    Notes: Summary In spite of dietary treatment, the infants of pregnant patients with abnormal glucose tolerance have hyperinsulinism and diabetogenic fetopathy in 10 to 36% of cases. Those patients, who require insulin to prevent from fetopathy cannot be reliably selected by maternal parameters such as blood glucose and glycosylated hemoglobin values. We recommend the measurement of amniotic fluid insulin between the 28 and 32 weeks of pregnancy to differentiate whether the fetus is compromised or not. Subjects with values above the 97th centile require insulin therapy. Inadequate insulin dosage or delayed fetal hyperinsulinism can be discovered by checking the amniotic fluid insulin level at 33 to 36 weeks. In a total of 88 gestational diabetic patients 19 had raised amniotic fluid insulin levels indicating the onset of diabetic fetopathy at an early stage. Diabetic patients with raised amniotic fluid insulin levels needed large doses of insulin, namely 64.6 ± 29.5 (Mean ± SD) U/24 h. This treatment reduced mean blood glucose levels from 98 ± 9 (Mean ± SD) mg/dl to 82 ± 10 mg/dl and was sufficient to prevent from diabetic fetopathy.

    Notes: Abstract. The paper discusses the relationship between labor economics' segmentation theory and the polarization theories of regional economics. It is argued that labor market segmentation and regional disparities are fundamentally related and that one is both cause and consequence of the other. The argument is developed around the locational requirements of various functions of entreprises on the one hand and worker's schooling and migration decisions on the other. As can be shown, interregional labor mobility that is usually considered to be an important factor in equilibrating regional disparities enforces the interregional differentiation in a model allowing for a segmented labor market. The paper closes with reference of a few fundamental hypotheses that can be derived from the model and cites empirical evidence supporting this view.

    Notes: Objective To establish cut off levels for oral glucose tolerance test in pregnancy using fetal hyperinsulinism as a clinical endpoint.Design Capillary blood glucose levels at 0, 1, and 2 hours after the ingestion of either 1 g/kg or 75 g glucose, at 28 (SD 5) weeks of gestation were analysed in 220 women with elevated amniotic fluid insulin levels [≥ 42 pmol/L (≥ 7 μU/mL)] after a mean (SD) of 31 weeks (3) and in 220 nondiabetic controls.Results In women with elevated amniotic fluid insulin levels the mean (SD) capillary blood glucose values at 0, 1, and 2 hours were 5.2 mmol/L (1.0) [94 mg/dL (18)], 10.5 mmol/L (1.4) [189 mg/dL (25)] and 8.2 mmol/L (2.0) [147 mg/dL (36)], respectively. The one-hour value had the highest sensitivity to predict elevated amniotic fluid insulin levels. The 5th centile of the one-hour blood glucose levels representing a detection rate of 95% was 8.9 mmol/L (160 mg/dL).Conclusion Glucose cut off levels in most established oral glucose tolerance test criteria are too high, to accurately predict amniotic fluid hyperinsulinism. A one-hour test may be sufficient for detecting amniotic fluid hyperinsulinism. Since different loads (1 g/kg, 75 g or 100 g) and blood fractions (venous plasma or capillary blood) have minimal impact on oral glucose tolerance test results, a single one-hour cut off of 8.9 mmol/L (160 mg/dL), independent of the sampling method, may be appropriate for the diagnosis of gestational diabetes mellitus severe enough to cause amniotic fluid hyperinsulinism.

    Notes: Abstract This paper investigates behavioural differences between non-profit organisations (NPOs), government organisations (GOs), and private market organisations (PMOs) as suppliers of nursery schools and retirement homes. On the basis of Weisbrod's undersupply model, the differentiated demand approach, and the public finance argument on the redistributive role of government, three hypotheses are theoretically specified and empirically tested: client differentiation; product differentiation; and price differentiation. NPOs and PMOs are shown to provide qualitatively better services and charge significantly higher prices than GOs. A clustering of clients with similar socio-economic backgrounds can be observed: GOs provide services for clients with lower economic and social status than private suppliers. GOs, holding more than two-thirds of the respective markets, are responsible for a ‘basic provision’ of services; they pursue distributional goals by producing services themselves; only rarely a ‘contracting out’ to private suppliers can be noted.

    Notes: Abstract The Combined Method is a non-parametric regression technique for long-term prediction of smoothed monthly sunspot numbers. Starting from a solar minimum, a prediction of the succeeding maximum is obtained by using a dynamo-based relation between the geomagnetic aa index and succeeding solar maxima. Then a series of predictions is calculated by computing the weighted average of past cycles of similar level. This technique leads to a good prediction performance, particularly in the ascending phase of the solar cycle where purely statistical methods tend to be inaccurate. For cycle 23 the combined method predicts a maximum of 160 (in terms of smoothed sunspot number) early in the year 2000.

    Notes: Zusammenfassung Gesetzliche Regelungen und direkte staatliche Eingriffe im Bereich der Arbeitswelt, des Verkehrs oder der Umwelt haben in einem großen Ausmaß die Vermeidung oder Reduzierung von Krankheits- und Unfallrisken zum Ziel. Nicht selten verläuft die öffentliche Diskussion über die Beurteilung dieser Maßnahmen im außerökonomischen Raum unter (bewußtem oder unbewußtem) Verzicht auf die Offenlegung der Kosten-Nutzen-Aspekte. Doch gerade die Umweltproblematik demonstriert deutlich die Aktualität der Frage, anhand welcher Kriterien die politischen Entscheidungsträger derartige Maßnahmen bewerten sollen. In dieser Arbeit wird die Frage der Bewertung von Sicherheitsmaßnahmen aus der Sicht des Arbeitsmarktes aufgegriffen. Nach einer theoretischen Einleitung gibt sie einen Überblick über verschiedene methodische Konzepte zur Quantifizierung des Nutzens von Maßnahmen der Arbeitsplatzsicherheit. Im empirischen Teil wird auf der Basis von Arbeitsmarktdaten aus dem Mikrozensus 1981 und nach Wirtschaftszweigen gegliederten Unfallstatistiken öffentlicher Versicherungsanstalten die implizite Bewertung des Arbeitsplatzrisikos geschätzt. Hiefür wird die sogenannte “Hedonic-price”-Methode angewandt, d. h. die impliziten Preise von Arbeitnehmer- und Arbeitsplatzcharakteristika werden aus beobachteten Daten auf dem Arbeitsmarkt mit Hilfe von Regressionsschätzungen ermittelt. Aus den Koeffizienten der Risikovariablen kann über den Betrag Aufschluß gewonnen werden, den, den die Gesellschaft für die Bereitstellung von Sicherheitsmaßnahmen zur Verminderung des Unfallrisikos zu zahlen bereit ist. Die Ergebnisse weisen darauf hin, daß auch in Österreich, trotz der in der politischen Diskussion dominierenden Auffassung, erhöhtes Arbeitsplatzrisiko sei nicht durchmonetäre Entschädigungen abzugelten, eine implizite Beziehung zwischen Lohnhöhe und Arbeitsplatzrisiko besteht. Weiters zeigt sich, daß eine Bewertung von Sicherheitsmaßnahmen anhand des gesamtwirtschaftlichen Einkommensentgangs aufgrund des Produktionsausfalls, der durch die Nichtbereitstellung dieser Sicherheitsvorkehrungen entstünde, die gesellschaftliche Bewertung dieser Maßnahmen schwerwiegend unterschätzt.
