Description / Table of Contents: Summary The distribution of the cytochemical activity of the adenosine triphosphatase in the cells of blood and bone marrow of 266 patients have been examined. A preferable smears method has been developed and described. Adenosine triphosphate is hydrolyzed predominantly in the eosinophils and plasmacells, and is exhibited by 62.4% of the total cells of the latter. In contrast to this the cells of the plasmocytom patients showed a low (52 patients having in 3.6% of their cells) or very low (11 patients in 1%, 5 patients in 2%, 2 patients in 4% and the rest of the patients in 6, 7, 8, 11, 16 and 17% of the cells respectively) or even no activity at all (25 patients). Only 3 cases of plasmocytom showed a high activity (in 21, 23 and 51% of the plasma cells). This enzyme was always present in more than 20% of the cells of non-myelom patients. The diagnostic value of the adenosine triphosphatase reaction for a plasmocytom is almost the same as that of the alkaline leukocyte phosphatase for a granulocytic leukemia.

Description / Table of Contents: Summary You find activity of adenosinetriphosphatase in the neutrophilic and eosinophilic granulocytes, monocytes and reticulum cells of the smears of bone marrow and blood. The reaction is of importance for the diagnosis of myeloma. Only 3.6% of the neoplastic plasma cells show activity (25 × 0, 11 × 1, 5 × 2, 2 × 4, 1 × 6, 7, 8, 11, 16, 17, 21, 23 and 51%), while 62.4% of the plasma cells of patients without myeloma are positive.

Description / Table of Contents: Summary With 23 serums (5 normals, 5 hypoglobulinemias, 6 myelomas, 4 typical, and 3 atypical macroglobulinemias) a comparative analysis was performed using the techniques of gel filtration (Sephadex G 200), ultracentrifugation, and paper electrophoresis. The respective results were brought into correlation and the question was investigated whether gel filtration can replace sedimentation analysis. While gel filtration was found to give satisfying results as “screening method” in cases of possible macroglobulinemia, it appeared not suited to differentiate between different types of macroglobulinemias; this task remains reserved to sedimentation analysis. The present investigation finally discusses details of the gel filtration method laying emphasis on the correction of its results by means of the specific extinctions of the Sephadex main fractions.

Notes: Abstract. Renal cell carcinoma in a horseshoe kidney is an unusual entity. To our knowledge, only 123 cases have been published to date. We report the first bilateral case of two clear-cell carcinomas in an asymmetrically fused kidney. Optimum preservation of renal function after radical tumor removal requires accurate preoperative imaging. Since the vascular supply in fusion anomalies is extremely variable, angiography is mandatory. Magnetic resonance imaging was most suitable to predict the tumor extent and localization, because it simultaneously gave the most comprehensive anatomical overview of the malformation.

Notes: Abstract. Ureteral obstruction is an infrequent complication after renal transplantation that may cause rapid loss of transplant function. We tested static fluid MR urography for determining the cause of graft hydronephrosis. Magnetic resonance urography was performed in nine transplants with dilated collecting systems on ultrasound. A heavily T2-weighted 3D turbo spin-echo sequence on a 1.5-T scanner was used and maximum intensity projections were obtained. The patients also underwent excretory urography (n = 1), renal scintigraphy (n = 1), antegrade pyelography (n = 3), voiding cystourethrography (n = 4), and non-enhanced CT (n = 2). Six patients had pathologic conditions including ureteral stricture, compression by lymphoceles, implantation stenosis, vesicoureteral reflux, and late-occurring transitional cell carcinoma at the implantation site. Static MRU was able to diagnose or exclude a dilation of the graft collecting system. It visualized the course of the ureters and localized the obstruction site in four of five obstructed transplants. In one case the ureter was obscured by lymphoceles, which were demonstrated by hydrographic MRU as well. The definite cause for obstruction was provided in only 2 of 5 cases. Dilation due to vesicoureteral reflux could not be differentiated. The current multimodality approach to renal transplant imaging already provides comprehensive assessment of graft hydronephrosis. Static MRU may be useful in some cases since complications associated with intravenous iodinated contrast or antegrade pyelography can be avoided. Its main drawback, the lack of functional information, may be overcome by combining it with contrast-enhanced MRU.
