    Notes: Summary Prolonged erections (pharmacologically induced erections over 6 hours) are the most inconvenient side effects encountered in the diagnosis and therapy of erectile dysfunction with intracavernous injection of vasoactive drugs. Out of a total of 29 prolonged erections, 26 were seen after diagnostic use in 300 patients and 3 after therapeutic application in more than 4800 protocolled autoinjection of a standardized vasoactive solution (papaverine hydrochloride 15 mg/ml and phentolamine mesylate 0.5 mg/ml). In 25 of these 29 prolonged erections intracavernous injections of the alpha-receptor stimulant metaraminol was done and led to flaccidity in all cases. Another patient with a papaverine-induced erection lasting 26 h did not respond to intracavernous metaraminol and required puncture and aspiration; he lost spontaneous erectability and cavernosal response to vasoactive drugs. Metaraminol was also injected intracavernosally in a further 32 patients with pharmacologically induced full erections: flaccidity was obtained in all patients.

    Description / Table of Contents: Summary. In a 39-year-old woman an anticonvulsant therapy was initiated because of focal attacks in the left arm and face. The patient experienced generalized maculopapular skin rashes in response to each of four chemically similar anticonvulsant drugs: phenytoin, carbamazepine, primidone and clonazepam. During administration of carbamazepine the clinical features included fever, hepatitis and haemotological eosinophilia in addition to the skin rash (anticonvulsant hypersensitivity syndrome). The anticonvulsant hypersensitivity syndrome is defined as an idiosyncratic reaction caused by disturbed drug metabolism. Positive lymphocytetransformation tests with carbamazepine and phenytoin indicate an immunological mechanism underlying the rashes in our patient. Patch testing with the four anticonvulsant drugs gave positive results only with carbamazepine. Skin biopsy showed the histological features of a delayed-type allergy. The anticonvulsant therapy was continued with a chemically unrelated preparation, valproic acid; this drug is well tolerated and has proved appropriate for prevention of seizures.
    Notes: Zusammenfassung. Bei einer 39jährigen Patientin wurde wegen fokaler Anfälle im Bereich des linken Armes und der linken Gesichtshälfte eine antikonvulsive Therapie eingeleitet. Dabei traten auf 4 chemisch verwandte antikonvulsive Pharmaka jeweils generalisierte makulopapulöse Exantheme auf: Phenytoin (Zentropil Tbl.), Carbamazepin (Sirtal Tbl. und Tegretal Tbl.), Primidon (Mylepsinum Tbl.) und Chlonazepam (Rivotril Tbl.). Unter der Einnahme von Carbamazepin war das Exanthem verbunden mit Fieber, Leberentzündung und Bluteosinophilie (sog. Antikonvulsiva-Hypersensitivitäts-Syndrom). Das Antikonvulsiva-Hypersensitivitäts-Syndrom wird als Idiosynkrasie-Reaktion bei gestörtem Arzneimittelmetabolismus aufgefaßt. Der positive Lymphozyten-Transformations-Test (LTT) auf die untersuchten Pharmaka Carbamazepin und Phenytoin weist auf einen immunologischen Prozeß bei den Exanthemen hin. In der Epikutantestung mit den 4 Antikonvulsiva trat nur auf Carbamazepin eine positive Reaktion auf, in der Probebiopsie ließ sich histologisch eine Spättyp-Reaktion nachweisen. Die antikonvulsive Therapie mit der chemisch nicht verwandten Valproinsäure (Ergenyl 300, 4×1 Tbl./d) wird reaktionslos vertragen und erweist sich als effektiv in der Anfallsprophylaxe.
