    Notes: Abstract. Erythrocyte sodium-potassium (Na+/K+) -ATPase and sodium-lithium (Na+/Li+) countertransport activities were measured in 18 children (aged 9.6 years, range 6–16 years) with idiopathic hypercalciuria (IHU) to evaluate cellular Na handling. The effect of chronic thiazide administration on these parameters and on bone mineral density was also evaluated. Patients with IHU had significantly lower erythrocyte Na+/K+-ATPase activity than 23 age-matched healthy controls (mean±SEM 2,156±110 μmol P/l erythrocyte per hour vs. 3,165±175, P〈0.01). Thiazide treatment significantly lowered urinary calcium excretion; this was followed by a slight suppression of intact parathyroid hormone (iPTH). The urinary calcium/creatinine ratio before and during treatment was 0.90±0.07 mmol/mmol versus 0.51±0.06 respectively, P〈0.01. The corresponding iPTH levels were 5.9±0.6 pmol/l and 5.1±0.7, P〈0.05. The Na + /K + -ATPase activity increased significantly (2,769±169 μmol P/l erythrocyte per hour vs. 2,156±110 in the control period, P〈0.01) and the Na + /Li + countertransport decreased (268±28 μmol Li/l erythrocyte per hour vs. 328 + 26 in the control period, P〈0.03). The bone mineral density Z score rose from –1.3±0.26 to –0.8±0.22 (P〈0.03). We conclude that IHU is accompanied by abnormalities of erythrocyte Na + /K + -ATPase and Na + /Li + countertransport which are corrected by chronic hydrochlorothiazide administration. These changes could model alterations in renal tubular transport mechanisms still to be elucidated. Chronic thiazide treatment also has a positive effect on bone mineral density.

    Notes: Abstract Urinary excretion of various renal prostaglandins was measured by radioimmunoassay and gas chromatography-mass spectrometry in children who had different degrees of metabolic control. Excretion in PGE2 in diabetic children was twice control values irrespective of the presence or absence of diabetic ketoacidosis (DKA). The urinary excretion of PGF2α was significantly increased in diabetic children with ketoacidosis, but not when diabetes was well controlled. The excretion of 13, 14 dihydro-15-keto PGE2, the major metabolite of circulating PGE2, was increased in all diabetic children, and was most elevated in ketoacidosis when it averaged 10 times basal excretion. Urinary excretion of PGI2α and of 6-keto-PGF1α, the metabolite of PGI2, was approximately doubled in DKA compared with values from healthy subjects. Excretion of PGE2 was twice control values in children with stable diatetes, whereas the equivalent value for TXB2, the metabolite of the active vasoconstrictor TXA2, was reduced by approximately 50%. We suggest that the increased excretion of prostacyclin metabolite may result from a protective biological action on the kidney opposing other vasoconstrictor hormone activity. PGE2 appears not to be involved in this process. The highly elevated excretion of PGE2 metabolite may represent an activation of systemic PGE metabolism during DKA.

    Notes: A method has been devised for the estimation in fermentation media, of 1-dehydro-17-α-methyltestosterone in the presence of 17-α-methyltestosterone. This method involves the extraction of the steroids with chloroform, and spectrophotometric measurement of the chromogen formed by a specific color reaction with sulfuric acid and 1-dehydro-17-α-methyltestosterone. The fermentation liquor and 17-α-methyltestosterone do not interfere. With this rapid (30 min.) method, quantitative tests on the progress of the steroid transformation can be made at frequent intervals during one fermentation run, these tests yielding the necessary information for an adequate control of the optimum conditions of production.

    Notes: Conclusion Extremely low plasma ANP levels in DKA associated with severe volume depletion suggest a negative feedback regulation of the hormone. In DKA the plasma volume and sodium regulating hormonal systems are markedly altered by the volume depletion. Plasma concentrations of vasoconstrictor and sodium and water retaining hormones like AVP, renin, aldosterone and noradrenaline are increased, whereas that of ANP, with potent vasodilator and natriuretic properties, is suppressed. Following volume repletion, most of the alterations are reversible.

    Notes: A rapid analytical method has been developed to follow the biological oxidation of 17-α-oxy-11-desoxy-corticosterone (Reichstein compound S) to hydrocortisone. Paper chromatography is too slow to follow the progress of a preparative microbiological steroid transformation. However, 17-α-oxy-11-desoxy-corticosterone can be separated in a sufficiently pure state for photometric assay by a simple extraction from fermentation broths containing several steroids. This extraction, coupled with the formation of a sulphuric acid chromogen, permits estimation of the steroid in fermentation liquors. The steady decrease of the 17-α-oxy-11-desoxy-compound thus estimated proved to be a true indication of the progress of the oxidation process, allowing it to be stopped at the most favourable time.

    Notes: An analytical method has been devised for the rapid determination of prednisolone produced in a fermentative oxidation process. It allows frequent checking of concentrations of the prednisolone formed during the oxidation of hydrocortisone.The estimation of prednisolone in the presence of hydrocortisone is based upon the diversity of colour shown by the chromogens produced from these steroids by sulphuric acid and ethanol. Using the optimal reagent mixture and conditions, the interference of hydrocortisone is negligible. The steroids are extracted from the fermentation broth with ethyl acetate. One test takes about 20 min.
