    Notes: Abstract The radiographic and sectional imaging features (ultrasound [US], computed tomography [CT], and magnetic resonance imaging [MRI]) of ureteral obstruction due to metastatic disease are reviewed. The radiologic findings depend on the pattern of the tumoral spread: hematogenous submucosal/mucosal metastasis, hematogenous adventitia metastases spreading along periureteral vessels, scirrhous metastatic spread along periureteral vessels, or metastatic spread into lymph nodes with perinodal desmoplastic reaction. Solitary or multiple extraluminal obstructions without substantial displacement of the ureter are characteristic. CT is the examination of choice for morphological analysis. Together with the patient's history and clinical data, a presumptive diagnosis of ureteral obstruction caused by metastatic disease can be made.

    Notes: Summary When a urinary reservoir intended to replace the bladder is made from bowel, it should meet several requirements: good capacity, viscoelasticity and compliance, voluntary control of micturition without residual (infected) urine, a sensation of the filled state and urinary continence. In addition, there should be no major metabolic changes due to malabsorption after bowel resection or due to reabsorption of urinary constituents by the reservoir. In this review several conflicting aspects of bladder reconstruction are addressed: the persisting intestinal peristalsis and urinary incontinence, the volume of the reservoir and its metabolic impact, the bowel segment to be used and the amount that can be resected without the risk of long-term sequelae. Our clinical experience with ileal bladder substitutes in 80 patients underlines the theoretical aspects. After careful instruction, our patients increased the functional capacity of their reservoirs to 500 ml, a precondition for good urinary continence. Provided that the patients were regularly followed-up, the functional, clinical and metabolic results were good. The operative procedure was easy to perform, and no major metabolic sequelae occurred during a maximal observation time of 6 years. Nevertheless, continuing careful follow-up for the detection of potential long-term sequelae, such as disturbances in lipid metabolism or chronic bone demineralisation, are required before definitive statements on the role of intestinal bladder substitutes can be made.

    Notes: Summary The discrepancy between the incidence of latent prostate cancer and that of clinically overt carcinoma suggests that there can be different courses in the biological progression of prostate cancer. As this cancer is detected increasingly at an infraclinical stage, markers are needed to indicate which lesions will progress and lead to the patient's death. To investigate the possibility that specific growth factors and/or proto-oncogenes are expressed differentially, we measured mRNA levels of transforming growth factors β1 (TGF-β1), TGF-β2 and TGF-β3 and of the c-fos and c-jun oncogenes by Northern blotting in normal prostate, benign prostatic hyperplasia (BPH) and prostate cancer. Our data demonstrate that expression of TGF-β1 increased, whereas that of TGF-β3 fell to an almost undetectable level in carcinoma. Expression of c-fos followed the TGF-β1 pattern, whereas no difference could be seen in c-jun expression in cancer as compared with BPH and normal prostate. The differential expression of TGF-β1, TGF-β3 and c-fos could possibly be used to improve the characterisation of prostate cancer. Long-term follow-up of patients may indicate whether mRNA levels of these growth factors and oncogenes correlate clinically and whether they can be used as markers for progression in human prostate cancer.

    Notes: Summary The clinicopathological features of two carcinosarcomas of the urinary bladder are reported. The tumours occurred in a 64- and a 66-year-old patient presenting with haematuria and both were polypoid. The epithelial component was consistent with small cell undifferentiated carcinoma with neuroendocrine differentiation, whereas the sarcomatous component did not display specific features. The carcinomatous component showed immunohistochemical reactivity for different epithelial markers as well as for chromogranin and neuron specific enolase. Conversely, the sarcomatous cells stained strongly for vimentin and in one case for muscle actin and smooth muscle actin. The differential diagnosis of biphasic tumours of the bladder is discussed and the literature reviewed.

    Notes: Abstract We show that the non-relativistic quantum mechanics of particles with spin coupled to an electromagnetic field has a naturalU(1)×SU(2) gauge invariance. Ward identities reflecting this gauge invariance combined with an assumption of incompressibility of a system of such particles in an appropriate external field and for suitable values of the particle density permit us to determine the form of the effective action of the system as a functional of small fluctuations in the electromagnetic field, in the large-distance-, adiabatic limit. In this limit, the action is found to have a universal form. We present explicit results for two-dimensional, incompressible electron fluids and apply them to derive the equations of linear response theory, describing a variety of generalized Hall effects. Sum rules for the Hall conductivities, magnetic susceptibilities and other quantities of physical interest are found.

    Notes: Abstract An action functional, related to the Higgs model to field theory, depending on a complex scalar field and aU(1) connection is defined. The complex scalar field is a section of a line bundle associated to a principalU(1)-bundle with base space ℝ3\{x 1,...,x n }. The pointsx 1,...,x n are the positions ofn magnetic monopoles of magnetic chargesm 1,...,m n, with $$\sum\nolimits_{i = 1}^n {m_i = 0} $$ . The existence of minimizers of the action functional is proven using direct methods of the calculus of variation. Regularity and decay properties of the minimizers are obtained. By constructing explicit comparison field configurations, we establish accurate upper and lower bounds for the action of the minimizers in a variety of special situations, e.g.n=2 andm 1=−m 2.

    Notes: Summary Using unfixed mouse bladder tumor cells (MBT) as target cells and a modified avidin-biotin-complex (ABC) method made it possible to detect a humoral immune response in C3H mice with growing MBT tumors. The rise of the serum levels is significant (p〈0.005) when compared to control animals and correlates with the tumor size. Mice with recurrences after surgical removal of the primary tumor had significantly (p〈0.05) higher serum values than animals without recurrence. Purulent or granulomatous inflammatory changes, muscle necrosis, growth of a lymphoma or an ovarian carcinoma did not significantly change the results (p〈0.05). Injection of myeloma cells from a different strain of mice caused a minimal, but significant increase in the values when compared to controls (p〈0.0001). However, the slope of the curve differed significantly (p〈0.05) from that in mice with MBT tumors. Irradiated MBT cells or Corynebacterium parvum used as immunomodulators significantly increased the serum values (p〈0.001). In the presence of a growing MBT tumor, however, the immunomodulation did not substantially falsify the results. The serum values were related to the corresponding tumor sizes.

    Notes: Summary An immunoassay using a human bladder tumor tissue culture cell line (J-82) as antigen and a modified avidin-biotin-complex (ABC) method were used to examine human serum samples. Optical density (OD) of 25 serum samples obtained from 19 patients with bladder tumors had a mean OD of 458+/-71 SD (328–570). This is significantly higher (p〈0.0005) when compared to the mean OD of 172+/−49 SD (107–301) found in serum samples from 28 patients with tumors other than bladder cancer and to the mean OD of 207+/−48 SD (136–310) of serum samples from 25 healthy donors. Thirty-one serum samples from 21 patients who had a bladder tumor surgically removed 1 to 36 months before were, with 2 exceptions, also higher than 320 (mean 412 [248–550]). All serum samples of patients with presence of a bladder tumor above the cut-off point of 320 and those from the 2 control groups below it shows, thus far, a high sensitivity as well as specificity of the method used and encourages further assessment of its potential usefulness as a serum marker assay in patients with bladder cancer.
