Notes: Summary Patients undergoing breast reconstruction often ask the plastic surgeon which is the surgical procedure that gives the best results. In order to answer this question, the results of three different techniques of breast reconstruction in 62 cases were compared and evaluated. In 37 cases, the reconstruction was performed by tissue expansion followed by insertion of silicone prostheses (EXP), in 9 cases by a pedicled myocutaneous latissimus dorsi flap with prostheses (LDF), in 16 cases by a microvascular rectus abdominis musculocutaneous flap. Both the subjective (patients' satisfaction and psychological attitude) and the objective data were taken into consideration to evaluate the results. The TRAM results were the best, followed by the EXP technique and then by the LDF technique. The number of immediate and delayed complications was approximately the same using all three techniques, but TRAM is a little safer. Although tissue expansion is the simplest and quickest technique which may be best for some patients, the use of autologous tissue (TRAM) is recommended.

Notes: Abstract The time course of serum transaminases (alanine aminotransferase and aspartate aminotransferase) has been followed in 30 selected consecutive patients presenting burn sizes ranging from 10% to 95% of the total body surface (mean 43.13) and a survival index from 0.99 to 0.00 (mean 0.59). The results show that in all the patients both transaminases change in parallel, increasing in 18 patients (60%). In nearly all patients both enzymes increase during the second week after injury and aspartate aminotransferase increases later than alanine aminotransferase. The higher transaminase levels are noted in moderately ill patients. No clear correlation between the overall increase of transaminases and the extent of burned surface area has been found. We conclude that functional liver alterations mostly contribute to the increase of serum transaminases in burns.

Notes: Abstract Drug plasma levels of N-desmethyldiazepam (10 and 20 mg/day given at night) and amylobarbitone sodium (200 mg/day given at night) were determined in 45 patients after 1 and 7 days of treatment together with estimates of hypnotic and residual effects. No relationships were found between clinical ratings of hypnotic effect and drug plasma levels, while significant correlations were observed between some performance tasks and drug plasma concentrations of amylobarbitone sodium, and of the higher dose of N-desmethyldiazepam.
