    Notes: Abstract  The reflex responses to brisk, ramp stretch perturbations of the human flexor pollicis longus muscle (FPL) were recorded during isometric and slow concentric or eccentric contractions at similar levels of muscle excitation. The subjects flexed their thumb to push down against a thumb-rest, whose position was controlled by a servo-controlled motor. In different runs, the stretch perturbations were imposed when the thumb-rest was stationary (isometric) or was flexing or extending the interphalangeal joint of the thumb at a constant velocity, i.e. during concentric or eccentric contractions of FPL. The latency of the most prominent component of the electromyographic reflex in the isometrically contracting muscle was about 60 ms, measured from the command signal. The amplitude of this response was sharply reduced during the non-isometric contractions. While not dependent on the direction, this modulation of the reflex response increased with the speed of active movement of the interphalangeal joint (flexion or extension). The response was greatly reduced during concentric or eccentric movements as slow as 1.6 mm·s–1 (approximately 5°·s–1 at the joint). When the force rather than the position of the thumb-rest was servo-controlled, the stretch response to perturbation again diminished with speed in a self-paced flexion task, compared with an isometric “hold” condition.

    Notes: Background:  Granular parakeratosis is suspected to result from an error in epidermal differentiation, leading to variably pruritic, hyperpigmented-to-erythematous patches and plaques. Characteristic histopathologic features include a thickened stratum corneum, compact parakeratosis with retention of keratohyalin granules, vascular proliferation, and ectasia. The pathogenesis of this entity is uncertain.Methods:  We present a case of axillary granular parakeratosis and review the literature.Results:  The lesion showed a thickened stratum corneum with compact parakeratosis, slight epidermal hyperplasia, and a sparse perivascular lymphohistiocytic infiltrate. Keratohyalin granules were diffusely present within the parakeratotic stratum corneum, and the retained granular layer showed focal vacuolization.Conclusions:  Granular parakeratosis is a rare form of parakeratosis most often seen in the axilla, although other intertriginous areas may be affected. Unique histopathologic findings allow for a specific diagnosis to be made. Although an irritant contact reaction appears causative, mechanical irritation may also play a role in inducing these skin changes.

    Notes: Abstract In prevention work, practices are suggested by theories. Thus, the idea that family-centered prevention is beneficial already embodies a theoretical perspective about why certain approaches to prevention are more appropriate than others. Logic and research evidence both suggest that the family is an indispensable resource in the primary prevention of destructive sexual decisions by adolescents. This paper describes how one project draws upon the family to accomplish primary prevention. An Alternative National Curriculum (AANC) is currently being disseminated in selected public schools in three states, while promoting parent-teen involvement. This project is sponsored by the Office of Adolescent Pregnancy Programs (OAPP), Department of Health and Human Services, which has a legislative mandate under Title XX of the Public Health Service Act to sponsor family-centered demonstration projects that help prevent premarital teen sexual relations and pregnancy. A specific theoretical view of human nature, family life, and societal well-being has been drawn upon to produce AANC. It focuses on ethical and responsible behavior, the importance of family linkages, and how quality family living across generations can be achieved.
