    Notes: Abstract Lipoblastomatosis is a rare benign tumor of mesenchymal origin that is seen primarily in the pediatric age group. Its usual location is in the head and extremities, and its presence in other regions may create diagnostic controversy. We present an infant with a perineal mass initially diagnosed as a perianal abscess. Following incision and drainage, two additional surgical interventions were necessary for radical excision of the tumor, which is known for its tendency to recur.

    Notes: Abstract Respiratory distress due to masses in the posterior mediastinum have been extensively described, including duplication cysts of the esophagus. Retained esophagus following failed repair of esophageal atresia (EA) and esophageal replacement was described only once. Double atresia and midportion esophageal cyst is also a rare entity. We describe a baby with respiratory distress following esophageal replacement for failed repair of EA with tracheoesophageal fistula who was cured by the removal of an overlooked mediastinal cystic mass of esophageal origin at the age of 16 months.

    Notes: Abstract Eighty children undergoing morning-session elective herniorrhaphy or orchidopexy were divided into two groups according to age (3–51/2 years and 57/12–12 years). Each group was again subdivided according to the presence or absence of either psychological or “pharmacological” preparation (the child falls asleep in the arms of the mother), yielding four subgroups: (1) psychological (Ps +); (2) pharmacological (Ph +); (3) (Ps + Ph +); and (4) (Ps−Ph−). At the beginning of anesthesia blood was drawn for cortisol level measurement. One month after surgery the children's adjustment scores were evaluated statistically. The cortisol level of the younger age group was lower than that of the older age group (P 〈 0.01) and that of children who received “parmacological” preparation was lower than in those who did not (P 〈 0.05). Concerning posthospital behavioral disturbances: the older age group benefited more from psychological preparation (Ps +), and its absence was detrimental to the mental health of this population (P 〈 0.05). The younger age group benefited more from induction of anesthesia in the presence of parents (Ph +), and its absence entailed risks to their mental health (P 〈 0.05).

No port of entry was definitely identified. The sepsis and myonecrotic process were controlled by resuscitative measures, extensive multisession debribement, and hyperbaric treatment.

    Notes: Abstract An isolated bowel segment (IBS) is created by a two-stage procedure: (1) initial coaptation of the bowel segment to host organs such as abdominal wall muscle, liver, or intestine; and (2) division of its mesentery several weeks later. Between operations, vascular collaterals develop across the coaptation to preserve viability of the IBS. IBS motility and absorption have been observed in previous studies; this experimental study was designed to angio-graphically observe the vascular collaterals. An IBS was created in four dogs by myoenteropexy between the undersurface of the rectus abdominis muscle and a segment of jejunum or colon. Both IBS ends were exteriorized and intestinal continuity was restored. Four weeks later, the IBS mesentery was divided in three dogs and a sham operation was performed in the fourth. Three weeks thereafter, the animals were killed and appropriate arteries related to the IBS were injected with Microfil radiopaque silicone yellow rubber compound at a pressure of 150 mmHg. After the dye had polymerized, soft tissue X-ray was employed to identify vascular collaterals to the IBS. With ipsilateral deep inferior epigastric artery injection in the dogs with IBS mesentery division, the intramural vessels of the IBS filled via collaterals over the myoenteropexy. Dye injected into the IBS mesenteric artery in the fourth dog outlined the abdominal wall vessels via collaterals that had developed across the myoenteropexy. These observations suggest that the IBS is nourished by the abdominal wall vessels through vascular collaterals that have developed across the myoenteropexy during the interval between the staged procedures, and that viability of the IBS is preserved after IBS mesenteric division.
