    Notizen: The author provides a personal history of efforts to define mental disorder in the various editions of the American Psychiatric Association's Diagnostic and Statistical Manual of Mental Disorders. Weary from his own efforts, as a psychiatric nosologist, to define “mental disorder,” and unsure about the importance of having a consensus definition (there is none for medical disorder), the author regards Wakefield's “harmful dysfunction” construct as a brilliant breakthrough. Wake-field's definition clarifies the underlying issues while the author believes that Ossario's definition muddles the issues.

    Notizen: Lilienfeld et al's critique of the use of the term comorbid in psychopathological research is critically examined. They are incorrect in asserting that the original use of the term in medical epidemiology was intended to restrict it to disease entities. Furthermore, the ways in which medical comorbidity can be understood apply equally well to understanding the comorbidity of psychiatric or psychological disorders. Just as it has been useful to use the general term psychiatric disorders to include symptom patterns and syndromes and few If any true diseases, so to it is useful to apply the generic term of comorbidity to the joint occurrence of psychiatric disorders. Because methodological factors can sometimes contribute to artifactual comorbidity which is uninformative about the disorders being studied is no reason to abandon the term comorbidity in psycho-pathological research.

    Notizen: Abstract To improve observer uniformity and objectivity in the psychiatric appraisal of parents of psychotic children, structured psychiatric interviews were administered to 64 parents of psychotic children, including 28 husband-wife pairs and 8 single parents. Judgments were recorded on the Spitzer-Endicott Psychiatric Status Schedule and the past section of their Current and Past Psychopathology Scales. There were no significant differences between fathers and mothers of organic and nonorganic children (children with and without evidence of neurological dysfunction). As a whole, the 56 parents in the 28 participating husband-wife pairs fell between a contrast group of 55 adult subjects attending a psychiatric clinic and another contrast group of 130 subjects living in the community in upper Manhattan in their trends to pathological symptoms as judged by the Psychiatric Status Schedule. Similarly, in the past section of the Current and Past Psychopathology Scales the parents showed a trend to more psychopathology than a community sample of 36 nonpatient adults. Finally, computer diagnoses based on the Psychiatric Status Schedule showed more schizophrenia in the parents of the psychotic children than in a sample of 130 nonpatient adults in the community. The data thus tend to support findings based on the unstructured interview of elevated schizophrenia rates in parents of psychotic children.

    Notizen: Summary This paper deals with a variety of issues involved in program evaluation and planning in a new community mental health service, the Washington Heights Community Service. The demographic and psychopathological characteristics of the hospitalized seriously ill on this service are described. The psychopathology of this group is compared with out-patients and non-patients from the same general community and their differentiating characteristics are delineated. These data have been used in the development of a hospital program involving in-patient, partial hospitalization and after-care and designed to meet the specific needs of this hospitalized population.

    Notizen: Summary For many years the New York State Department of Mental Hygiene has used two nomenclatures to record psychiatric diagnoses. The state hospitals used the 1942 “Revised Classification,” a revision of the American Medical Association's 1934 classification, and all other mental health facilities used the first edition of theDiagnostic and Statistical Manual of the American Psychiatric Association. As of October 1968 all New York State mental health facilities will begin using one nomenclature: the second edition of theDiagnostic and Statistical Manual of the American Psychiatric Association. This article is an introduction to this new nomenclature for readers who have been using the 1942 Revised Classification. The background to the new nomenclature and the differences between it and the 1942 Revised Classification are discussed. Finally, a conversion table is presented which lists each diagnosis in the old classification and its counterpart, if present, in the new one.
