    Notes: Summary Sequential biopsies taken from a patient with a solitary giant xanthogranuloma, an exaggerated macronodular (〉5 cm in diameter) variant of juvenile xanthogranuloma, and from a patient with benign cephalic histiocytosis, revealed a characteristic time sequence of histopathological findings. Early stages of the diseases showed a monomorphous infiltrate of mononuclear vacuolated histiocytes positive for KiM1p. HAM56 and factor XIIIa and were characterized by clusters of comma-shaped bodies. This was followed by a polymorphous mixture of various mononuclear and mullinucleate histiocytes additionally labelling with KP1 (CD68) and, in occasional cells, for the adherence of peanut agglutinin. A variety of ultrastructural changes were found, including dense and regularly laminated bodies or lipid droplets. Our findings indicate that both entities are variants of a xanthogranulomatous reaction.

    Description / Table of Contents: Zusammenfassung Es wird über zwei Beobachtungen mit Gefäßveränderungen berichtet, die im Rahmen der generalisierten Recklinghausenschen Neurofibromatose bzw. bei solitären Neurofibrom aufgetreten sind. Diese selten beobachtete Manifestation der Neurofibromatose beruht wahrscheinlich auf einer Proliferation des gefäßeigenen Nervengewebes und wurde von Feyrter „vasculäre Neurofibromatose“ genannt. Die Gefäßwand ist meist schwer deformiert und die Lichtung eingeengt. Der Befall von verschiedenen Gefäßschichten kommt in der Einteilung in fünf verschiedenen Gruppen zum Ausdruck. Morphologie und Klinik werden diskutiert.
    Notes: Summary Feyrter suggested the term “vascular neurofibromatosis” for rarely described vessel-wall changes in generalized neurofibromatosis. These changes, followed by severe alteration of the wall and obliteration of the lumen, probably are produced by a proliferation of nerve fibers in the wall of the blood vessels. The classification into 5 different forms, as proposed by Feyrter and Reubi, is based on the different localization of the process in the vessel wall. The general morphological and clinical features of two recently observed cases of this rarely diagnosted disease are discussed.

    Notes: Abstract  An extranodal high-grade B-cell lymphoma, centroblastic type, with exclusive involvement of the heart, stomach and small bowel was detected at post-mortem examination following the death of an 80-year-old man. Autopsy revealed massive cardiomegaly with a total heart weight of 1800 g owing to an intramyocardial tumour involving the right ventricle, and multiple mucosal tumour plaques and nodules in the stomach and small bowel. The case highlights the difficulties of diagnosing cardiac lymphoma clinically even in the presence of a large tumour mass.

    Notes: Abstract The intermediate filament of mature human Sertoli cells is vimentin. A co-expression of vimentin together with cytokeratin has been demonstrated in Sertoli cells during embryonal development and under pathologic conditions in adult testes. We analysed the presence of vimentin, cytokeratin, and desmin in Sertoli cells of fetal testes (n=20), in seminiferous tubules of cryptorchid testes (n=10) and adjacent to testicular germ cell tumours (n=47) using specific monoclonal antibodies and single and double-labelling immunohistochemistry. During embryonal development prominent cytokeratin expression disappears after the 20th week of gestation. Interestingly, we also found desmin in immature intratubular Sertoli cells between weeks 11 and 14. In adult cryptorchid testes and in peritumour tubules, desmin was also prominently present in Sertoli cells in the vast majority of the cases investigated, as well as vimentin and cytokeratin co-expression. This first description of desmin immunoreactivity may shed some light on the ontogeny of human Sertoli cells and demonstrates that this cell type is able to express three types of intermediate filaments in a complex manner.

