    Notizen: Eruption disturbances of permanent molars may become clinically and radio-graphically manifest as impaction, primary retention or secondary retention. This may result in clinical problems such as malocclusion and loss of neighboring teeth due to caries and periodontal disease. Which of these disorders will develop, depends primarily on the eruptive stage. Factors that may interfere with the eruptive stages (i.e. follicular growth, pre-emergent eruptive spurt, postemergent eruptive spurt, juvenile occlusal equilibrium, circumpubertal occlusal eruptive spurt, adult occlusal equilibrium) and the clinical consequences of that interference are reviewed. Treatment recommendations are given.

    Notizen: SUMMARY The maximal interincisal distance added to the vertical overlap is generally used as a measure for temporomandibular joint mobility. However, the length of the mandible also has an influence on this measure. The angle of mouth opening as a measure of temporomandibular joint mobility is independent of the length of the mandible. The aim of the study was to develop a mandibular goniometer to measure the angle of mouth opening and to test its realiability. Ten subjects, two males and eight females (mean age 28.1 years) participated in the study. The measurements were performed during four independent sessions of three measurements each. The results were analysed with ANOVA. Intraobserver variability was found to be 2–0° when the result of all the measurements were analysed, and were found to be 1–2° when the mean value of the sessions were used for analysis. Based on the results of this study it can be concluded that the angle of mouth opening canbe measured reliably.

    Notizen: The aim of this study was to investigate the potential clinical relevance of testing bite force endurance in patients with articular temporomandibular disorders. The endurance of a 50 N bite force was measured in 51 patients with painful temporomandibular joint disorders. The results were compared to those of a control group of 20 subjects. The force exerted was sustained until this taks could not be continued because of intolerable pain or fatigue. The endurance test was repeated following therapy.Testing bite force endurance could be reliably carried out (paired t-test not significant, procluct-momel11 correlation coefficient 0.87). The mean endurance time in the patient group was significantly different from that of the control group (t = 7.43, df = 69, P 〈 0.01). The 95% confidence intervals for patients and controls did not show any overlap. No difference in endurance time between diagnostic subgroups could be cletectcd (F = 1.30, df = 4,46, P 〈 0.28). Following treatment, all patients showed a significant increase of enclurancc time (t = 8.09, df = 50, p 〈 (1.01) and reported a decrease in post-test pain. The mean difference between pre- and post-treatment endurance was 60s. Subjecls of the control group stopped the biting effort predominantly because of muscle fatigue. By contrast, the main reason of the patients to cease the effort was TMJ pain.The results of this study indicate that the discriminatory power of the test is sufficient to justify ils utility as a complementary tool in assessing the functional capacity of the masticatory system.

    Notizen: summary  The craniofacial morphology of 31 male patients diagnosed with obstructive sleep apnoea syndrome (OSAS) and 37 healthy male subjects were compared using cephalometric evaluation of lateral skull radiographs. The aim was to evaluate which cephalometric variables related to craniofacial morphology discriminate between OSAS patients and non-OSAS subjects. Sixteen cephalometric measurements were determined to study the craniofacial morphology. Differences in cephalometric variables between the two groups were analysed with unpaired Student t-tests (significance level 0·05). A multiple regression analysis was used to evaluate the joint predictive value of selected variables. With regard to five variables, significant differences were observed between OSAS patients and non-OSAS subjects (P 〈 0·05). Data indicated a relatively anterior position of the maxilla, an increase of the mid-facial height and a more inferiorly located hyoid bone in the OSAS patient group. The multiple regression analysis yielded a model discriminating between OSAS patients and non-OSAS subjects, which included two hyoid bone variables and one related with the intermaxillary relationship. A causal relationship between cephalometric variables of craniofacial morphology and OSAS is not sufficiently supported by our data. Presumably, hyoid bone position is of predictive value in the cephalometric discrimination between OSAS patients and non-OSAS subjects.

    Notizen: In permanent temporomandibular disc displacement (TMJ-DD) outcome studies many authors claim positive effects of arthroscopic surgery, arthrocentesis and physical therapy. This literature review was undertaken to analyse whether the claimed effects are based on acceptable methodology. The recorded papers were analysed by two independent observers according to (1) method of investigation, (2) therapeutic intervention studied, (3) therapeutic outcome variables used, and (4) claimed effectiveness of the intervention. Agreement between observers was calculated. Twenty-four papers were found in which therapeutic outcome of interventions on temporomandibular disorders were studied. Six studies applied a true experimental design. Each of these six studies compared a different set of interventions. Twenty-two papers used maximal mouth opening (MMO) as an outcome variable, nine studied pain intensity on a visual analogue scale, one paper assessed the mandibular function impairment questionnaire. Kappa for overall agreement concerning the reviewing criteria was 0·82 (P≦0·001). No distinguishing effects on MMO, pain or function impairment were reported between arthroscopic surgery, arthrocentesis and physical therapy. Results of methodological sound outcome studies evaluating the effects of arthroscopic surgery, arthrocentesis and physical therapy are needed.

