Summary Tests with insulin, amino acids and insulin with amino acids were made on 4 young men during 3 hours. The following parameters were estimated before and during the experiments: Amino acids (AS), blood sugar (BZ), 11 hydroxycorticosteroids (11-OHCS), free fatty acids (FFS) and free glycerol (FGlyc). Results. 1. Insulin-stress-test. a) The following results already known were confirmed: All the parameter decrease in the initial phase after insulin injektion. A reactive increase of the 11-hydroxycorticosteroids in the plasma results from the hypoglycämia. At the end of the test, blood sugar and 11-hydroxycorticosteroids return to the basal level, the amino acids stay low, whereas the free fatty acids and the free glycerol show a reactive increase upon the basal level. b) Furthermore the results show, that the decrease of the individual amino acids varies and that there is a negative correlation between the prozentual decrease of certain amino acids and that of blood sugar. c) During the alteration of the amino acids, there are several minima over the time in a periodic course. 2. Amino acid-infusion. a) During infusion with amino acids all the plasma amino acids increase to a maximum within 30 minutes. The increase of the individual amino acids is different. After ca. 1 hour, the basal levels were reached again in an intermittent decrease. b) Blood sugar increases slightly until 30 minutes compared to the controls. c) A small decrease of blood sugar is followed with an indicated minimum between 45–60 minutes. At the same time free fatty acids and free glycerol also decrease. d) The infusion of amino acids does not influence the glucocorticoids in the plasma. There is the same decrease in the controls corresponding to the daily rythm. 3. Infusion of amino acids with injection of insulin at the same time. a) Within 30 minutes during simultaneous infusion of amino acids and insulin injection, the increase of almost all essential amino acids is diminished, that of the nonessential amino acids is equal or greater than in the experiments with amino acids infusion alone. b) Up to 60 minutes the rate of amino acid decrease is more quickly. c) 30 minutes after beginning the exepriment, the decrease of blood sugar is diminished by the amino acid infusion compared with insulin alone. At 45 minutes the blood sugar curve changes to the normal increase of that in the insulin-stress-test. d) The reaktive increase of the 11-hydroxycorticosteroids, corresponding to the reduced hyperglycämia is diminished and retarded with gradual differences between three test persons. e) The decrease of free fatty acids is rather like that with insulin alone, but stays a bit longer at 2 test persons. An equal tendency appears with free glycerol. Gluconeogeneses from amino acids in liver and kidney will be discussed as the cause for the relation between the amino acids, blood sugar and 11-hydroxycorticoids. Furthermore a partial inhibition of gluconeogeneses from special amino acids is supposed as a function to insulin.

Zusammenfassung An 4 gesunden jungen Männern wurden Belastungsversuche mit Insulin, mit Aminosäuren, sowie mit Insulin und Aminosäuren über die Zeit von 3 Std durchgeführt. Folgende Parameter wurden vor und im Abstand von 15 bzw. 30 min nach Versuchsbeginn im Plasma (Serum) bestimmt: Aminosäuren (AS), Blutzucker (BZ), 11-Hydroxycorticosteroide (11-OHCS), freie Fettsäuren (FFS) und freies Glycerin (FGlyc). 1. Insulin-Stress-Test. a) Folgende bekannte Befunde wurden bestätigt: Unter Insulin fallen alle Parameter initial ab. Es tritt als Folge der Hypoglykämie ein Anstieg der Glucocorticoide im Plasma auf. In der Spätphase des Versuchs erreichen der Blutzucker und die Glucocorticoide wieder die Ausgangswerte; die Aminosäuren bleiben etwas erniedrigt, während die freien Fettsäuren und das freie Glycerin reaktiv über die Basalwerte ansteigen. b) Weiter wurde gefunden, daß die Abnahme der einzelnen Aminosäuren unterschiedlich ist und zwischen dem prozentualen Abfall bestimmter Aminosäuren und dem Grad der Hypoglykämie eine Beziehung im Sinne einer negativen Korrelation besteht. c) Bei der Veränderung der Plasmaaminosäuren über die Zeit treten mehrere Minima auf. Es deutet sich ein periodischer Verlauf an. 2. Aminosäureinfusion. a) Unter der Aminosäurebelastung kommt es zu einem Anstieg aller Aminosäuren im Plasma mit einem Maximum bei 30 min. Die Größe der Zunahme der einzelnen Aminosäuren variiert, auch prozentual zur infundierten Menge und ist individuell unterschiedlich. In intermittierendem Abfall werden nach ca. 1 Std die Ausgangswerte wieder erreicht. b) Die Blutzuckerwerte sind bei 30 min gegenüber Kontrollwerten leicht erhöht. c) Es folgt ein geringer Abfall des Blutzuckers bis zu einem angedeuteten Minimum zwischen 45 bis 60 min mit gleichzeitiger Abnahme der freien Fettsäuren und des freien Glycerins. d) Die Glucocorticoide im Plasma werden durch die Aminosäureinfusion nicht beeinflußt, sondern nehmen, wie in den Kontrollversuchen, entsprechend dem Tagesrhythmus fortlaufend ab. 3. Aminosäureinfusion mit gleichzeitiger Injektion von Insulin. a) Unter gleichzeitiger Belastung mit Insulin und Aminosäuren ist bei 3 Personen der Konzentrationsanstieg fast aller essentieller Aminosäuren bis zu 30 min prozentual geringer, der der nichtessentiellen Aminosäuren jedoch gleich oder größer als bei alleiniger Aminosäureinfusion. b) Die Abnahmegeschwindigkeit des AS bis zu 60 min ist größer. c) Der Abfall des Blutzuckers wird durch die gleichzeitige Aminosäureinfusion bei Insulinbelastung bis zu 30 min stark verringert. Erst bei ca. 45 min geht der Verlauf des Blutzuckers in die normale Anstiegskurve beim Insulintest über. d) Entsprechend der verminderten Hypoglykämie ist der reaktive Anstieg der 11-Hydroxycorticosteroide geringer und verzögert mit deutlichen graduellen Unterschieden bei den einzelnen Personen. e) Der Konzentrationsabfall der freien Fettsäuren entspricht etwa dem bei alleiniger Insulininjektion, bleibt jedoch bei 2 Personen länger bestehen. Eine ähnliche Tendenz zeigt sich bei dem freien Glycerin. Als Ursache der Beziehungen zwischen den Plasma-Aminosäuren, dem Blutzucker und den 11-Hydroxycorticosteroiden wird die Gluconeogenese aus Aminosäuren in Leber und Niere diskutiert. Es wird weiter eine teilweise Verminderung der Gluconeogenese aus bestimmten Aminosäuren durch die Injektion von Insulin angenommen.
