    Notes: Summary Sequential biopsies taken from a patient with a solitary giant xanthogranuloma, an exaggerated macronodular (〉5 cm in diameter) variant of juvenile xanthogranuloma, and from a patient with benign cephalic histiocytosis, revealed a characteristic time sequence of histopathological findings. Early stages of the diseases showed a monomorphous infiltrate of mononuclear vacuolated histiocytes positive for KiM1p. HAM56 and factor XIIIa and were characterized by clusters of comma-shaped bodies. This was followed by a polymorphous mixture of various mononuclear and mullinucleate histiocytes additionally labelling with KP1 (CD68) and, in occasional cells, for the adherence of peanut agglutinin. A variety of ultrastructural changes were found, including dense and regularly laminated bodies or lipid droplets. Our findings indicate that both entities are variants of a xanthogranulomatous reaction.

    Notes: Summary Distant metastases from occult papillary carcinoma of the thyroid, which is defined as a tumor less than 15 mm in diameter, are extremely rare. A 21-year-old patient with miliary micronodular densities in both lungs is described, in whom pulmonary lymphangitic spread of occult papillary thyroid carcinoma was diagnosed by thransthoracic lung biopsy.

    Description / Table of Contents: Zusammenfassung Es wird über zwei Beobachtungen mit Gefäßveränderungen berichtet, die im Rahmen der generalisierten Recklinghausenschen Neurofibromatose bzw. bei solitären Neurofibrom aufgetreten sind. Diese selten beobachtete Manifestation der Neurofibromatose beruht wahrscheinlich auf einer Proliferation des gefäßeigenen Nervengewebes und wurde von Feyrter „vasculäre Neurofibromatose“ genannt. Die Gefäßwand ist meist schwer deformiert und die Lichtung eingeengt. Der Befall von verschiedenen Gefäßschichten kommt in der Einteilung in fünf verschiedenen Gruppen zum Ausdruck. Morphologie und Klinik werden diskutiert.
    Notes: Summary Feyrter suggested the term “vascular neurofibromatosis” for rarely described vessel-wall changes in generalized neurofibromatosis. These changes, followed by severe alteration of the wall and obliteration of the lumen, probably are produced by a proliferation of nerve fibers in the wall of the blood vessels. The classification into 5 different forms, as proposed by Feyrter and Reubi, is based on the different localization of the process in the vessel wall. The general morphological and clinical features of two recently observed cases of this rarely diagnosted disease are discussed.

    Description / Table of Contents: Zusammenfassung Um die Natur der umstrittenen großen, eosinophilen Zellen mit PAS-positiven Granula im Cytoplasma zu klären, wurden 2 Fälle von sog. granulomatöser Orchitis elektronenoptisch untersucht. Es wird nämlich seit der ersten Beschreibung (Gruenberg, 1926) diskutiert, ob diese Zellen Sertolischer oder reticulocytärer Herkunft sind. Die elektronenoptische Untersuchung beider Fälle ergab, daß eine morphologische und funktionelle Ähnlichkeit dieser umstrittenen Zellen mit Makrophagen besteht. Für Sertolizellen charakteristische Strukturen wurden nicht gefunden. Die PAS-positiven Granula haben sich als große Phagosomen bzw. Restkörper erwiesen. Einen interessanten Befund stellen die zahlreichen phagocytierten Bakterien deshalb dar, weil bisher Bakterien bei der granulomatösen Orchitis nie eindeutig nachgewiesen wurden. Auch Calcospherulen wurden bei einem der untersuchten Fälle gefunden. Es wird die Frage diskutiert, ob die granulomatöse Orchitis nur eine gewisse Ähnlichkeit mit der Malakoplakie aufweist, oder ob es sich um zwei morphologisch und auch ätiopathogenetisch identische Krankheiten handelt.
    Notes: Summary Two cases of so-called granulomatous orchitis were investigated by means of electron microscopy to clarify the nature of the contested large eosinophilic cells with PAS-positive granules in the cytoplasm. Since the first description by Gruenberg (1926), there has been some discussion as to whether these cells are of “sertolic” or reticulohistiocytic origin. Electron microscope examination has proved a morphologic as well as a functional similarity between these contested cells and macrophages. Structures characteristic of Sertoli cells have not been found. The PAS-positive granules turned out to be large phagosomes or residual bodies. One important observation is the numerous phagocyted bacteria in both cases; bacteria have never previously been demonstrated in this disease. Furthermore, calcium-containing inclusions (calcospherules) such as occur in malakoplakia of the bladder and testes were found. The question as to whether there is any difference between granulomatous orchitis and malakoplakia is discussed.

