    Notes: The early experience at the University of Michigan with the Rashkind ASD occluder in three children with a secundum atrial septal defect (ASD) is reported. Successful transcatheter ASD closure was accomplished in one child. The procedure was not successful in the other two. In one, the ASD occluder was inadvertently implanted in the right atrium. The ASD was successfully closed in the other child, but the occluder device partially detached from the atrial septum 4 days later and required emergency surgical retrieval from the left atrium. Based on this early experience, revisions in the occluder device and the technique of its delivery are suggested that should improve the safety and efficacy of transcatheter ASD occlusion in the future.

    Notes: Necrotizing enterocolitis is common in preterm and low birth weight infants but rare beyond the neonatal period. Various infectious agents including bacterial and viral organisms have been implicated in its pathogenesis. Rotavirus has recently been shown to be associated with necrotizing enterocolitis. In this report we describe two infants with congenital cardiac disease who developed fulminating necrotizing enterocolitis after cardiac catheterization. Both infants were beyond the newborn period and developed enterocolitis during an epidemic of rotaviral infection. We postulate a relationship between rotavirus infection and the development of necrotizing enterocolitis in older infants with congenital heart disease after cardiac catheterization.

    Notes: Abstract The utility of ω-123I-hexadecanoic acid myocardial scintigraphy as a metabolic probe of cardiomyopathies was investigated. Sixteen patients with a variety of cardiomyopathies and myopathies that involve cardiac muscle and ten volunteers were imaged in the postabsorptive state in a 40° LAO projection after a standard dose of ω-123I-hexadecanoic acid. An elimination T1/2 was calculated from the left ventricular myocardial time-activity curve. An uptake index, corrected for chest wall attenuation, was also computed in 7 of 10 volunteers and 8 of 16 patients. Of the 16 patients, only 2 had distinctly abnormal ω-123I-hexadecanoic acid myocardial tracer kinetics. The first patient had a metabolic disorder of which cartine deficiency was one component. The second patient had endocardial fibroelastosis, a process which has been linked to disorders which deprive the myocardium of oxygen and energy. Therefore, the cardiomyopathy may have been caused by some abnormality of cardiac metabolism other than carnitine deficiency. Although of limited utility in the overall cardiomyopathic population, ω-123I-hexadecanoic acid myocardial scintigraphy should be further investigated as a screening test for carnitine deficiency and related metabolic abnormalities in patients at risk.

    Notes: Summary No data exist concerning the total sinoatrial conduction time (TSACT) in children that compare values determined by the atrial extrastimulation technique (TSACTS) with those generated by the atrial pacing method (TSACTN). In this study, TSACT in 55 patients, age 0.2–18.5, was measured using both techniques. TSACTN was performed at a mean 90% (TSACTN−90) (n=32) or a mean 95% (TSACTN−95 and (n=38) of sinus cycle length (SCL). When data generated during determination of TSACTN−90 and TSACTS were compared, SCL and recovery cycle length (REC) were similar for both techniques. Likewise, TSACTS (128±40 ms) and TSACTN−90 (126±74 ms) were not significantly different. Coefficient of correlation wasr=0.82,p〈0.001. Chi-square analysis demonstrated a strong association of normal and abnormal values between TSACTS and TSACTN−90. In contrast, when values generated during TSACTN−95 and TSACTS were compared, TSACTS exceeded TSACTN−95 (137±38 vs 105±58 ms;p〈0.001). Values for SCL and REC were similar while correlation between TSACT determined by the two techniques remained strong (r=0.82,p〈0.001). Despite a good correlation between TSACTN−90 and TSACTS, individual differences in magnitude and direction were noted between the two techniques. In summary, TSACTN−90 approximates TSACTS in children. TSACTN−90 is preferable to TSACTN−95, probably due to more complete sinus node capture during atrial pacing. However, the behavior of the sinus node in response to extrastimuli (single or train) precludes favoring one technique over the other. More precise evaluation of sinoatrial conduction will require direct recording of sinus node activity.

