    Notes: L-Tryptophan (L-TP) has been used in migraine and other pain conditions. The mechanism underlying the analgesic effect is still partly undefined. In this study the effects of subchronic administration of L-5-hydroxy-tryptophan (L-5HTP) (with and without carbidopa) on plasma ß-endorphin (ß-EP) levels and subjective pain threshold and tolerance were investigated in seven healthy volunteers. To measure also an objective indicator for pain, the nociceptive flexion reflex threshold was studied. L-5HTP treatment with and without carbidopa administration increased ß-EP levels significantly (p 〈 0.05). L-5HTP plus carbidopa induced an increase in ß-EP significantly (p 〈 0.05) higher than that after L-5HTP alone. Neither subjective pain threshold and tolerance nor RIII threshold was modified by either treatment. Our data seem to point to the existence of a complex linkage between plasma opioid levels and pain perception.

    Notes: Sommario Scopo del lavoro è di discutere alcuni aspetti relativi all'inquadramento nosografico delle cosiddette cefalee sintomatiche o secondarie, con particolare riferimento alla Classificazione della International Headache Society (IHS), attualmente in corso di revisione. Le cefalee sintomatiche rappresentano infatti un gruppo eterogeneo di disordini e condizion talvolta di grande rilevanza clinica, come nel caso delle cefalee sintomatiche di patologia intracranica vascolare e non vascolare. Peraltro, rientrano invece in questo capitolo quadri clinici mal definiti (aspecifici) e con incerta relazione causa-effetto con i vari fattori ed eventi con i quali un quadro di cealea o di dolore cranio-facciale si associa. Esiste poi un'importanza speculativa per le cefalee secondarie, rappresentata dalla possibilità di considerare alcune forme quali modelli in vivo per approfondire i meccanismi patofisiologici alla base della produzione del dolore cefalico. Sulla base di queste considerazioni vengono discusse, rispettivamente, le cefalee di interesse neurochirurgico (come sintomi premonitori di patologia maggiore intracranica), le cefalee secondarie a variazioni degli ormoni sessuali nel corso della vita riproduttiva e nel caso della somministrazione di estroprogestinici, le cefalee post-traumatiche per il rilievo fisiopatologico che assumono nei confronti di alcune forme di emicrania e di cefalea cervicogenica.

    Notes: Summary Platelets and monoaminergic neurons share many morphological, biochemical, and pharmacological properties. In addition to its similarities to serotonergic nerve endings, the platelet has also been considered a good model for the study of the noradrenergic function, since it can selectively take up, store, and release norepinephrine (NE). In this study platelet and plasma levels of free catecholamines (CA) in 20 healthy subjects have been determined by HPLC with a reversed phase (C18) ionpairing system equipped with a coulometric detector. Direct correlation was observed between platelet and plasma levels of NE. A positive correlation was also found between the age of the sujects and both platelet and plasma NE levels. While showing that the peripheral noradrenergic hyperactivity of the elderly is also reflected in platelet NE content, this study also demonstrates that simultaneous assay of platelet and plasma CAs can be a useful tool for an integrated and more complete evaluation of sympathetic nervous system activity.