    Notes: Abstract Detailed investigation of cell growth and nucleolar organizer region associated argyrophilic proteins (Ag-NORs) is necessary to asses a possible impact of Ag-NOR quantification on the diagnosis and prognosis of tumours. In this study, cellular proliferation of the transitional-cell carcinoma cell line HOK-1 was modulated over a period of 11 days by starvation and subsequent medium addition. Proliferation was determined daily by DNA flow cytometric estimation of S-phase fraction (SPF) and mitotic index (MI) calculation. The number and area of interphase Ag-NORs were quantified by automated image analysis daily and the number of Ag-NOR bearing chromosomes in metaphase was counted. In interphase nuclei, Ag-NOR area showed a highly significant correlation with SPF (p〈0.0001) whereas interphase Ag-NOR number showed significant correlation with MI (p〈0.05). A positive relationship between the number of Ag-NOR bearing chromosomes in metaphases and cellular proliferation was also observed. There is variability in Ag-NOR quantity during interphase and metaphase depending on growth conditions in vitro. Correlations of the number of interphase Ag-NORs with the MI on one hand and Ag-NOR area with SPF on the other provide further evidence that distribution and quantity of Ag-NORs are strongly influenced by the cell cycle phase within the structural-functional unit of the nucleolus.

    Description / Table of Contents: Zusammenfassung Silastic-Implantate, die in der plastischen Chirurgie verwendet werden, verursachen fallweise im angrenzenden Gewebe chronische Entzündungen, die zu einer Wucherung des Bindegewebes und Verformung der Prothese führen. In Dünnschnitten und gefriergetrockneten Gewebepräparaten wurde mittels Kombination von SE-Rasterbildern, Si K a -Verteilungsbildern und energiedispersiven Punktanalysen gezeigt, daß innerhalb bestimmter Zellen (Makrophagen) Si-Anreicherungen vorkommen, die anμm-große Partikel gebunden sind. Daneben wurden auch extrazelluläre Si-Anreicherungen festgestellt, die keinen Fremdkörpern im Gewebe zugeordnet werden können. Mit der Mikrosonde konnte bewiesen werden, daß Makrophagen Silastic-Teilchen aufnehmen und abbauen, so daß Siliciumanreicherungen im Gewebe zurückbleiben. Im anderen Fall wurde Thorium in Leber und Milz von Patienten nachgewiesen, denen Thorotrast als Kontrastmittel für eine Röntgenuntersuchung injiziert worden war. Th wird in Leber, Milz und Knochenmark gespeichert und verursacht Tumore und Zirrhosen. Wie gezeigt werden konnte, erfolgt die Speicherung des Th in der Leber vorwiegend im Bindegewebe.
    Notes: Summary In two areas of medical research, the electron microprobe has proved to be a useful tool. The most attractive feature of a scanning electron microprobe quantometer in medicine is the possibility of simultaneous observation and X-ray analysis of domains in their original environment of cells and tissue. In the present study, microprobe analysis was used to identify foreign elements not normally present in the human body. Thorium has been found previously in the liver by chemical methods, but the usefulness of the microprobe was to locate the areas of thorium enrichment within the connective tissue. “Silastic”-implants used in plastic and reconstructive surgery frequently cause complications in the adjacent tissue known as constrictive fibrosis. Electron microprobe investigations demonstrated Si-enrichments in the tissue sourrounding “Silastic”-prostheses obviously deriving from the implant. Constrictive fibrosis is therefore caused by Silicon infestation in the tissue deriving from the prosthesis owing to processes which are not yet known.

    Notes: Summary Exocrine testicular function was determined in 30 prepubertal and pubertal patients who had undergone detorsion and fixation for unilateral torsion of the testis, in 46 men who had undergone unilateral orchidopexy, and in 16 patients with unilateral absence of the testis. In 42 patients with seminomas and non-seminomatous testicular tumors, sperm analyses were performed before orchiectomy. The semen analyses in patients with unilateral cryptorchidism were normal in 13, doubtful in 23 and pathological in 10. The semen quality in patients with torsion was normal in 15, doubtful in 3 and pathological in 12. Of the 16 patients with monorchidism, 14 had normal sperm analyses. Of the 42 patients with seminomas and non-seminomatous testicular tumors, only two patients with HCG-negative seminomas had normal sperm analyses before orchiectomy. In patients with doubtful and pathological sperm analyses after testicular torsion and unilateral orchidopexy, elevated follicle-stimulating-hormone levels were observed. HCG-positive testicular tumors showed decreased FSH levels but elevated 17-β-estradiol and prolactin levels.

    Notes: Summary Histologic differentiation, tumor volume and survival rate show that incidental prostatic carcinoma is not a uniform tumor. A retrospective study of 169 consecutive patients with incidental carcinoma was performed; stage and histologic pattern are shown for 135 patients; in 105 patients stage and histologic differentiation pattern are compared to the 5- and 10-year survival rates. In a small series of patients a morphometric analysis was performed to determine the relative and absolute amounts of prostatic carcinoma cells in transurethral prostatic specimens. The histologic pattern in stage A1 (= focal) disease was well differentiated in 62% and moderately differentiated in 38%. Patients with diffuse (= A2) disease showed a well differentiated histologic pattern in only 13%, a moderate differentiated in 35% and a poorly differentiated in 52%. The overall 10-year-survival rate in patients with stage A1 disease was calculated to be 59%, that of stage A2 lesions 40%. Depending upon the histologic pattern in stage A2 patients there is a significant difference in the 10-year-survival rate (A2 G1: 63%, A2 G2: 49%, A2 G3: 28%). Regarding the relative and absolute tumor amounts the morphometric analysis shows as significant difference between A1 and A2 lesions.
