    Notes: Zusammenfassung Der tuberkulöse Rundherd in der Lunge weist ein charakteristisches Schattenbild auf, das kaum mit einem anderen Gebilde verwechselt werden kann, wenn er einen scharf begrenzten, münzengroßen Rundschatten mit einem kalkdichten Zentrum zeigt. Er kann solitär oder multipel auftreten. Schwieriger und oft unmöglich ist seine Differenzierung, wenn er einen vollkommen homogenen Rundschatten bildet. Von den parasitären Lungenerkrankungen brauchen nur die beiden Arten des Echinococcus in Differentialdiagnose gezogen zu werden. Die Cysticerkose, die Pentastomiasis und die Porocephaliosis bilden kleine Fleckschatten, die als charakteristisches Merkmal entsprechend ihrem „Ernährungszapfen” eine nabelförmige Einziehung erkennen lassen.

    Notes: Abstract In this paper, a Working Group on Gastro-Oesophageal Reflux discusses recommendations for the first line diagnostic and therapeutic approach of gastro-oesophageal reflux disease in infants and children. All members of the Working Group agreed that infants with uncomplicated gastro-oesophageal reflux can be safely treated before performing (expensive and often unnecessary) complementary investigations. However, the latter are mandatory if symptoms persist despite appropriate treatment. Oesophageal pH monitoring of long duration (18–24 h) is recommended as the investigation technique of choice in infants and children with atypical presentations of gastro-oesophageal reflux. Upper gastro-intestinal endoscopy in a specialised centre is the technique of choice in infants and children presenting with symptoms suggestive of peptic oesophagitis. Prokinetics, still a relatively new drug family, have already obtained a definitive place in the treatment of gastro-oesophageal reflux disease in infants and children, especially if “non-drug” treatment (positional therapy, dietary recommendations, etc.) was unsuccessful. It was the aim of the Working Group to help the paediatrician with this consensus statement and guide-lines to establish a standardised management of gastro-oesophageal reflux disease in infants and children.

    Notes: Abstract Antiphospholipid antibodies, i.e. circulating auto-antibodies to negatively charged phospholipids, are mainly observed in patients with systemic lupus erythematosus, but may also occur in individuals who lack evidence of a well-defined systemic disease. Several studies have suggested that they may play a direct role in the pathogenesis of recurrent vascular thrombosis, repeated abortions, and thrombocytopenia (socalled antiphospholipid antibody syndrome), although the mechanism by which this occurs is still poorly understood. The management of patients with antiphospholipid antibody-related thrombosis includes anti-aggregants, anticoagulants, corticosteroids, and cytotoxic drugs, but the optimum treatment is still controversial. Here we review the test systems used to detect antiphospholipid antibodies and their significance in paediatrics.

    Notes: Abstract Takayasu arteritis is a rare chronic vasculitis primarily involving the aorta and its main branches. We report an adolescent girl with Takayasu arteritis who presented with an isolated aortic valve regurgitation as part of a systemic inflammatory process. This patient was initially misdiagnosed as having rheumatic heart disease and the correct diagnosis was made only 1 year later. Conclusion Takayasu arteritis should be considered among the diagnostic possibilities in patients who present with an unexplained systemic inflammatory syndrome and a cardiac murmur.

    Notes: Abstract A 5.5-year-old boy with Schönlein-Henoch purpura presented flank pain, macroscopic haematuria and voiding of blood clots. Radiologic examination showed a stenosing ureteritis. Treatment with prednisone was accompanied by resolution of the urologic manifestations. Ureteritis is a potentially serious complication of Schönlein-Henoch purpura and its incidence is probably under-estimated. Less than half of the reported patients have had symptoms suggesting a urologic complication. Microscopic examination of the urinary sediment can be helpful for the precocious diagnosis of ureteritis in the Schönlein-Henoch purpura.

    Notes: The degradation of chlorsulfuron was studied in laboratory experiments in three oxisols from south and south–east Brazil. Three soil profiles were sampled by horizon, and classified according to USDA soil taxonomy and the Brazilian system. Degradation assays were made to evaluate the influence of temperature, humidity and liming on chlorsulfuron decomposition. Further experiments were set up to study enhanced biodegradation. Abiotic degradation was also studied in sterile soils, to evaluate, by comparison with non–sterile soils, the role of microorganisms in degradation. The degradation always followed first–order kinetics and was generally faster in samples from A than B horizons. An increase in temperature (from 25 to 40°C) increased chlorsulfuron degradation. Further, an increase in moisture content increased chlorsulfuron degradation in samples from the A horizons of all soils, whereas for two out of three soils, degradation in samples from the B horizon was greater at lower water content. The biotic contribution to degradation was significant only for the soil with higher fertility. Soil liming significantly increased chlorsulfuron half–life in all samples. Significant enhancement of degradation (decrease in half–life on reapplication) was observed only in soil from A horizons, where a higher microbial activity was likely.

    Notes: Zusammenfassung Es wird das Röntgenbild eines alten verkalkten tuberkulösen Primärkomplexes gezeigt, bei dem der Erstherd in der rechten Lungenspitze und der dazugehörige Lymphknotenpol nicht im Hilus, sondern rechts paratracheal liegt. Das Bild gibt Anlaß zur Rekonstruktion des Infektionsweges, wobei die Ansicht vertreten wird, daß der Prozeß vom Lungenherd über die apikale Pleura zu den paratrachealen Lymphknoten gewandert ist und nicht umgekehrt.

    Notes: Abstract A child with myositis ossificans progressiva was treated for 8 years with ethane-1-hydroxy-1,1-diphosphonate (EHDP) 30–40 mg/kg per day. Latterly he complained of severe, progressive bone and joint pain which made standing and walking almost impossible. A radiographic skeletal survey showed diffuse ricket-like lesions. Withdrawal of EHDP therapy produced substantial improvement in his general condition as well as in the radiographic appearance of the bones. Multiple exostoses were observed in this case and, particularly those around the knees, presented a peculiar morphology. This supports the theory that exostoses originate from a defect of metaphyseal modelling.
