    Notes: Background: The miniaturization of implantable cardioverter defibrillators (ICDs) has made pectoral implantation possible. However, postoperative pain following the procedure has not been systematically studied. The aim of the current study was to prospectively assess patient discomfort and identify factors influencing pain perception during follow-up. Methods: Pain related to device implantation was quantified in 21 consecutive patients (age, 61 ± 11 years; 17 men and 21 women; 16 of 21 had coronary artery disease; left ventricular ejection fraction, 32%± 15%) undergoing pectoral ICD implantation with conscious sedation (fentanyl 118 ± 72 μg, midazolam 14 ± 9 mg). Patients completed the Visual Analogue Scale (VAS, 0–100) and the McGill Pain Questionnaire 24 hours and 1 month postoperatively. Regression analysis was used to define clinical and procedure related variables affecting patient discomfort and frequency of postoperative analgesic use. Results: The mean VAS score was 34 ± 20 24 hours postoperatively. A single (4.8%) patient described postoperative pain as severe. Pain was reported to be moderate by 10 (47.6%) patients and mild by 10 (47.6%) patients. Intraoperative fentanyl requirement was a predictor of postoperative pain (R = 0.51, P = 0.036), and procedural duration was a strong predictor of postoperative analgesic use (R = 0.75, P 〈 0.001). Pain at 1 month decreased to a VAS score of 19 ± 18 (P = 0.002 vs 24 hours) and was rated to be severe, moderate, and mild by 1, 3, and 17 patients, respectively. Late pain was related to a VAS score at 24 hours (R = 0.67, P = 0.004). Conclusions: (1) Pectoral ICD implantation using conscious sedation is well tolerated. (2) Postoperative discomfort correlates with longer procedural times and larger intraoperative narcotic requirements.

    Notes: Retention Wire Fracture. The Telectronics Accufix atrial “J” lead has been associated with injury and death. These complications have occurred as the result of retention wire fracture, extrusion, und perforation of cardiac or vascular structures. This report describes a patient who was found to have protrusion of the proximal end of a nonfractured retention wire during: fluoroscopic surveillance. The wire extrusion appeared to result from angulation of the lead. The lead was extracted and replaced without adverse consequences. Further prospective analysis will be needed to determine the clinical implications and approach to this complication.

    Notes: Two patients presented with monomorphic ventricular tachycardia after blunt chest trauma. In both cases, the arrhythmia had a left bundle branch block, inferior axis morphology comparable to that seen with idiopathic ventricular tachycardia originating from the right ventricular outflow tract (RVOT). In one patient, the arrhythmia persisted and required catheter ablation. A history of cardiac trauma should be considered in patients presenting with RVOT tachycardia.

    Notes: Abstract – WHO basic methods were used to assess CPITN, DMFT, and treatment requirements in a population of 178 persons aged 12–64 yr living in urban and rural areas of Nicaragua. Two groups were examined: individuals presenting for dental treatment at health clinics (30%) and subjects chosen randomly at other locations (70%). Mean ages of both groups were similar although females were found in higher percentages among those presenting for dental care. With the exception of one young individual, all subjects had calculus in at least one sextant. About 26% of the subjects who presented for treatment had a 〈inlineGraphic alt="geqslant R: gt-or-equal, slanted" extraInfo="nonStandardEntity" href="urn:x-wiley:03015661:CDOE190:ges" location="ges.gif"/〉6 millimeter pocket compared to about 14% of the remainder of subjects. The mean DMFT for health clinic subjects was 16.1 compared to 10.3 for subjects from other locations. Subjects presenting for treatment had twice as many missing teeth and a mean of 0.5 filled teeth compared to 2.4 filled teeth for other subjects. Subjects at the clinics were in greater need of restorative care and extractions, while subjects from other locations were found to have more teeth that did not require any treatment. Health clinic subjects reported more visits to the dentist in the last year than other subjects. Only 80% of those presenting for treatment owned a toothbrush compared to 97% of those examined in other locations, and the latter group reported brushing their teeth more frequently. Attrition was a commonly encountered tooth disorder with 53% of all subjects exhibiting this condition.

    Notes: Abstract A rural dental program located on La Gonâve, an island near the coast of Haiti is described along with demographic characteristics of the local population. The oral health of 61 adolescents and young adults seen during screening examinations was assessed. Caries free individuals made up 14.8% of the population. The mean DMFT for the group was 4.20. The OHIS score for the sample was 1.35; the PI score was 0.39. No significant association was found between DMFT, OHIS, PI and sex. One subject had a PI score 5.8 standard deviations from the mean score for the group and exhibited signs suggesting juvenile periodontitis.

    Notes: Summary Chromoblastomycosis is a distinct cutaneous mycotic infection. Histologically it is characterized by the presence of fungus invariably in the yeast phase and by macrophages in large numbers. The morphology of fungus in both yeast and hyphal phases and its interaction with mononuclear phagocytes were examined using ultrastructural techniques. The major structural difference between the fungus in cutaneous tissue and that from a culture medium was a striking increase in cell wall thickness in the former. In the skin, the organisms were phagocytised by macrophages and enclosed in large membrane limited intracytoplasmic vacuoles. The thick, stratified, electron-dense fungal cell wall was observed in stages of alteration of varying severity. Most common was a granular modification of the outermost layers of the cell wall in contact with the vacuolar content. Fragmentation, splitting and rupture of this and deeper layers was also seen. Several ultrastructural features suggested that cell wall damage resulted from an active host cell digestion. The cell wall changes were in sharp contrast with the usual normal fungal cytoplasmic appearance. Only rare intracellular debris which we supposed to represent dead yeast cells were found. These findings suggested that the yeast responsible for chromoblastomycosis resisted fungicidal activities of cutaneous macrophages which possessed the ultrastructural features of stimulated phagocytes.

    Description / Table of Contents: Zusammenfassung Während der letzten 20 Jahre wurde bei 156 Patienten eine direkte und indirekte Aortenisthmusplastik durchgeführt. Frühkomplikationen traten in 7,6% auf. Bei 5 Patienten (3,8%) entwickelte sich als Spätkomplikation ein Aneurysma im Bereich der Isthmusplastik. Die Gesamtletalität betrug 4,5 %. Kontrolluntersuchungen ergaben keinen Anhalt für funktionell kritische Restenosen. Die von Vossschulte entwickelte Aortenisthmusplastik stellt ein chirurgisch elegantes Verfahren der funktionellen Kontinuit ätswiederherstellung mit minimaler Letalität und Restenosegefahr dar.
    Notes: Summary During the last 20 years, 156 patients have received direct and indirect aorthenisthmusplastic. Early complications occurred in 7.6 percent. Five patients developed an aneurysm in the area of the isthmus graft. Lethality was 4.5 percent. There was no evidence of recurrent coarctation in a group of children and adult patients. The indirect aortenisthmusplastic first described by Vossschulte is an elegant surgical method for functional re-establishment of the aortic lumen by retention of the posterior wall for further growth potential. This technique has a low mortality and the danger of recurrent coarctation of the aorta is minimal.
