Notizen: The first case of Hemicrania continua in Greece is reported; a female of 47 years with a 17-year history of right-sided headache. For the last year, she had a moderate pain on the left side (amounting to 10–15% of the right-sided one and occurring only occasionally, i.e. when the right-sided pain was severe). After 17 years the pain still had a remitting course with pain periods of around three weeks interrupted by pain-free periods of approximately ten days. The indomethacin effect is complete.

Notizen: Another case of hemicrania continua is described-that of a 54-year-old woman, who for the past 3 years has had continuous, left-sided headache. Before this period she had non-continuous headache, otherwise resembling the present one, for 3–4 years. In the chronic stage, the headache fluctuates considerably. Indomethacin in a dosage of 150 mgday abolishes the headache completely. In hemicrania continua, there may thus, as in chronic paroxysmal hemicrania, be a non-chronic stage, antedating the chronic one. The relative importance of the two stages cannot be assessed at present.

Notizen: Evaporimeter measurements of forehead sweating during attack were carried out in six female patients with chronic paroxysmal hemicrania. The patients served as their own controls. Increased sweating on the symptomatic side was found in two patients. both when compared to the non-symptomatic side during attack and when compared with the symptomatic side outside attack. The remaining four patients, two of whom had only weak attacks at the time of study, showed sweat values within the control range. In one patient, attacks could be precipitated by head flexion and by pressure against certain circumscribed points in the neck (“mechanical” precipitation of attacks). These attacks were also associated with increased sweating on the symptomatic side. Increased forehead sweating does not seem to be a sine qua non in CPH.

Notizen: The forehead sweating function has been assessed in SUNCT syndrome-a short-lasting, unilateral, neuralgifonn headache syndrome with autonomic phenomena on the symptomatic side (conjunctival injection, lacrimation, etc.). In the three patients (of a total of six) who could be studied during paroxysms, increased evaporation was present on the symptomatic side of the forehead compared to the non-symptomatic side during attacks or to the symptomatic side between attacks. Basal sweating was generally within control limits, so long as the attack frequency was not so high as to influence the interictal level. During attacks precipitated by eating chocolate or sour apple (in the case of one of the patients), forehead sweating was also increased on the symptomatic side. The forehead sweating responses to heating and pilocarpine were without any notable or systematic asymmetries. The forehead sweating pattern in SUNCT syndrome may differ from the patterns in unilateral headaches like cluster headache, on the one hand (in which there is generally an asymmetry during heating and pilocarpine tests), and chronic paroxysmal hemicrania (CPH) and cervicogenic headache, on the other (where there is no systematic increase during attacks).

Notizen: Eleven patients with cervicogenic headache took part in a radiological diagnostic workup related to the head and neck. All the patients were female with a mean age of 43 years (range 25–59) at the onset of the study. Cerebral and cervical computer tomography as well as standard X-ray of the spine were carried out in all patients. Six patients underwent cerebral angiography and six cervical myelography. The different investigations showed no typical characteristic pathology in the group. No indication of a common therapeutic approach in this group of patients could therefore be derived from these investigations.

Notizen: Cluster headache periods (bouts) have, at the lower end of the scale, been supposed to last for periods as short as 1–2 weeks. Patients who usually have bouts of ordinary duration occasionally have brief attacks in the remission phase (“abortive attacks”). The group described here had a temporal pattern, clearly at variance with both these patterns: The six patients had mostly or entirely bouts of 〈 1 week's duration. We have termed such short-lasting bouts minibouts, on the supposition that such cases belong within the cluster headache, cycle. All the patients were men and had a history of unilateral headache without side shift, and usually the solitary attacks had some autonomic accompaniments, although of a moderate degree. In four cases only mild attacks occurred, whereas in two cases the attacks were rather severe and not consistent with full working ability. The group as such may not be homogeneous. The nosologic status of patients with bouts lasting only 1–2 days is somewhat uncertain. As long as the substrate of cluster headache is not outlined, the lower end of duration of minibouts cannot be accurately identified. The fact that three patients recently have experienced bouts of ordinary length strengthens the view that such cases really belong to the cluster headache cycle.
