    Notes: Percutaneous tracheostomy is a well established technique used primarily to assist weaning from mechanical ventilation on many intensive care units. We report our experiences of a total of 36 procedures performed with the new Blue Rhino™ Percutaneous Tracheostomy Introducer Set developed by Ciaglia. The technique was successful in all cases and was simpler and quicker to perform than with the earlier Ciaglia percutaneous tracheostomy set. Difficulties were encountered when using Shiley™ tracheostomy tubes. Significant complications included one posterior wall tear and one tracheal cartilage ring fracture.

    Notes: The production of macrophage activation factor (MAF) by rainbow trout, Oncorhynchus mykiss (Walbaum), head kidney leucocytes was examined after culturing in vitro with extracellular products (ECP) collected from Mycobacterium sp. Cultures of leucocytes were prepared from naive fish, or fish previously vaccinated with either the ECP or with formalin killed whole cell preparations (WC) of the bacterium. The cells were then incubated with the ECP in vitro and the ability of their supernatants to activate macrophages assessed. Macrophages from control fish were incubated with the supernatants, and their ability to reduce nitroblue tetrazolium (NBT) measured as an indicator of macrophage activation. Incubation of head kidney macrophages from naive fish directly with 1, 10 or 100 μg mL–1 of ECP for 48 h significantly enhanced macrophage activation compared with control macrophages. Vaccination of fish with either ECP or WC had no significant effect on the respiratory burst of control macrophages 4 weeks post-vaccination. By the eighth week, however, absorbance levels of respiratory burst reflecting both the primary (cells from vaccinated fish cultured in vitro with PBS) and the secondary (cells cultured in vitro with ECP) MAF responses of fish vaccinated with ECP and WC, had peaked and these were significantly different from the non-vaccinated controls. This activity had fallen to levels similar to control fish by week 12 for fish vaccinated with WC.

    Notes: Five monoclonal antibodies (MAbs: F2B1, 1E4, 13B10, 4D4 and F3G12) were produced against lysed Photobacterium damselae ssp. piscicida (Ph. d. ssp. piscicida). The MAbs recognized three antigens of differing molecular weight on the Western blot of Ph. d. ssp. piscicida. They also cross-reacted with five different species of Vibrio. An enzyme linked immunosorbent assay (ELISA) with MAbs, F3G12 and 4D4 demonstrated differences between Ph. d. ssp. piscicida and three Ph. d. ssp. damselae type strains, indicating differences in the surface antigenicity between these two groups of bacteria. Antigen retrieval in conjunction with immunohistochemistry (IHC) using MAb 13B10, revealed colonies of bacteria in the kidney, spleen and liver of sea bass, Dicentrarchus labrax, infected with pasteurellosis. A number of positive colonies were observed around the mucosal layers of the intestinal tissue, especially within the lamina propria. In addition, a number of bacterial colonies were associated with red blood cells and blood vessels of the organs examined.

    Notes: We present a comprehensive theoretical investigation to optimize 3.5–4.5 μm InGaSb/InGaAlSb quantum-well (QW) lasers grown on ternary InGaSb substrates. We use an eight-band k⋅P Hamiltonian to calculate the Auger recombination and optical absorption coefficients in the active region, as well as the gain and threshold characteristics. The dominant Auger process involves hole excitation from the QW to unbound valence states. For structure optimization we varied the Ga content in the substrate and QW barrier layers. The optimized structure was obtained by maximizing the strain in the QWs, despite the Auger coefficient also increasing with strain. It is, therefore, demonstrated that the main aim for midinfrared laser optimization can be minimization of the threshold carrier density rather than reduction of the Auger coefficient. © 2001 American Institute of Physics.

    Notes: Dilute nitrogen alloys of InSb exhibit strong band gap bowing with increasing nitrogen composition, shifting the absorption edge to longer wavelengths. The conduction band dispersion also has an enhanced nonparabolicity, which suppresses Auger recombination. We have measured Auger lifetimes in alloys with 11 and 15 μm absorption edges using a time-resolved pump-probe technique. We find the lifetimes to be longer at room temperature than equivalent band gap Hg1−yCdyTe alloys at the same quasi-Fermi level separation. The results are explained using a modified k⋅p Hamiltonian which explicitly includes interactions between the conduction band and a higher lying nitrogen-related resonant band. © 2001 American Institute of Physics.

    Notes: This review begins by outlining the history of probability theory, exposing cultural differences between scientists and lay people in the way risks are viewed. The basic principles of the science of risk perception are described, and the various methods of communicating risk in health care, both verbal and numerical, are then discussed critically. These concepts are then applied to the practice of anaesthesia. Risk perception may affect anaesthetists' choice of career and may be involved in the genesis and evolution of critical incidents; we also discuss possibilities for training in risk perception issues. The place of risk communication in informed consent and its ethical implications are discussed.
