    Notes: Background: The authors recently assessed the incidence and determinants of immunologic sensitization to flour in apprentice pastry-makers. The aim of this work was to determine the incidence of work-related rhinoconjunctivitis (RC) symptoms and their determinants.Methods: For this 188/230 entrants (81.7%) were evaluated before starting exposure to flour, and again 10.8 and 16.8 months after. Questionnaires and skin prick testing to common and work-related allergens were administered at each visit. Bronchial responsiveness to methacholine was assessed at baseline in all subjects and in a subgroup at follow-up.Results: Thirty subjects (16.1%) reported new work-related RC symptoms (13.1 per 100 person–years); in three subjects (1.6%), these were accompanied by incident skin prick test reactivity to flour-derived allergens. Skin prick test reactivity to grass pollens (OR = 3.0, 95% CI, 1.3–6.7) and to pets (OR = 2.5, 95% CI, 1.1–5.9), persistent rhinitis (OR = 3.1, 95% CI, 1.1–8.4), seasonal RC (OR = 2.5, 95% CI, 1.1–5.5), RC on contact with pets (OR = 2.3, 95% CI, 1.03–5.0) and skin prick test reactivity to wheat flour (OR = 10.5, 95% CI, 2.3–46.8), assessed at baseline, were significantly associated with the incidence of work-related RC symptoms. Multivariate logistic regression analysis yielded significant OR of skin prick test reactivity to wheat flour at baseline (OR = 7.1, 95% CI, 1.7–35.1) and persistent rhinitis (OR = 3.9, 95% CI, 1.01–9.6) for the incidence of work-related RC symptoms. Increased bronchial hyperresponsiveness at follow-up was more frequent, although not significantly, in subjects positive to skin prick test to flour on entry and reporting new work-related symptoms (3/5), than in other subjects (4/17).Conclusions: The incidence of work-related RC symptoms among apprentice pastry-makers was high (16.1% 30/186), while a minority (3/30) also developed skin prick test reactivity to flour. Skin prick test reactivity to wheat flour and persistent allergic rhinitis on starting exposure to flour are significant determinants for the development of work-related RC symptoms.

    Notes: Abstract A small leg length inequality, either true or functional, can be implicated in the pathogenesis of numerous spinal disorders. The correction of a leg length inequality with the goal of treating a spinal pathology is often achieved with the use of a shoe lift. Little research has focused on the impact of this correction on the three-dimensional (3D) postural organisation. The goal of this study is to quantify in control subjects the 3D postural changes to the pelvis, trunk, scapular belt and head, induced by a shoe lift. The postural geometry of 20 female subjects (x – = 22, σ = 1.2) was evaluated using a motion analysis system for three randomised conditions: control, and right and left shoe lift. Acute postural adaptations were noted for all subjects, principally manifested through the tilt of the pelvis, asymmetric version of the left and right iliac bones, and a lateral shift of the pelvis and scapular belt. The difference in the version of the right and left iliac bones was positively associated with the pelvic tilt. Postural adaptations were noted to vary between subjects for rotation and postero-anterior shift of the pelvis and scapular belt. No notable differences between conditions were noted in the estimation of kyphosis and lordosis. The observed systematic and variable postural adaptations noted in the presence of a shoe lift reflects the unique constraints of the musculoskeletal system. This suggests that the global impact of a shoe lift on a patient’s posture should also be considered during treatment. This study provides a basis for comparison of future research involving pathological populations.

    Notes: Abstract Scoliosis, especially idiopathic scoliosis, is a complex three-dimensional deformity of the spine in which the vertebral deformities are known, cuneal deformation being the most commonly known deformity but not the only one. We report here data concerning these specific vertebral deformities in chickens. A pinealectomy was performed in a controlled series of animal experiments. This technique induces progressive scoliosis in more than 80% of chickens, with the advantage of being non-aggressive to the spine. Vertebrae included in major thoracolumbar curves were observed in 17 chickens (11 male, 6 female) and classified into three types of vertebral deformities. Vertebral deformity type 1 is characterized by three-dimensional corporeal torsion, which defines the horizontal disorientation of the curve. Vertebral deformities type 2 and 3 define lateral imbalance in the election plane of the curve. Radiological and anatomical data collected throughout the progression of the scoliosis indicate that there is a correlation between structural vertebral deformities and growth/ maturation patterns. We compare our results with those reported in literature concerning human idiopathic scoliosis and experimental animal scoliosis.

    Notes: Abstract The objective of this study was to quantify the relationship between gibbosity and spinal deformation expressed by the angle of Cobb before and during treatment with a brace for different classes of idiopathic scoliosis patients. As part of the standard treatment with the Dynamic Corrective Brace (SpineCor), 89 idiopathic scoliosis patients underwent an initial radiological examination and gibbosity measurement with a scoliometer wearing and not wearing the brace. The 89 patients were classified in relation to the apex of the scoliosis curves: thoracic (n = 29); thoracolumbar (n = 40); lumbar (n = 7) and double (n = 13). With the dynamic corrective brace, the patients showed a mean decrease of 8.3° for the major Cobb angle, and a mean decrease of 2.3° for their gibbosity. There was a significant positive relationship between gibbosity and Cobb angle with and without the brace for the thoracic and thoracolumbar curves. A linear regression analysis identified a small mean estimation error for the thoracic curves (7.4° no-brace; 2.7° with brace) and thoracolumbar curves (5.2° no-brace; 5.3° with brace), indicating a predictive potential of the scoliometer. The measure of gibbosity with the scoliometer provides a fairly reliable estimation of Cobb angle at the initial clinical examination of a scoliosis patient. However, when initial Cobb angle and gibbosity are considered, the measure of gibbosity when wearing a brace provides the clinician with a highly reliable estimation of the Cobb angle while in a brace. This relationship also exists for the follow-up with a brace, permitting a judgement of the patient’s evolution under the treatment with SpineCor.

