    Notes: Summary Distant metastases from occult papillary carcinoma of the thyroid, which is defined as a tumor less than 15 mm in diameter, are extremely rare. A 21-year-old patient with miliary micronodular densities in both lungs is described, in whom pulmonary lymphangitic spread of occult papillary thyroid carcinoma was diagnosed by thransthoracic lung biopsy.

    Notes: Summary Sequential biopsies taken from a patient with a solitary giant xanthogranuloma, an exaggerated macronodular (〉5 cm in diameter) variant of juvenile xanthogranuloma, and from a patient with benign cephalic histiocytosis, revealed a characteristic time sequence of histopathological findings. Early stages of the diseases showed a monomorphous infiltrate of mononuclear vacuolated histiocytes positive for KiM1p. HAM56 and factor XIIIa and were characterized by clusters of comma-shaped bodies. This was followed by a polymorphous mixture of various mononuclear and mullinucleate histiocytes additionally labelling with KP1 (CD68) and, in occasional cells, for the adherence of peanut agglutinin. A variety of ultrastructural changes were found, including dense and regularly laminated bodies or lipid droplets. Our findings indicate that both entities are variants of a xanthogranulomatous reaction.

    Notes: Summary The Silicon Elastomers, which are widely being used in Plastic and Reconstructive Surgery cause a constrictive fibrosis to a varying degree. The intensity determinations of Silicon radiation show that all capsules around those implants contain intra- and extracellular particles, deriving from the implants. In order to measure the suitability of implant materials manufactured from Silicon Elastomers as well as to test the tissue reactions of different implantation sites, this microchemical technique appears appropriate. A model study of mammary type implants in rabbits revealed that the capsules around Silica free “Barrier” coated implants contained twice as much Silicon as the non coated standard material, that inflatable spheres produced only one third of the Silicon infestation compared to the gel-filled pillows and that the intensity of the phagocytosis of the Silicon Elastomer implant material in the muscle is only one quarter compared to the tissue reactions occuring in the subcutaneous layer.

    Notes: Summary One year after augmentation mammaplasty the axillary lymph nodes on both sides were found to be heavily loaded with material deriving from the intact gel-filled retromammary prostheses. The histologic and electron microscopic findings were confirmed by the microchemical analyses (Si K-α-radiation). The route of lymphatic drainage of the material and the differential diagnosis from other storage diseases are discussed and the importance of further observation of distant scattering of Silicone implant particles is emphasized.

    Notes: Summary Exocrine testicular function was determined in 30 prepubertal and pubertal patients who had undergone detorsion and fixation for unilateral torsion of the testis, in 46 men who had undergone unilateral orchidopexy, and in 16 patients with unilateral absence of the testis. In 42 patients with seminomas and non-seminomatous testicular tumors, sperm analyses were performed before orchiectomy. The semen analyses in patients with unilateral cryptorchidism were normal in 13, doubtful in 23 and pathological in 10. The semen quality in patients with torsion was normal in 15, doubtful in 3 and pathological in 12. Of the 16 patients with monorchidism, 14 had normal sperm analyses. Of the 42 patients with seminomas and non-seminomatous testicular tumors, only two patients with HCG-negative seminomas had normal sperm analyses before orchiectomy. In patients with doubtful and pathological sperm analyses after testicular torsion and unilateral orchidopexy, elevated follicle-stimulating-hormone levels were observed. HCG-positive testicular tumors showed decreased FSH levels but elevated 17-β-estradiol and prolactin levels.

    Notes: Summary Since fertility is decreased in patients with unilateral torsion, a detrimental influence of the damaged tesis on the contralateral side has been postulated. In previous studies, we could not detect such an effect 2 months after the application of torsion. The present study was designed to search for an early defect that may be followed by regeneration as well as for a defect that appears only later. At 2 weeks, 4 months and 7 months after unilateral testicular torsion and sham operation, respectively, in rats, both testes were examined. By means of morphometry, the different testicular components of the contralateral testes were quantified, but neither early nor late damage could be detected. Therefore, despite some other conflicting studies, we do not believe that contralateral testicular damage necessarily occurs after torsion. The decreased fertility may be explained by an immunologic attack on the spermatozoa on their way out of the genital tract.

    Notes: Summary Tissue reactions around various silicone implants, contaminated by Staphylococcus albus and bacillus subtilis, have been studied in rabbits by Silicone Radiation Measurements. No increase of phagocytosis was found around the contaminated materials compared to the non-contaminated implants. The morphology of the capsule was the same in the contaminated and the non-contaminated groups. Concerning the specific tissue reaction in silicone implants, a prophylactic use of antibiotics in clinical augmentation mammaplasty seems not mandatory.

    Notes: Summary In 15 rats, silastic implants were replaced by bone, cartilage, and dermal grafts after 6 to 8 weeks. At the same time, part of these grafts were also implanted outside the sheaths. The grafts were examined 6 to 9 months later. Within the silastic induced sheaths, only the vitality of the dermal grafts was impaired to any degree. Bone and cartilage took as well inside the induced sheaths as in a normal wound-bed. Since there were only thin membranes of the normal reaction type I observed in all animals, after 6 to 8 weeks, no conclusion can be drawn concerning the behaviour of the grafts within a thicker, more severe reaction type sheath.

    Notes: Summary Constrictive fibrosis is a frequent and often grave complication of mammary augmentation with Silicone implants. In addition to tension or pressure, which lead to local callus formation and the rare apathogenic infection which is followed by tissue reaction, constrictive fibrosis is initiated by phagocytosis of Silicone particles from the surface of the implants. Shape, position, and consistency of the augmented breasts, which have been impaired by constrictive fibrosis, can be restored by capsulotomy, and if necessary even by repeated capsulectomies. Further development, selection, and manufacture of more suitable plastic material may prevent constrictive fibrosis in the future.

    Notes: Summary The tissue reaction of 4 different implant materials has been compared by Silicon Radiation Measurements in rabbits in order to evaluate the biocompatibility of the in-situ vulcanized Silastic 382. Although the preoperatively vulcanized Silastic 382 elicits less phagocytosis than the in-situ vulcanized Silastic 382, the radiation rates of the latter are similar to the rates obtained from the capsules around gel-filled mammary type implants, thus indicating an equal rating for the in-situ vulcanized RTV Silastic 382. This suggests that phagocytic activity following use of the in-situ Polymerisation technique in masking a pectus excavatum is not prohibitive.
