    Notes: Abstract A case of internal carotid artery aneurysm with prominent calcification is presented. A 73-year-old woman came to Okayama University Hospital afflicted to Dental School complaining of dry mouth. The panoramic radiograph and the lateral view showed an oval radiopaque body with a smooth margin at the posterior border of the right mandibular ramus. The CT scan showed a spherically-shaped calcification about 3 cm in diameter with liquid content lying medial to the right parotid gland and posterior to the right maxillary bone. Follicular adenoma of thyroid was also found by the CT examination. But the correlation between the calcified body and the tumor was denied by the laboratory data and the histology of the tumor. When the results of the 3D-CT view, the patient's advanced age and the history of renal disease were considered together, the calcified body was thought to be the calcified aneurysm of the internal carotid artery induced by arteriosclerosis. Though it is quite rare for the image of a calcified aneurysm to appear on the radiograph incidental to routine dental radiographic examination, the possibility of a calcified aneurysm also should be taken into consideration whenever localized calcifications are detected.

    Notes: Abstract Thermographic images of 48 cases of malignant tumors were analyzed and compared with those of other lesions: 93 cases of acute inflammation, 48 cases of chronic inflammation, 48 cases of benign tumors and 28 cases of cystic lesions. The majority of the cases with malignant tumors showed higher skin temperature over the affected area, but usually not so remarkable such as those of the cases with acute inflammation. The border of the thermal pattern of the tumors showed the tendency to be unclear. Both malignant tumors and acute inflammation appeared to cause an increase in skin temperature of the common carotid artery region on the affected side as compared with the opposite side. Malignant tumors showed the tendency that the temperature of the common carotid artery region of the affected side is equal or lower than the affected region in many cases. It is suggested that with further sophistication of the system suitable for the maxillofacial region, thermography may serve a role in the clinical diagnosis of malignant tumors.

    Notes: Abstract Fifty-two patients who developed antro-oral fistula following tooth extraction were examined clinically and radiographically. Of these, 39 patients had only perforation of the antral floor and the other 13 had the tooth root pushed into the maxillary sinus. Maxillary sinusitis was present in 43 cases. The most frequent site of antro-oral fistula was the upper first molar, and the palatal root of the upper first molar was pushed into the antrum the most frequently. Our study suggested that most of the patients with antro-oral fistula had associated maxillary sinusitis before the extraction of the tooth. The highest risk of the occurrence of antro-oral fistula appeared to be at the time of extraction of the upper first molar associated with periapical and periodontal lesions.

    Notes: Abstract Oral radiographic findings often provide important clues that lead to early recognition and diagnosis of systemic diseases. The purpose of this study was to identify and analyze the oral radiographic manifestations of various systemic diseases. Conventional radiographs (panoramic, Waters' and dental radiographs) were evaluated in 68 patients with systemic diseases to assess abnormalities retrospectively. Furthermore, high-resolution CT and 3D-CT reformations were available in 2 cases. Various oral radiographic manifestations were present in 36 patients (52.9%): 22 patients with congenital, developmental and hereditary abnormalities; 7 patients with endocrine disturbance, a patient with blood disease and 6 patients with other diseases. The oral radiographic appearance of systemic diseases is considered to provide valuable information for the diagnosis and treatment planning of systemic diseases, especially with regard to abnormalities of the teeth and jaw bones. Furthermore, by using modalities such as CT, further detailed findings can be obtained.

    Notes: Abstract It is thought that the phase of inflammatory extension in osteomyelitis of the mandible varies according to the primary site of infection, that is, the causative tooth. This study was conducted to analyze the relationship between the extension phase of inflammatory bone changes and the causative tooth in patients with radiographically diagnosised osteomyelitis of the mandible. Between 1983 and 1993, a total of 219 patients with osteomyelitis of the mandible were seen in our department. In the age distribution, 135 cases occurred in men and 98 cases in women. Osteomyelitis was most prevalent in patients in their sixties (39 cases) followed by patients in their forties and fifties (38 cases each). The causative tooth was identified in 97 cases of osteomyelitis in the mandible. In the 39 cases in which the primary infection was caused by the third mandibular molar, distal extension was most prevalent (30 cases). In the 58 cases in which a tooth other than the third mandibular molar was the cause (front tooth, premolar, deciduous tooth and other molars), equal extension in the mesio-distal direction was most prevalent (40 cases). In the upper-lower extension phase, lower extension beyond the mandibular canal was most prevalent in the first, second and third molar teeth. In the cases involving teeth other than the mandibular molars, the inflammatory bone changes were rather limited to the upper part of the mandibular canal.

    Notes: Summary A case of tuberculous lymphadenitis with multiple bilateral calcifications in the submandibular and cervical regions has been reported. Radiographic findings revealed that multiple calcifications of various sizes and forms were arranged in rows bilaterally. A possible mode of calcification and infection ways in the lymph nodes was discussed.

    Notes: Summary This is a retrospective study that reviewed 235 malignant cases of the oral and maxillo-facial region in Okayama University Hospital attached to Dental School between April 1982 and March 1988, of these it was found 6 cases of metastatic tumors to the mouth and jaws from a distant area. Malignant tumors of the lung provided the largest number of mouth and jaw metastasis in our series, the appearance of the lesions was as that of adenocarcinomas. Six patients were ranging in age from 42 to 82 years, and 3 were males and 3 were females. The most frequent symptoms observed were swelling and pain. Metastasis to the jaw bones were found predominantly in the mandible, and metastasis to the oral soft tissue occurred in two cases. Metastasis to the mandibular molar region radiographically showed central bone destruction, and metastasis to other regions showed peripheral bone destruction. Biopsy and radionuclide examinations were very useful for diagnosis of the metastatic tumors to the oral and maxillo-facial region.

    Notes: Abstract A case of acute lymphocytic leukemia (ALL) in a child is presented. We also reviewed the recent literature of similar cases describing leukemic jaw bones involvement in Japan. Radiographic examinations of the patient disclosed diffuse osteolytic changes of the entire jaws, the corticated border of the crypts of developing teeth, loss of lamina dura, a thickened periodontal space and disappearance of the radiopaque lines that delineate the borders of the mandibular canal. Pronounced regression and restoration of the alveolar bone and the crypts were detectable on the radiographs taken after a course of the treatment for ALL. An apparent radiographic change of jaw bone after the treatment indicates that the radiographic examinations of children with leukemia in the leukemic process are of quite diagnostic value.
