    Notes: Abstract From 1980 to 1992 we followed 12 patients with cardiac myxomas for an average of 4.4 years (8 months–11 years). Presenting symptoms were neurological in four patients (hemiparesis, aphasia, visual field deficits, progressive dementia or vertigo), progressive dyspnoea in six, pulmonary embolism in one, and peripheral arterial or renal emboli in three. The diagnosis was suspected clinically in 11 patients. It was confirmed by echocardiography in ten and by thoracic CT in one. All these patients had cardiac surgery. One diagnosis was made at autopsy; the patient died unexpectedly during surgery for emboli to the leg arteries. At follow-up, two additional patients had died, one from myocardial infarction and one from rhabdomyosarcoma. Only one of the nine surviving patients had recurrent symptoms after cardiac surgery. His dementia continued to progress. The patients without new symptoms after cardiac surgery had normal MRI of the brain or residual ischaemic lesions. MRI of the patient with progressive dementia showed multiple cerebral lesions with a bright centre and a dark rim on T1- and T2-weighted spin-echo images. On CT there were many calcified lesions. CT, MR angiography and contrast angiography revealed multiple fusiform aneurysms. The rare occurrence of progressive neurological symptoms after myxoma resection with multiple cerebral lesions and aneurysms should suggest myxoma metastases to the brain.

    Notes: Abstract Six patients with Hashimoto thyroiditis (HT) and associated encephalopathy (HE) are described and compared with 14 well-documented cases retrieved from the literature. HE typically affects patients when they are euthyroid and, in an appropriate clinical situation, antithyroid autoantibodies are the main indicators of HE. Since clinical features of HE are unspecific, other aetiologies such as infectious, metabolic, toxic, vascular, neoplastic, and paraneoplastic causes have to be excluded. Our own six cases and those from the literature show that two types of initial clinical presentation can be differentiated: a vasculitic type with stroke-like episodes and mild cognitive impairment in nine patients, and a diffuse progressive type with dementia, seizures, psychotic episodes or altered consciousness in 11 patients. These types may overlap, particularly in the long-term course without treatment. Response to steroids was usually excellent with complete remission in 80%. Eighteen of the 20 patients were women. Characteristic, though unspecific, findings were abnormal EEG (90%) and CSF (80%). Together with quantitative neuropsychological testing, these proved sensitive for monitoring the efficacy of therapy. Conversely, antithyroid autoantibody titres did not correlate with the severity or type of clinical presentation. The link between HE and HT is not clear. A pathogenetic role for antithyroid autoantibodies in the central nervous system seems unlikely.

    Description / Table of Contents: Zusammenfassung Wir berichten über die Resultate von 27 zwischen 1972 und 1976 operierten mehrfragmentären Humeruskopffrakturen. Es wurden nur Brüche mit mindestens drei bzw. vier Hauptfragmenten sowie Trümmer- und Luxationsfrakturen berücksichtigt. Die Osteosynthese erfolgte bei 17 Patienten mit Abstützplatten, bei 10 Patienten mit einer Minimalfixation durch eine Zuggurtungsschlinge oder Einzelschrauben. Mindestens 3,5 Jahre postoperativ wiesen von 27 Patienten acht ein sehr gutes, acht ein gutes, sechs ein mäßiges und fünf ein unbefriedigendes Gesamtergebnis auf. Radiologisch wurden nebst passageren Dystrophien sechs (22,2%) avaskuläre Nekrosen des Humeruskopfes festgestellt (bei einer Dreifragment-, einer Vierfragmentfraktur, drei Trümmerbrüchen und einer Luxationsfraktur). Fünf Nekrosen (34%) traten nach Plattenosteosynthese, eine Nekrose (10%) nach alleiniger Drahtcerclage des Tuberculum majus auf. Die Inzidenz der prognostisch ungünstigen Kopfnekrose ist vorwiegend abhängig von der primär vorliegenden Frakturform. Ein zusätzliches operationsbedingtes Nekroserisiko ist laut unseren Ergebnissen bei schonender Exploration und sparsamer Osteosynthese geringer als bei der bezüglich Zugang aufwendigeren Platten-osteosynthese. Wir glauben daher heute, der bewegungsstabilen Minimalfixation den Vorzug geben zu können.
    Notes: Summary This paper reports the results of 27 multi-fragmented fractures of the proximal humerus treated between 1972 and 1976. Only 3- and 4-part fractures as well as fracture dislocations were included. Internal fixation was performed in 17 cases with buttress plates and in 10 patients by minimal fixation with a tension band wire or simple lag screw. On an average of 3.5 years postoperatively out of 27 patients eight had excellent, eight a good, six a fair, and five a poor overall result. Radiologically six cases of avascu1ar necrosis of the humeral head (22.2%) were detectable in one 3-part, four 4-part and one fracture dislocation. Five times necrosis was observed after plate fixation (34%) once after wiring of the greater tuberosity. The incidence of unfavorable head necrosis is dependent on the primary fracture type. The results point to the fact that further damage due to surgery and its risk to the remaining blood supply is less prominent in gentle exploration and minimal osteosynthesis than with a generous exposure needed for plate fixation. Minimal fixation thus should be preferred.

