    Notizen: Objective To examine the association between fetal outcome and the steady state haematology of mothers with homozygous sickle cell disease.Design A retrospective observational study. The data were taken from dockets kept at the Sickle Cell Clinic and verified by interview with 45% of the patients.Setting The Sickle Cell Clinic at the University Hospital of the West Indies or two peripheral clinics operated by the staff of the MRC Laboratories.Subjects All women aged 14 years or older with homozygous sickle cell disease who had experienced at least one pregnancy in the period 1977 to 1986.Main outcome measures Three fetal outcomes including miscarriages, perinatal deaths, and birthweight.Results There were 270 singleton pregnancies in 175 women with an overall fetal wastage of 32.2%. There was a significant increased risk of perinatal death with low maternal fetal haemoglobin level, but there were no haematological associations with miscarriages or birthweight.Conclusions These data suggest that maternal steady-state haematology has little influence on fetal outcome, with the exception that mothers with high HbF levels are less prone to perinatal deaths. Further study is required to investigate acute haematological changes associated with pregnancy.

    Notizen: Objective  To compare pregnancy outcome in sickle cell-haemoglobin C (SC) disease with that in homozygous sickle cell (SS) disease and age-matched controls with a normal haemoglobin (AA) genotype.Design  A cohort study followed from birth.Setting  Sickle Cell Clinic, University Hospital and other Jamaican hospitals.Population  Ninety-five pregnancies in 43 patients with SC disease, 94 pregnancies in 52 patients with SS disease and 157 pregnancies in 68 controls.Methods  Systematic review of all pregnancies occurring in sample population. Kaplan–Meier analysis for interval to first pregnancy, and the t test, χ2 test or Fisher's exact test as appropriate; correction was made for multiple testing and multiple linear regression was used for analysis of determinants of birthweight.Main outcome measures  Age at menarche, interval to first pregnancy, outcome of pregnancy, maternal complications and possible predictors of low birthweight.Results  Menarche was marginally delayed in SC disease compared with AA controls (median age 13.7 vs 13.0 years, P= 0.02) but age at first pregnancy was similar (median age 22.5 vs 20.1 years, P= 0.32). Pregnancy outcome in SC disease did not differ from AA controls but compared with SS disease there were marginally fewer miscarriages, more live deliveries and greater birthweight. The prevalence of pregnancy-induced hypertension, pre-eclampsia, antepartum or postpartum haemorrhage in SC disease did not differ from AA controls but the prevalence of sickle-related complications was similar to SS disease.Conclusions  Contrary to some claims, pregnancy outcome in SC disease is generally benign compared with SS disease.

    Notizen: Abstract Forearm blood flow (FBF) was measured in 23 patients with homozygous sickle cell (SS) disease and 20 controls with normal hemoglobin genotype (AA) using venous occlusion air plethysmography to examine the pattern of peripheral blood flow in both groups at rest, and in response to thermal stimuli. The mean resting FBF in SS patients was more than twice that in AA controls (P〈0.001). Indirect cooling (15°C) of the left forearm for 2 minutes evoked a significant increase (P〈0.005) in FBF in the opposite arm in both SS patients and AA controls, with the response being greater in the controls. Indirect heating (40°C) for 2 minutes evoked a decrease in FBF of a similar magnitude in both groups. The increased FBF in response to moderate cooling in SS patients may be part of an important reflex mechanism which could support the concept of the ‘steal’ syndrome, as recently postulated, as a possible explanation for the cold-related initiation of the painful crisis of SS disease.
