    Description / Table of Contents: Abstract A 36-year old woman was presented to our hospital with congenital ventricular septal defect and one-vessel coronary artery disease (75% proximal left main coronary artery) for CABG and repair of the VSD. After induction, a transesophageal echocardiographic (TEE) baseline examination was performed, showing a severely dilated coronary sinus (CS) measuring approximately 3 cm (abnormal 〉1 cm). We suggested a persistent left superior vena cava (PLSVC) draining into the CS. PLSVC is a common venous congenital anomaly, with a reported incidence of 0.5% in general population and in 3–5 % of patients with congenital heart defect. Injection of echo-contrast solution in a left arm vein, visualizing microbubbles passing through the PLSVC into the CS confirmed our suspicion. The diagnosis of a PLSVC and dilated CS is a contraindication for retrograde cardioplegia because of the loss of cardioplegia into the PLSVC resulting in a inadequate myocardial protection. It may be difficult to pass a pulmonary artery catheter (PAC) through a left internal or left subclavian vein and it may be associated with arrythmias. A chest radiograph shows the anomalous course of the PAC along the left heart.
    Notes: Zusammenfassung Eine 36jährige Frau, mit einem kongenitalen Ventrikelseptumdefekt (VSD) und einer koronaren 1-Gefäßerkrankung (75% Hauptstammstenose) wurde unserem Krankenhaus zur Koronarbypassoperation und Verschluß des VSD zugewiesen. Nach der Narkoseeinleitung führten wir eine transösophageale echokardiographische Basisuntersuchung durch. Es zeigte sich ein deutlich dilatierter Koronarsinus (CS) von ca. 3 cm Durchmesser (abnormal 〉1 cm). Man vermutete eine persistierende linke obere Hohlvene (PLSVC), die sich in den CS drainiert. Die PLSVC ist eine venöse kongenitale Annomalie mit einer Inzidenz von 0,5% in der Normalbevölkerung und von 3–5% bei Patienten mit kongenitalen Herzerkrankungen. Nach Injektion von Echokontrastmittel über eine linke Armvene zeigten sich sog. Microbubbles im CS und bestätigten unsere Verdachtsdiagnose. Die Diagnose einer PLSVC und eines dilatierten CS gilt als eine Kontraindikation für die retrograde Kardioplegie, da es über die PLSVC zum Verlust von Kardioplegie, mit daraus resultierender unzureichender Kardioprotektion kommt. Die Plazierung eines Pulmonalarterienkatheters über die linke V. jugularis interna oder die linke V. subclavia kann erschwert sein und zu Herzrhythmusstörungen führen. Eine Röntgenthoraxaufnahme zeigt den ungewöhnlichen Verlauf des PAK entlang des linken Herzschattens.

    Description / Table of Contents: Abstract. This article focuses on internet applications, related quality goals and quality assurance techniques, including testing approaches. Firstly the peculiarities of internet applications, especially those influencing quality assurance and testing are discussed. On the basis of established practical methods special techniques for testing internet applications are introduced. Finally, the authors' experience regarding testing tools, test automation and organizational issues of testing is summarized.
    Notes: Zusammenfassung. In diesem Artikel werden die für Internet-Anwendungen relevanten Qualitätsziele und die sich daraus ergebenen Qualitätssicherungs- (QS-) und Testaufgaben dargestellt. Zunächst werden die besonderen Architekturmerkmale von Internet-Anwendungen, sofern sie Einflußauf die QS haben, erläutert. Auf der Basis eines in der Praxis bewährten generellen Vorgehens für die Qualitätssicherung und den Test bei der Software-Anwendungsentwicklung werden dann spezielle Vorgehensweisen für die QS und den Test von Internet-Anwendungen eingeführt. Erkenntnisse aus der Praxis zum Thema Testwerkzeuge, Testautomatisierung und Testorganisation runden den Beitrag ab.

    Notes: Summary Microscopic sections obtained from paraffin blocks of 30 consecutive neurilemomas stored in 1989 were investigated immunohistochemically with the new monoclonal antibody MIB 1 directed against recombinant parts of Ki-67 antigen. The immunohistochemical staining was carried out on dewaxed microwave-oven-processed paraffin sections of formalin-fixed tumour tissues. The labelling index (LI) obtained was below 1% in 21 cases, 1–3% in 7 cases and more than 3% in 2 cases. There was no correlation between the LI and the age of the patients or the tumour size. The major advantage of MIB 1 over previous immunohistochemical methods is its usefullness in long preserved sections which are ideal for retrospective studies.

    Notes: Summary Formalin-fixed paraffin sections of 55 consecutive bilateral vestibular schwannomas in 46 patients with neurofibromatosis 2 (NF2), and 50 patients with unilateral vestibular schwannomas were investigated immunohistochemically with the monoclonal antibody MIB 1 directed against recombinant parts of Ki-67 antigen. The immunohistochemical staining was carried out on dewaxed microwave oven-processed paraffin sections of formalin-fixed tumour tissues. The labelling index (LI) obtained was compared to clinical and histological findings in both groups. There was no correlation between the LI and age of the patients, tumour size, or histological type of tumour (Antoni A or B). Vestibular schwannomas in NF2 showed higher LI than unilateral vestibular schwannomas: the maximal LI found per section (LI max) ranged from 0.4 to 17.6% (mean, 2.7%) in NF2 schwannomas, and from 0 to 9% (mean, 2.2%) in unilateral schwannomas. These differences may express immunohistochemically some clinical and morphological differences between bilateral and unilateral vestibular schwannomas.