    Description / Table of Contents: Résumé Des biopsies de ligaments supra-épineux (LSE), interépineux (LIE) et jaune (ligamentum flavum, LF) ont été prélevées chez 12 patients âgés de 17 à 77 ans. Ces prélèvements ont ensuite été examinés au microscope optique (MO), électronique à balayage (MEB) et à transmission (MET). La structure des ligaments normaux fut d'abord établie, puis une comparaison des ligaments normaux et pathologiques a été effectuée. Le LSE et le LIE sont composés surtout de collagène, alors que le LF contient des fibres élastiques. Dans les ligaments normaux, les fibrilles de collagène sont disposées en hélice et les fascicules présentent une ondulation régulière. Des filaments de protéoglycans ont été démontrés dans les trois types de ligaments. Ces protéoglycans s'attachent d'une façon régulière aux fibrilles de collagène pour former des liaisons interfibrillaires. Des changements dégénératifs ont été révélés dans la plupart des ligaments de patients présentant une hernie discale. A l'échelle du MO, ces altérations se manifestent par une métaplasie en fibrocartilage et par'un début de calcification. Au niveau ultrastructural, nous avons observé une transformation des fibroblastes en chondrocytes. Une accumulation et une désorganisation des protéoglycans ainsi que des nécroses cellulaires ont êté également rencontrées. Les conséquences de ces changements dégénératifs d'une part, sur les propriétés mécaniques et, d'autre part, sur les douleurs rachidiennes sont discutées.
    Notes: Summary Biopsies of supraspinous ligaments (SSL), interspinous ligaments (ISL) and ligamenta flava (LF), removed at operation from 12 patients aged from 17 to 77 years, were examined by light microscopy, scanning electron microscopy and transmission electron microscopy. The structure of normal ligaments was first established, and then a comparison between normal and pathological ligaments was made. It was found that the SSL and ISL were composed of collagen, while the LF contained predominantly elastic fibres. In normal ligaments, a helical arrangement of the collagen fibrils was found. The fascicles were characterized by a regular waviness. Proteoglycan filaments were demonstrated in the three types of ligaments, forming a regular interfibrillar linking between adjacent collagen fibrils. Degenerative changes were found in most of the ligaments obtained from patients with disc herniation. On light microscopy, metaplasia into fibrocartilage and calcification was commonly seen. Ultrastructurally, the flattened fibroblasts were replaced by chondrocytic cells, and an accumulation of proteoglycans with a random arrangement and a cellular necrosis was encountered. The relevance of these degenerative changes to the mechanical properties and to low back pain is discussed.

    Description / Table of Contents: Résumé   Les ligament croisés antérieurs de dix chiens ont été prélevés avec leurs insertions osseuses. Un fixateur externe a été utilisé pour garder le ligament à sa longueur de repos. Les ligaments ont été colorés au chlorure d’or puis divisés en tiers proximal, moyen et distal. Chaque section a ensuite été coupée de façon sériée à intervalle de 5 microns et montée sur lame pour étude au microscope. Le nombre moyen de récepteurs par tiers du ligament étais: proximal 67, moyen 43 et distal 18. Les résultats de l’analyse de variance ont démontrés une différence entre les moyennes de récepteurs de chaque tiers (P〈0.001). Le test statistique de Sheffé a montré que le tiers proximal contenait un plus grand nombre de récepteurs (S=3.8) alors qu’il n’y avait pas de différence significative entre les tiers moyens et distaux du ligament.
    Notes: Abstract  Ten canine anterior cruciate ligaments (ACLs) were harvested while preserving their bony attachments. Specimens were stained using a modified gold chloride technique, divided into thirds, and serially sliced at 0.5 microns. The slides were viewed to count the mechanoreceptors present. The average numbers of receptors found were: proximal 67, middle 43, and distal 18 (ANOVA: P〈0.001). The statistical test (Sheffé) revealed that the proximal third contained a greater mean number of receptors (S=3.8). No significant difference was found between the number of receptors in the middle and distal thirds (S=0.85).

    Notes: Summary Cellulolytic enzymes from a laboratory anaerobic digester fed municipal solid waste were examined with respect to pH and temperature. The pH optimum was pH 6.6, considerably lower than the pH range in which digesters are normally operated (pH 7.2–7.6). The optimum temperature was between 50 and 60°C, rather than the 35–37°C range in which most digesters are controlled.

    Notes: The objective of this project was to characterize soft tissue bonding to porous coated implants such as those that would be used for resection-reconstruction of osteosarcoma cases. We were interested in determining conditions which would provide both mechanical attachment of the implant to the surrounding tissue and produce a vascularized interface. In a bilateral canine implant model, both femoral midshafts were replaced with a porous coated cobalt-chrome segmental implant fabricated with average pore sizes of 300 μm or 900 μm. Twelve implants, six of each pore size, were used in six dogs. Two dogs were sacrificed at 2, 4, and 6 months after implantation. The soft tissue-implant interface was characterized mechanically with peel tests and histologically using light microscopy and immunohistochemistry. At all periods, a nonvascularized fibrous membrane surrounded the smaller pore size implants, without ingrowth or mechanical bonding. In contrast, a vascularized membrane developed within and around the larger pore size implants; the attachment strength increasing with implantation time. The vascularity increased in size and quantity with time. This study demonstrates the feasibility of obtaining vascularized soft tissue attachment to tumor replacement implants with appropriate porous coated implant design.