    Description / Table of Contents: Zusammenfassung Zur Evaluation des Kraftanteiles des langen Bicepsmuskels wurden 10 konservativ und 5 operativ behandelte proximale Sehnenrupturen mit dem Cybex-II-Meßgerät isokinetisch mit verschiedenen Winkelgeschwindigkeiten geprüft und mit 20 Kontrollpersonen verglichen. Die Krafteinbuße betrug bei konservativ behandelten Sehnenrissen am Ellbogen 16% Beugekraft, am Vorderarm 11% Supinationskraft und am Schultergelenk 16% Abduktionskraft. Bei operierten Patienten ergaben sich Verluste von 8%, 7% bzw. 20% für die entsprechenden Funktionen. Die Operation kann somit den Beuge- und Supinationskraftverlust um ca. die Hälfte verringern, ist indessen der Schulterabspreizkraft abträglich. Entsprechend kann sie jüngeren Patienten, Handwerkern und Sportlern empfohlen werden. Selten ist sie indiziert aus kosmetischen Gründen oder wenn ein zurückbleibender proximaler Sehnenstumpf eine subakromiale Entzündung verursacht.
    Notes: Summary To measure muscular strength after rupture of the long head of the biceps brachii, we evaluated ten patients treated conservatively and five surgically with the Cybex II testing machine, and compared the values with the data on 20 healthy individuals. In the conservative group, the strength of elbow flexion was diminished by 16%, of supination of the forearm by 11%, and of shoulder abduction by 16%. The patients who underwent surgery lost, on average, only 8% strength for elbow flexion and 7% for forearm supination. Shoulder abduction was decreased by 20%. A moderate advantage was thus found for elbow flexion and forearm supination. Surgical treatment can be recommended for younger individuals, manual workers, and sportsmen, and rarely for cosmetic reasons, or when a subacromial impingement due to a disturbing proximal tendon stump is present.

    Description / Table of Contents: Summary We report a young woman presenting with „painful diplopia” caused by inflammation of external eye muscles affecting both eyes sequentially. Orbital MRI disclosed swelling, signal hyperintensity and enhancement of isolated eye muscles. Corticosteroid treatment led to a complete remission within a few days. The compiled data of a literature review enclosing 52 sufficiently documented patients diagnosed as orbital myositis is reported with respect to clinical features, laboratory findings, associated disease, value of imaging procedures (CT, MRI). Therapeutic aspects and differential diagnosis are reviewed.
    Notes: Zusammenfassung Eine junge Patientin mit subakuter schmerzhafter Diplopie als Folge von entzündlichen Veränderungen der äußeren Augenmuskeln, die sequentiell beide Augen betrafen, wird vorgestellt. Das orbitale Kernspintomogramm zeigte eine Schwellung, Signalhyperintensität und Kontrastmittelanreicherung einzelner Augenmuskeln. Die Behandlung mit Kortikosteroiden führte zu einer vollständigen Remission der Symptomatik innerhalb weniger Tage. Aufgrund der Daten von insgesamt 52 gut dokumentierten Patienten mit okulärer Myositis aus der Literatur stellen bewegungsabhängige retrobulbäre Schmerzen (94%) mit Diplopie (85%) die Kernsymptome des Krankheitsbildes dar. Ein weiterer klinischer Hauptbefund ist die konjunktivale Injektion (73%), meistens an der Insertionsstelle des betroffenen Muskels. Die exophthalmische Form mit zusätzlichem Lidödem, Ptose, Chemose und Exophthalmus tritt seltener auf (24%), eine Visusabnahme ist die Ausnahme. Ein beidseitiger Augenmuskelbefall, typischerweise sequentiell, wird in 40% beobachtet. Frauen sind häufiger betroffen (73%), das mittlere Erkrankungsalter liegt bei 34 Jahren. Der M. rectus medialis ist der am häufigsten betroffene Muskel (70%). Die Diagnosesicherung gelingt heute mit der Kernspintomographie auch in leichteren Fällen. Der durch die Entzündung erhöhte Wassergehalt der befallenen Muskeln erklärt die gegenüber der Computertomographie erhöhte Sensitivität der Kernspintomographie. Therapie der Wahl ist die Behandlung mit Kortikosteroiden, hierunter kommt es in 90% innerhalb von Tagen zur Abheilung. Bei zu kurzer Therapiedauer sind Rezidive häufig. Bei Therapieresistenz ist die niedrigdosierte Bestrahlung meist effizient.
