygroma fluid.

    Notes: Summary The clinical course of 11 patients with a subdural hygroma, who underwent neurosurgery, was studied. In the first week after the operation no improvement of the mental status could be detected. We considered therefore that operation hardly contributed to improvement. Thus the need to operate is limited to special cases such as acute or space-occupying subdural hygroma.

    Notes: Summary In an experimental glioma model, using ethylnitrosourea induced and subsequently intracerebrally implanted tumours in BD-IX rats, the effectiveness of intratumoural application of methotrexate (MTX) by stereotactic implantation of polymethylmethacrylate (PMMA) pellets containing MTX, has been studied. Tumour volume 10 days after pellet implantation as well as survival rates of treated, untreated and control animals have been the criteria of the effect of treatment. Tumour volume was significantly smaller in treated compared to untreated animals. The survival rate of untreated to treated animals increased 150 and 233% respectively when compared with the control animals. Thus a positive therapeutic effect of MTX-PMMA pellet implantation in the experimental glioma could be proven. Possible consequences for the treatment of human gliomas are shortly discussed.

    Notes: After many years of unsuccessful conservative treatment 16 patients suffering from hemicrania are relieved of their pain or are improved by operative treatment. Hemicranial attacks or permanent hemicrania is found to be caused by upper cervical nerve root compression. Vascular compression of C2 (n = 9) or scar tissue surrounding C2 (n = 1) or C3 (n = 1) is the pathology identified in cases of cervicogenic headache or “cluster headache-like” headache. Compression attributable to tumor, prolapsed disc, or spondylotic changes is found to be a cause of permanent headache. Only in those patients with permanent headache are radiological or electrophysiological findings helpful for diagnosis. In patients with hemicranial attacks and compression of nerve root C2 (n = 10) or C3 (n = 1), only vasoactive tests (provoking or relieving pain) or local anaesthesia prove to be helpful in diagnosing and localizing the origin of pain. The operation involves freeing the nerve roots from vascular compression. In two patients the C2 ganglion is resected. Thirteen patients subsequently become pain free. In three patients, hemicrania improves. Four of the 16 patients experience a recurrence of pain after the decompressive operation. After additional thermorhizotomy two patients have no further complaints and one patient has improved. One patient can tolerate his pain with occasional analgesics. The problem of referred pain into the fronto-ocular region is discussed.

    Notes: Abstract The effects of controlled moderate hyperventilation (paCo2∼31 mmHg) on cardiovascular parameters and whole-body-oxygen-uptake ( $$\dot VO_2 $$ )was studied in 10 patients with head injuries, who were unconscious and artificially ventilated at the time of the investigation. $$\dot VO_2 $$ was measured with the Beckmann Metabolic Measurement Cart and also calculated from cardiac output and arterio-mixed venous oxygen content difference. A mean increase in $$\dot VO_2 $$ (+19.6% of control) was observed after 2 h of hyperventilation. Cardiac output remained unchanged. This resulted in a decrease of venous oxygen content and impaired full oxygenation of the blood. Our results indicate that hyperventilation should not be a routine procedure. Close monitoring of arterial and venous blood gases is recommended, when hyperventilation is applied.
