    Notes: Background Inpatient dermatology has not been properly described in many countries. National differences might be important in the evaluation of its usefulness and the applicability of politics of health expenditure restrictions.Objective To describe inpatient activity and readmission rates in a dermatology department in Spain.Study design Cross-sectional prospective study in a single hospital.Setting Secondary care hospital of the National Health Service in Pontevedra (Spain).Methods From May 1997 to December 2000, all discharge sheets (1048) were included in the study, codified and described.Results Surgery was the reason for admission in 37% of the inpatients. The most frequent diagnosis were: neoplasm (36%), infection (15%), psoriasis (10%), other (10%), dermatitis (6%) and drug reaction (5%). Readmission rates were 1.8% within 30 days, and 12.5% within 1 year.Conclusions Inpatient dermatology is different in different countries. Compared with what has been described in the USA or UK, our data suggest an important surgical content of inpatient dermatology in Spain, not reported in those countries. Medical diagnoses also differ, consisting of more infections, and less psoriasis and dermatitis in our setting. Readmission rates are low when compared with previously published ones, a finding that supports a long-term benefit of hospitalization.

    Notes: A 48-year-old man, previously healthy and asymptomatic, showed a unique skin lesion located on the right buttock. The histopathological study of the lesion disclosed an adenocarcinoma metastatic to the skin. The primary tumour was found at the head of the pancreas, and was confirmed by biopsy. Six months after the beginning of treatment with Gemcitabine the patient is still asymptomatic, and the cutaneous lesion has disappeared. Carcinomas of the pancreas represent less than 5% of human malignant neoplasms, skin involvement is rare, and metastasis generally multiple and situated in the periumbilical area. To our knowledge, very few patients have been reported with a solitary cutaneous lesion disclosing a pancreatic adenocarcinoma. The atypical location, the absence of related symptoms or analytical disorders, and the good progress of the patient make this case especially interesting.

    Notes: Summary The presence of calcitonin gene-related peptide (CGRP) in the skin of pig snout and human fingertip was investigated using immunohistochemical techniques. CGRP immunoreactivity was found in Merkel cells and nerve fibres of both species. In pig snout skin, Merkel cells containing CGRP were seen forming clusters at the tips of rete ridge epidermis and in the external root sheath of sinus hair follicles (vibrissae). Human Merkel cells immunostained for CGRP were found isolated or forming small groups in the basal layer of glandular epidermal ridges. In all cases, immunoreactivity was more intense on the side of the Merkel cell facing the associated nerve terminal (which was never positive for CGRP). This part of the Merkel cell has the greatest density of dense-cored granules, suggesting that CGRP must be stored in these granules. Nerve, bundles containing CGRP-immunoreactive fibres were found at dermal and hypodermal level, and blood vessels were often surrounded by CGRP nerve fibres. In pig snout skin some nerve fibres containing CGRP penetrated the epidermis and terminated as free endings, and in the human fingertip a small number of CGRP-immunoreactive nerve fibres were seen in Meissner's corpuscles.

    Notes: Summary Using immunohistochemical methods, we have investigated the cellular distribution of prothymosin alpha (ProT) in adult rat testis. A policlonal antibody raised against thymosin alpha 1 conjugated to keyhole limpet hemocyanin was used. ProT immunoreactivity was observed in the cytoplasm and nucleus of spermatogonia and primary spermatocytes in initial phases of the first meiotic division, preleptotene, leptotene and zygotene. However, in pachytene phase they already showed a weak or negative staining. On the other hand, secondary spermatocytes, spermatids, spermatozoa and Sertoli cells were not stained. Based on this fact we suggest that ProT is present in the proliferative cycle in the final steps of G1 phase, throughout the S and G2 phases and in initial steps of the prophase.

    Notes: Summary The cellular distribution of prothymosin alpha (ProT) was studied in ovarian follicles of adult cycling rats. We found positive granulosa and theca cells throughout follicular maturation. When both ProT and proliferating cell nuclear antigen (PCNA) immunoreactivity was studied, we observed that both proteins were expressed in the same granulosa and theca cells, although sometimes ProT immunoreactivity was weak or absent in the mitotic (M) phase. Moreover, both peptides share the nuclear distribution, but ProT immunoreactivity was never seen in nucleoli. Therefore, we conclude that in mitotic cells ProT is expressed only in actively proliferating cells, since all ProT-positive cells were also positive for PCNA. ProT and PCNA immunoreactivities during the meiotic division were studied in oocytes. The presence of PCNA was, unlike ProT, constant throughout follicle development (except atretic oocytes). Oocytes expressed ProT from primordial follicles to the eighth generation, but more developed oocytes and atretic oocytes were not immunoreactive. In hypophysectomized rats, all oocytes were immunoreactive. Interestingly, in hypophysectomized rats treated with follicle stimulating hormone (FSH) that promoted follicle development, the more developed oocytes did not show ProT immunoreactivity. Since hypophysectomized rats were not treated with luteinizing hormone we conclude that ProT expression is not required to complete meiotic division I.

    Notes: The presence of serotonin in the Merkel cells of pig snout epidermis was investigated by the peroxidase-antiperoxidase immunohistochemical technique. Serotonin-like immunoreactive Merkel cells were found in groups located at the base of epidermal rete pegs and in the external root sheath of sinus hair follicles (vibrissae). Immunoreactivity was stronger on the basal side of the Merkel cells, where dense-cored granules are most numerous. Neither the nerve terminal associated with the Merkel cell nor the neighbouring epidermal cells were immunostained.These results are the first evidence of serotonin-like immunoreactivity in mammalian Merkel cells. The fact that immunoreactivity is strongest in those parts of the Merkel cells with the highest granule density suggests that in these cells serotonin is probably localized in the dense-cored granules.

    Notes: Background: Prothymosin alpha (ProT) is a polypeptide widely distributed in the organism and expressed by cell types with a high proliferative capacity. The aim of the current work was to investigate if ProT was localized in the progenitor compartment of the adrenal cortex which, following the cell migration theory, corresponds to the zona glomerulosa.Methods: We studied by immunohistochemical and in situ hybridization methods the distribution of ProT in rat and human adrenal cortex. Immunohistochemical techniques for the study of the proliferating cell nuclear antigen (PCNA) and incorporation of 5-bromo-2′-deoxyuridin during DNA synthesis were also done. Immunoelectron microscopic procedures were performed to determine the exact subcellular localization of ProT.Results: ProT was found in the zona glomerulosa cells, but not in the cells of the remaining cortical layers (zonae fasciculata and reticularis). Glomerulosa cells showed immunostaining for ProT only in the nuclei, excluding the nucleoli. Variability in immunostaining intensity was found between different glomerulosa cells. In situ hybridization of ProT mRNA confirmed that ProT synthesis in adrenal cortex occurs only in the zona glomerulosa. The results obtained for proliferating cell nuclear antigen and incorporation of 5-bromo-2′-deoxyuridin confirmed that adrenocortical proliferation occurs in the zona glomerculosa. Ultrastructural immunocytochemistry showed labelling for ProT over the euchromatin, but not on the heterochromatin aggregations nor the nucleoli.Conclusions: The results presented here: (1) support the migration theory for the adrenocortical cell renewal, (2) demonstrate that ProT is present in the nuclei of proliferating cells (being associated with euchromatin), and (3) suggest that the study of ProT expression would be useful in distinguishing cycling from resting cells. © 1994 Wiley-Liss, Inc.
