    Notes: Introduction: The aim of this study was to investigate the early qualitative and quantitative structural changes in the left atrial wall after endocardial microwave ablation in patients with chronic atrial fibrillation (AF) undergoing mitral surgery. Methods and Results: Seven patients with chronic AF of for at least 6 months underwent surgical microwave energy ablation. Linear isolation of pulmonary veins was performed in all patients by microwave energy applications to the endocardial surface delivered by catheter at 65-W constant power for 45 seconds. Biopsies were obtained from a selected site (below the right lower pulmonary vein) of the left atrial posterior wall before and after the ablation procedure in all patients. Control tissues from the same sites were obtained at autopsy from patients with noncardiac causes of death. Light and electron microscopy was used to examine qualitative and quantitative changes in tissue morphology. Tissues after endocardial ablation procedure showed significantly increased loss of contractile material. Electron microscopy of atrial tissue demonstrated loss of profile of perinuclear and plasma membranes of myocytes, disruption of the endothelial cells of capillary vessels, and presence of macrophages. Conclusion: Lesions created by endocardial microwave energy ablation revealed a transmural effect on the left atrial wall without a significant reduction in thickness but a significant increase in the myolytic areas involving the entire cytosol and occlusion of the small intramyocardial vessels within the ablative lesion.

    Notes: 4-Amino-1-azadienes 1 react with α,β-unsaturated Fischer carbene complexes at -40°C to give stereoselectively a variety of substituted 3H-4,5-dihydroazepines 3; similarly, 1-hydroxy-1-azadienes (α,β-unsaturated oximes) 6 afforded the corresponding azepine derivatives 7. Chiral, nonracemic carbene complexes 11 gave azepines 12-13 (d.e. = 40-44%) upon reaction with oxime 6a; the major isomers were obtained in a diastereomerically and enantiomerically pure form (45-50% overall yield) after crystallization. An X-ray structure of 12a allowed assignment of the absolute stereochemistry. The acid hydrolysis of azepines synthesized provided racemic and enantiomerically pure 1,6-dicarbonyl compounds (±)-5, (±)-9, and (-)-14, as well as diol (-)-15. The mechanism of the reaction of 1 and 2 was investigated by multinuclear (1H, 13C, 15N, and 183W) NMR characterization of four intermediates (A, B, C, and D) at low temperature. The experimental sequence of events involves: i) 1,2-nucleophilic addition of the unsubstituted imine nitrogen of 1 to the metal carbene function (zwitterion A, -60°C), ii) cyclization to the seven-membered ring with 1,2-migration of the pentacarbonyl metal (zwitterion B, -40°C), iii) reductive elimination and coordination of the metal to the amine nitrogen (intermediate C, -40°C), and iv) thermal decomplexation and tautomerization (intermediate D and compound 3, above -20°C).

    Notes: -Reversibly self-assembling host-guest complexes of the softball type are characterized by electrospray ionization mass spectrometry. Quaternary ammonium ions serve simultaneously as guests and ion labels. Isotope pattern analysis, inclusion of labeled guests, heterodimer experiments, size and shape dependence, and collision-induced fragmentation reveal that the capsular structure of these hydrogen-bonded complexes is retained in the gas phase with the guests inside their cavity. The results parallel findings from solution-phase NMR experiments and show that the guests are encapsulated inside the cavity. These results also demonstrate the value of mass spectrometry for high-order structural examination of supramolecular architecture.