    Notizen: SUMMARY Temporomandibular joint (TMJ) mobility is assessed in different ways. Measurement of the interincisal distance in maximal mouth opening added to the vertical overlap of the dentition, i.e. linear mouth opening (LMO); measurement of angular displacement of the mandible relative to the cranium at maximal mouth opening, i.e. angle of mouth opening (AMO); assessment of the condylar position relative to the articular eminence performed on transpharyngeal radiographs in maximal mouth opening, i.e. condylar mobility (CM); measurement of angular displacement of the mandible at maximal mouth opening relative to the closed mouth position of the mandible, assessed on transpharyngeal radiographs in maximally opened and in closed mouth position, i.e. radiographic angle of mouth opening (RAMO). To compare these TMJ mobility assessment methods and to analyse the relationship between LMO, AMO and mandibular length (ML), 28 healthy volunteers (13 females, 15 males), mean age 29.6 years (range 21–41) with a symmetrical mouth opening pattern were assessed. LMO, AMO, ML, and RAMO were measured and CM was assessed on transpharyngcal radiographs. LMO and AMO were strongly and significantly related (r=0.71). LMO was significantly influenced by AMO and ML. No significant differences between AMO and RAMO were found. CM was strongly and significantly related to AMO (r=0.72) and weakly but significantly related to LMO (r=0.42). Based on the results of this study recommendations are made regarding the appropriate application of TMJ mobility assessment methods.

    Notizen: summary  The objective of the present study was to analyse the effects of implant supported overdentures on masticatory function in patients with an extremely resorbed mandible, and to compare the masticatory function in these patients using three differing types of implant treatment protocols. The mandibular overdentures were retained by a transmandibular implant, by four endosseous implants following augmentation of the mandible, and by four short endosseous implants, respectively. Sixty patients (50 women, 10 men, mean age 59·4 years) were randomly allocated to one of the three treatment groups. Masticatory function was assessed before and after treatment using a questionnaire, a masticatory performance test, and a structured interview. The patient-based masticatory function improved significantly. Concerning these parameters there were no significant differences between the three groups before and after treatment. A significant difference existed between the three groups for the laboratory-assessed masticatory function before treatment, but after treatment this difference was no longer significant. From this study it can be concluded that patients with an extremely resorbed mandible and functional complaints of their lower denture report significant improvement in masticatory function after implant-overdenture treatment. Differences in masticatory function between the three studied modalities were not significant after treatment.

    Notizen: Fitting a temporomandibular joint (TMJ) prosthesis to the skull by using stock prostheses seems to be an appropriate method. However, fitting the skull with one stock part requires many differently shaped parts. Therefore, we fitted the skull with two connected stock parts. The aim of the study was to test whether it is possible to achieve a close fit to the skull with this design, with a maximum of 10 different parts. The articular eminence was fitted with a gully-shaped fitting member, which was rotationally connected to a basic part that fitted to the lateral side of the TMJ. The relevant dimensions of 20 dry skulls were measured and the results were used to derive the optimal dimensions of the prosthesis parts. Prototypes were subsequently fabricated. The fit of the prototypes was tested by measuring the maximum gap between fitting member and skull. All skulls could be fit with a set of four different basic parts and three different fitting members. The average maximum gap between fitting member and skull was 0·20 mm (range 0·11–0·43 mm). It was concluded that a close fit to the skull can be achieved with two connected stock parts and with a total number of seven parts.

    Notizen: Theoretically, mouth opening not only reflects the mobility of the temporomandibular joints (TMJs) but also the mandibular length. Clinically, the exact relationship between mouth opening, mandibular length, and mobility of TMJs is unclear. To study this relationship 91 healthy subjects, 59 women and 32 men (mean age 27·2 years, s.d. 7·5 years, range 13–56 years) were recruited from the patients of the Department of Oral and Maxillofacial Surgery of University Hospital, Groningen. Mouth opening, mobility of TMJs and mandibular length were measured. The mobility of TMJs was measured as the angular displacement of the mandible relative to the cranium, the angle of mouth opening (AMO). Mouth opening (MO) correlated significantly with mandibular length (ML) (r=0·36) and AMO (r=0·66). The regression equation MO=C1×ML×AMO+C2, in which C1=0·53 and C2=25·2 mm, correlated well (r=0·79) with mouth opening. It is concluded that mouth opening reflects both mobility of the TMJs and mandibular length.

    Notizen: summary In a controlled clinical trial, treatment effects of mandibular overdentures on two different implant-systems in edentulous patients were compared one year after insertion of the new dentures. The implant-systems used were the Brånemark system (Brå) and the IMZ-system. Treatment was randomly assigned to 60 patients according to a balanced allocation method. Evaluation included peri-implant and radiographical parameters. According to the Delphi method a clinical implant performance scale (CIP) was constructed based on all conceivable complications of the different implant systems. During the osseointegration period, five Brå- and one IMZ-implants were lost. The results of one of the peri-implant parameters and the radiographical score showed significant differences considering the (pseudo) pocket probing depth (Brå better than IMZ, P〈 0.001) and the radiographic-score (IMZ better than Brå, P 〈 0003). The results for the CIP-scale were less favourable for the Brå-group than for the IMZ-group; however, these differences were not significant.