    Description / Table of Contents: Zusammenfassung Um die Natur der umstrittenen großen, eosinophilen Zellen mit PAS-positiven Granula im Cytoplasma zu klären, wurden 2 Fälle von sog. granulomatöser Orchitis elektronenoptisch untersucht. Es wird nämlich seit der ersten Beschreibung (Gruenberg, 1926) diskutiert, ob diese Zellen Sertolischer oder reticulocytärer Herkunft sind. Die elektronenoptische Untersuchung beider Fälle ergab, daß eine morphologische und funktionelle Ähnlichkeit dieser umstrittenen Zellen mit Makrophagen besteht. Für Sertolizellen charakteristische Strukturen wurden nicht gefunden. Die PAS-positiven Granula haben sich als große Phagosomen bzw. Restkörper erwiesen. Einen interessanten Befund stellen die zahlreichen phagocytierten Bakterien deshalb dar, weil bisher Bakterien bei der granulomatösen Orchitis nie eindeutig nachgewiesen wurden. Auch Calcospherulen wurden bei einem der untersuchten Fälle gefunden. Es wird die Frage diskutiert, ob die granulomatöse Orchitis nur eine gewisse Ähnlichkeit mit der Malakoplakie aufweist, oder ob es sich um zwei morphologisch und auch ätiopathogenetisch identische Krankheiten handelt.
    Notes: Summary Two cases of so-called granulomatous orchitis were investigated by means of electron microscopy to clarify the nature of the contested large eosinophilic cells with PAS-positive granules in the cytoplasm. Since the first description by Gruenberg (1926), there has been some discussion as to whether these cells are of “sertolic” or reticulohistiocytic origin. Electron microscope examination has proved a morphologic as well as a functional similarity between these contested cells and macrophages. Structures characteristic of Sertoli cells have not been found. The PAS-positive granules turned out to be large phagosomes or residual bodies. One important observation is the numerous phagocyted bacteria in both cases; bacteria have never previously been demonstrated in this disease. Furthermore, calcium-containing inclusions (calcospherules) such as occur in malakoplakia of the bladder and testes were found. The question as to whether there is any difference between granulomatous orchitis and malakoplakia is discussed.

    Notes: Abstract Detailed investigation of cell growth and nucleolar organizer region associated argyrophilic proteins (Ag-NORs) is necessary to asses a possible impact of Ag-NOR quantification on the diagnosis and prognosis of tumours. In this study, cellular proliferation of the transitional-cell carcinoma cell line HOK-1 was modulated over a period of 11 days by starvation and subsequent medium addition. Proliferation was determined daily by DNA flow cytometric estimation of S-phase fraction (SPF) and mitotic index (MI) calculation. The number and area of interphase Ag-NORs were quantified by automated image analysis daily and the number of Ag-NOR bearing chromosomes in metaphase was counted. In interphase nuclei, Ag-NOR area showed a highly significant correlation with SPF (p〈0.0001) whereas interphase Ag-NOR number showed significant correlation with MI (p〈0.05). A positive relationship between the number of Ag-NOR bearing chromosomes in metaphases and cellular proliferation was also observed. There is variability in Ag-NOR quantity during interphase and metaphase depending on growth conditions in vitro. Correlations of the number of interphase Ag-NORs with the MI on one hand and Ag-NOR area with SPF on the other provide further evidence that distribution and quantity of Ag-NORs are strongly influenced by the cell cycle phase within the structural-functional unit of the nucleolus.

    Description / Table of Contents: Zusammenfassung 10 Ganglien wurden lichtmikroskopisch und elektronenmikroskopisch untersucht. In der Wand der Ganglien fanden sich lichtmikroskopisch ähnliche Veränderungen wie bei der Hygromentstehung. Elektronenoptisch zeigte sich eine spezielle Differenzierung von Mesenchymzellen zu synovialisähnlichen Zellen, die einzeln im Bindegewebe der Wand und als auskleidende Schicht der Ganglionhöhle auftreten. Die Entwicklung einer Synovialisschicht aus unspezifischen Bindegewebszellen wird auf die charakteristische funktionelle Belastung im Bereich von Gelenken zurückgeführt. theorien, über die Tumorgenese (Ledderhose, 1893), zur Annahme eines traumatisch bedingten Entartungsvorganges (Payr, 1898; Soren, 1966). Licht-, immunofluoreszenz-und elektronenmikroskopische Untersuchungen an mehreren Ganglien sollen einen Beitrag zur Ganglienfrage bringen.
    Notes: Summary 10 ganglia were studied by means of light and electrone microscope and with immunfluorescence methods. The light microscopic findings in the wall of a ganglion resemble on developing hygroma. The mesenchymal cells ultrastructurally show differentiation to synovia-like cells, either surrounding the ganglion in the connective tissue wall or lining the cavity. The development of synovial tissue from undifferentiated mesenchymal cells is due to the typical mechanical stimulus affecting joints.

    Description / Table of Contents: Zusammenfassung Es wird über ein Histiocytom am linken Talus berichtet. Nach Auftreten eines Rezidives mit einem Infekt nach versuchter Operation kam der Patient an unsere Klinik. Der Tumor war als Metastase eines malignen Melanoms aufgefaßt worden. Die endgültige histologische Diagnose konnte nach Durchsicht der ursprünglichen histologischen Schnitte gestellt werden. Es mußte daher eine Knieexartikulation durchgeführt werden und der Patient einer adjuvanten Chemotherapie unterzogen werden.
    Notes: Summary A malignant histiocytoma of the left talus is described. The patient was introduced to our hospital because of an infected recidivation of a partly exstirpated tumor. It was classified as a metastasis of a malignant melanoma. The final histologic diagnosis could be proved by the original slides. The exarticulation of the knee was followed by chemotherapy.
