    Notes: Abstract We report two case histories of previously healthy patients who both developed persistent dyskinetic syndromes (spasmodic torticollis and cranial dystonia, respectively) following the intake of norpseudoephedrine (NPE) as an appetite suppressant. The symptoms took a chronic course even after NPE intake was discontinued. Similar drug-induced dyskinesias have been described for amphetamine and neuroleptic drugs. This side effect has, however, not yet been reported for NPE, which is pharmacologically related to amphetamine. One of the patients may also have had multiple sclerosis. Structural lesions in the basal ganglia area might predispose the development of such a movement disorder. The potential relationship between NPE intake and the development of dyskinesia is discussed. Appetite suppressants, often taken without the neurologist's knowledge, may be the cause of dyskinetic syndromes.

    Description / Table of Contents: Summary The discussion of the Osheroff vs Chestnut Lodge court case in the ”American Journal of Psychiatry” raised the question whether individual psychotherapy or pharmacotherapy is the appropriate treatment for severe depression. A summary of the crucial points and a review of the clinical and scientific implications of this case is presented. In addition, the difference between psychiatrists and psychotherapists regarding the assessment of depressive disorders is examined by analysing results of the ICD-10 research criteria study. In the case of one patient with dysthymia (ICD-10 RC: F34.1) psychotherapists tended more towards a reactive or neurotic etiology than psychiatrists. By contrast, the percentage of clinicians suspecting an endogenous etiology and thus, diagnosing a psychosis and starting a psychopharmacotherapy was markedly higher among psychiatrists. In conclusion, the integration of different conceptualizations and treatment modalities is emphasized as an essential task for both psychotherapists and psychiatrists. For certain patients, an integrated therapy may be more effective.
    Notes: Zusammenfassung Die juristische Auseinandersetzung Osheroff versus Chestnut Lodge hat im „American Journal of Psychiatry” zu einer Diskussion der Frage geführt, ob eine Psychotherapie oder eine Pharmakotherapie die angemessene Behandlung einer schweren Depression sei. Wir referieren die wesentlichen Details dieser Auseinandersetzung, aus denen sich klinische und wissenschaftliche Implikationen ergeben. Weiterhin untersuchen wir die Frage, inwieweit Psychiater und Psychotherapeuten sich in bezug auf die diagnostische und therapeutische Beurteilung depressiver Störungen unterscheiden und analysieren dazu Befunde der ICD-10 Forschungskriterienstudie. Deren Ergebnisse zeigen, daß Psychotherapeuten bei der Beurteilung einer Kasuistik mit der ICD-10-Diagnose einer Dysthymia (F34.1) im Vergleich mit den Psychiatern häufiger eine reaktive Ätiologie und eine Neurose angenommen haben. Umgekehrt sind die Psychiater häufiger von einer endogenen Ätiologie ausgegangen, haben häufiger eine Psychose diagnostiziert und eine Psychopharmakotherapie empfohlen. Abschließend betonen wir die Bedeutung der Integration verschiedener theoretischer Konzepte und Therapiemethoden als eine wesentliche Aufgabe von Psychotherapeuten und Pharmakotherapeuten. Für bestimmte Patienten darf von einer kombinierten Therapie ein größerer Therapieeffekt erwartet werden.
