    Notes: Summary Three technical modifications have been introduced into the Dornier HM-3 lithotriptor at the Marienhospital Herne: a low-pressure generator, which reduces the energy at the second focus by 30%, a modified semi-ellipsoid with an increased aperture and altered geometric dimensions, and the twin-pulse technique, releasing two impulses synchronized with a single R-wave. The treatment standard achieved with the conventional HM-3 device could be maintained, with a radiological success rate for stone disintegration of 95%. Procedural time could be reduced. The mean number of impulses and rate of second treatments increased, mainly due to the increasing proportion of ureteral and branched calculi. Cutaneous petechiae were rarely observed, suggesting less tissue damage with the lower pressures used. The profound reduction in pain perception with these technical modifications led to a new anesthesia technique. A combination of an oral anxiolytic and oral analgesic was effective in 94%. The results suggest that patient comfort and safety during ESWL treatment have been considerably improved.

    Description / Table of Contents: Zusammenfassung Mit dem Ziel, neue Voraussetzungen für experimentelle, histologische und biologische Untersuchungen der menschlichen Prostata zu schaffen, haben wir humanes Prostataadenomgewebe in die Oberschenkelmuskulatur neonataler Ratten verpflanzt. Die notwendige Immunsuppression wurde durch subcutane Injektion von Antilymphocytenserum erzielt. 3 Wochen nach der Heterotransplantation entsprach das histologische Bild eines Teils des implantierten Prostataadenomgewebes weitgehend dem Originalgewebe. Ebenso verhielten sich histochemischer Reaktionsausfall und Verteilungsmuster der sauren Phosphatase, der unspezifischen Esterase und der Succinodehydrogenase. Daneben wurden metaplastische Drüsenstrukturen angetroffen, die durch einen Aktivitätsabfall der untersuchten Enzyme gekennzeichnet waren.
    Notes: Summary Under the effect of antilymphocytic serum as an immunsuppressive regime it was possible to transplant human adenomatous prostatic grafts in neonatal rats. On the one hand the histological and histochemical examination of the grafts proved the maintenance of vitality and integrity of the prostatic tissue up to 3 weeks after heterologous transplantation. On the other hand metaplastic changes in the glandular epithelium accompanied with diminished enzymatic activity was also observed in a part of the grafted tissue.

    Notes: Summary The capacity of Leydig cells for regeneration was investigated in 12 patients with prostatic carcinoma, who underwent subcapsular orchidectomy, and in rats after testicular necrosis produced by cadmium chloride. In rats, reappearance of Leydig cells originating from the tunica albuginea could be demonstrated by histology. Testosterone concentrations increased parallel to regeneration of Leydig cells, while LH concentrations declined. In contrast to these findings, no rise of testosterone concentrations could be observed in patients up to 8 months after subcapsular orchidectomy. Human Leydig cells seem to have no capacity for regeneration, or endocrine function, despite the fact that some of these cells, which are present morphologically in the tunica albuginea or spermatic cord, remained.

    Notes: Abstract The aim of the present paper is to report on the frequency of pyelocalyceal diverticula and the relation to other renal abnormalities in 5000 routine IVPs. Pyelocalyceal diverticula are mostly occasional findings and they usually cause symptoms when complicated by inflammation or stones. These complications will characterize the symptoms. There is an indication for surgical treatment when the diverticula are complicated by stones or inflammation which fail to respond to simple antibiotic treatment. According to our experience, the removal of the diverticulum bearing renal segment is a safe procedure with good postoperative results.

    Notes: Summary The antiandrogen, cyproterone acetate (CPA), prevents development of prostatic hyperplasia, induced in castrated dogs by a 6 month-treatment with 5α-androstane-3α,l 7β-diol (A)alone or in combination with 17β-oestradiol (E 2). The immunoperoxidase technique was used to study functional cell types in the pars distalis of the pituitary gland and to detect growth hormone (GH) and prolactin (PRL) target sites in the prostate gland. Homologous radioimmunoassays for estimation of serum canine GH and PRL concentrations were also performed. Treatment with the combinations A + E 2 and A + E 2 + CPA resulted in morphological indications of stimulated GH and PRL cells and depressed gonadotrophs. This correlates well with an increase in PRL-dependent staining in glandular epithelium and fibromuscular tissue of the prostate gland. However, basal serum PRL and GH levels were not significantly affected. Treatment with A and A + E2 stimulated, while additional treatment with CPA clearly suppressed adrenocorticotrophin/melanotrophin (ACTH/MSH) cells. These findings indicate that an endocrine imbalance in hypothalamic-pituitary-adrenal function may be involved in induction and prevention of prostatic hyperplasia in the dog.