    Notes: Summary The lungs of ten newborn infants who died of hypoplastic left ventricle syndrome were studied by a morphometric technique that (1) determined the percentage wall thickness of injected pulmonary arteries, (2) determined the ratio between the number of alveoli per high-power field and the number of corresponding arteries, and (3) examined in detail the extension of medial smooth muscle to the vessels at the periphery of the acinus. The findings in the lung were related to the gross cardiac morphological changes and to echocardiographic and hemodynamic findings. The echocardiograms of eight neonates demonstrated small left ventricles. The aortic root was hypoplastic in seven and the left atrium was small in three of the eight. Pulmonary artery hypertension and elevation of the left atrial pressure were present in all infants in whom measurements were obtained. The mean percentage wall thickness of all vessels was greater in afflicted infants than in normal age-matched control subjects. There was a normal ratio between the number of alveoli per high-power field and the number of corresponding arteries, and all infants had extension of muscle to the peripheral vessels at the alveolar duct and alveolar wall levels. The pulmonary vascular abnormalities observed in the neonate with hypoplastic left ventricle syndrome may represent persistence of fetal vascular abnormalities associated with the abnormal fetal circulatory hemodynamics resulting from the malformation. These abnormalities may influence the success of surgery proposed for hypoplastic left ventricle syndrome.

    Notes: Summary The mental and motor development of 173 infants with congenital heart disease was assessed by means of the Bayley Scales of Infant Development and clinical neurological examinations. The relationship between age, sex, congestive heart failure, hypoxemia, hospitalization, and test results was evaluated. The presence of congestive heart failure was found to be significantly associated with both mental and motor developmental delay. Hypoxemia and hospitalization were associated with delayed motor development. Developmental delay could be recognized as early as 2 months of age.

    Notes: Summary To help children with congenital heart disease, and their parents, cope with the stress of hospitalization for cardiac catheterization, a videotape was developed entitled “Leonard Z. Lion Presents … Learning About Your Heart Catheterization.” Fifty-three patients with congenital heart disease, 3 to 12 years of age, were randomly assigned to experimental (N=31) or control (N=22) groups. Each viewed the catheterization videotape or an unrelated control videotape before cardiac catheterization. The child's knowledge, anxiety, and adjustment during and after hospitalization for catheterization were evaluated. The experimental group displayed a significantly greater knowledge of events and sensations experienced during hospitalization than controls (P〈.0001). Children in the experimental group tended to sleep more during the catheterization procedure (P=.05) and when awake to ask more questions (P=.02). After discharge, the experimental group also tended to demonstrate more independent behavior. We conclude that viewing this videotape on heart catheterization helped children with congenital heart disease to cope with the stress of hospitalization for the procedure.

    Notes: Summary In order to evaluate the preventive health care practices in children with heart disease, 499 families were surveyed in outpatient settings. Data were collected on 215 children with heart disease and 284 control children without known chronic illness. There was no significant difference between the groups in the type of primary physician utilized or frequency of visits to the primary care physician. Immunizations were incomplete in 32.7% of the children with heart disease compared to only 2.5% in the control group (P〈0.0001). Among the children over 3 years of age with heart disease, 29% had not received routine dental care within the past year compared with 23.4% in the controls (P=NS). The parents of children with heart disease were found to pay less money out-of-pocket for their child's health care than the parents of control children (P〈0.0001). The data suggest that important aspects of primary health care were neglected in a large group of children with heart disease and that cost was not a major cause for the inadequate preventive care delivery. An educational program directed at health care professionals and parents is proposed.

    Notes: Summary We report the case of a seven-month-old infant with a clinical diagnosis of patent ductus arteriosus whose two-dimensional and Doppler echocardiographic examinations were consistent with a coronary artery fistula. At angiography, a left circumflex coronary artery to right ventricle fistula was diagnosed. Echo-Doppler techniques for the noninvasive diagnosis of this lesion are discussed and the literature is reviewed.
