Notes: Summary The pharmacokinetic mechanism of the theophylline-terbutaline interaction has been studied. Sustained release theophylline 200–400 mg b.d. was given with placebo or terbutaline 2.5 mg t.d.s. to six adult asthmatic patients. Terbutaline decreased the serum trough theophylline levels from 8.1 to 7.3 µg/ml, improved daily the clinical score from 1.51 to 1.26 and increased the peak expiratory flow rate from 316 to 370 l/min. In a single dose study following the chronic therapy, it was shown that there was no change in the peak theophylline concentration or in the timing of the peak, but the t1/2 was reduced from 9.0 to 7.5 h, and the systemic clearance was increased from 20.2 to 24.8 ml·h−1·kg−1. Thus, terbutaline reduced the serum theophylline concentration by increasing its systemic clearance.

Notes: A combination of propranolol and hydrallazine was administered to 13 patients with longstanding hypertension during 15 pregnancies. Hydrallazine was continued through labour and delivery in all patients, while in eight patients propranolol was discontinued 2 to 15 days before delivery. Blood pressure control was uniformly good and superimposed pre-eclampsia did not occur during combined therapy. There were 14 livebirths and one unexplained stillbirth. Except for two cases of mild hypoglycemia, there were no neonatal complications.

Notes: Summary Four cases of rupture of the quadriceps tendon in patients on long-term haemodialysis and one in terminal renal failure are reported. Biopsy showed connective tissue elastosis which is known to develop in chronic acidosis. These changes may weaken the tendons.

Notes: Abstract Serum renin concentration (SRC) was determined in 97 hypertensive patients under basal conditions and during stimulation of renin secretion. Renin secretion was stimulated by upright posture and by either of the following means: (a) diet containing 20 mEq Na/24 hours for 3 days; (b) 300 mg i.v. diazoxide injections; (c) oral ingestion of 80 mg furosemide; (d) oral hydrochlorothiazide (HCT), 25 mg twice daily for 3 days. HCT was used in 6 patients previously treated with diazoxide and in 8 patients previously treated with furosemide. Using pairs of basal and stimulated SRC determinations, the patients could be classified as high, normal, or low renin hypertensives. HCT proved to be the most convenient stimulus as far as efficacy, reliability and the patients' tolerance were concerned, and compared well with sodium restriction.
