Notes: Abstract The left-lobe subxiphoid approach has been the standard way to drain left hepatic duct occlusion. A new, right-side biliary drainage approach is described. Five patients, among 25 patients with bilateral biliary obstruction, were treated by an internalinternal drainage system. Three types of internalinternal drainage techniques are described: internal-internal luminal, type 1; internal-internal luminal, type 2; and internal-internal transductal, type 3. All 5 patients succeeded in biliary drainage. The internal-internal biliary drainage technique, performed from the right-side approach, is effective, avoids additional liver puncture and tract dilatation, and allows the combination of several biliary drainage techniques, adding greatly to the flexibility when treating intrahepatic obstruction. No complications were observed in our small series but special precaution should be taken with the transductal technique due to the possibility of bleeding.

Notes: Abstract A case with association of congenital cystic dilatation of the common bile duct and congenital diverticulum of the hepatic duct is reported. The etiology and classification are briefly discussed and a radiological approach is recommended.

Notes: Abstract A case of resolution of mesenteric angina is reported with percutaneous transluminal angioplasty of a superior mesenteric artery stenosis, using the Grüntzig technique with a balloon catheter. Technical aspects are described. Short-term follow-up reveals symptomatic relief, disappearance of the abdominal bruit, and normalization of d-xylose absorption.

Notes: Abstract Five cases of parasitic and mycotic infection affecting the biliary tract are reported. Special attention is paid to the radiological presentation of chlonorchiasis, ascariasis, echinococcosis, and candidiasis of the biliary tract. Each entity is briefly described, and the differential diagnosis and specific radiological patterns are discussed. Biliary tract manipulation with a balloon catheter and endoprosthesis is presented as an option for management of the biliary obstruction in some cases.

Notes: Abstract A case of successful obliteration of a bleeding splenic artery pseudoaneurysm secondary to necrotizing pancreatitis is reported. The material used for embolization was stainless steel coils. A variation of the technique is introduced. Instead of occluding the pseudoaneurysm cavity itself, embolization only of the afferent vessel is advised.

Notes: Abstract Purpose To describe the spectrum of radiologic findings in crutch-induced axillary artery injury and the effects of its unique pathophysiology on diagnostic evaluation and treatment. Methods Three patients with axillary crutch-induced axillobrachial injury were studied angiographically and percutaneous intervention was performed in two cases. Results One patient required surgical bypass of a thrombosed axillary artery aneurysm. One patient responded to percutaneous transluminal angioplasty (PTA) of a focal axillary artery stenosis. One patient with an axillary artery aneurysm and distal thromboembolic disease was treated by both thrombolysis and surgical thromboembolectomy and later by PTA of an axillary artery intimal disruption. Conclusion Chronic axillary crutch use may be associated with axillary artery stenosis, aneurysm formation, and secondary axillobrachial thromboembolic disease. Mural injury can be successfully treated by PTA and thromboembolic disease by thrombolytic therapy. Early identification of the axillary artery lesion is critical for long-term therapeutic results.

Notes: Abstract A stent-graft was custom made to close a high-flow traumatic arteriovenous fistula of the left superficial femoral artery, present for 30 years, in a 60-year-old man with congestive heart failure and ischemic ulceration in the left foot. A balloon expandable Palmaz stent (P394; 2.5 mm × 3.9 cm) was covered with a polytetrafluoroethylene (PTFE) graft and was inserted percutaneously through an 11 Fr vascular sheath. Follow-up Doppler ultrasound at 6 months demonstrated occlusion of the arteriovenous fistula, patency of the artery, and luminal integrity of the artery and vein.
