    Description / Table of Contents: Abstract Anaesthesia both for adenotomy (AT) and for tonsillectomy (TE) frequently presents a challenge. On one hand, children scheduled for adenotomy often have upper airway infections and are thus at risk of laryngo- and bronchospasm; on the other hand the ENT surgeon and the anaesthetist have to share the “workspace” in the patient's mouth. Since the succinyl choline debate in the early 1990s, the question of the best muscle relaxant has gone hand in hand with that of the most appropriate means of securing the airway. The concept of the laryngeal mask as airway was initially greeted with scepticism. Following several years' use of the mask for this purpose in AT and TE in young children, we report our experience and summarise the literature on this topic. The laryngeal mask represents a safe alternative to intubation, provided there is close cooperation with the ENT surgeon.
    Notes: Zusammenfassung Die Narkosen zur Adenotomie, aber auch zur Tonsillektomie stellen für Anästhesisten immer wieder große Herausforderungen dar. Zum einen haben die Kinder, die zur Adenotomie anstehen, häufig Infekte der oberen Luftwege und sind damit laryngo- und bronchospasmusgefährdet, zum anderen müssen sich HNO-Arzt und Anästhesist den “Arbeitsraum” im Mund teilen. Seit der Succinylcholindebatte der frühen 90er-Jahre stellte sich mit der Frage nach dem geeigneten Muskelrelaxans die Frage nach der geeigneten Atemwegssicherung. Der Anwendung der Larynxmaske als Airway bei AT und TE wurde zunächst mit Skepsis begegnet. Nach langjähriger Anwendung der Larynxmaske zur Atemwegssicherung bei AT und TE im Kleinkindesalter wird hier nicht nur über Ergebnisse und langjährige Erfahrungen berichtet, sondern auch die Literatur zu diesem Thema zusammengefasst. Daraus ergibt sich, dass die Larynxmaske bei AT und TE eine sichere Alternative zur Intubation darstellt. Voraussetzung dazu ist jedoch eine gute Kooperation mit dem HNO-Arzt.

    Notes: Abstract  The term "background" is discussed and a definition is suggested to put an end to the currently unsatisfying (non-)definition of geochemical or natural background. Based on the requirements stated in the definition, several simple and robust statistical methods are applied to different data sets (n〉50) from the atmosphere, pedosphere, and biosphere in order to explore their potential for the evaluation of a useful and robust background. Compared with the original data set both the calculated distribution, based upon the lower 50% of the values, as well as a 2σ-approximation of the normalised data set lead to promising and realistic results. Both methods seem appropriate for a fast and reliable evaluation of likely upper limits of background values. Nevertheless, even this robust method is not able to present absolute and doubtlessly correct background values. True quantification of any natural or geochemical background still requires a thorough investigation and is impossible without costly expert knowledge.

    Notes: Abstract All variables of several large data sets from regional geochemical and environmental surveys were tested for a normal or lognormal data distribution. As a general rule, almost all variables (up to more than 50 analysed chemical elements per data set) show neither a normal or a lognormal data distribution. Even when different transformation methods are used more than 70 % of all variables in every single data set do not approach a normal distribution. Distributions are usually skewed, have outliers and originate from more than one process. When dealing with regional geochemical or environmental data normal and/or lognormal distributions are an exception and not the rule. This observation has serious consequences for the further statistical treatment of geochemical and environmental data. The most widely used statistical methods are all based on the assumption that the studied data show a normal or lognormal distribution. Neglecting that geochemcial and environmental data show neither a normal or lognormal distribution will lead to biased or faulty results when such techniques are used.

    Notes: Abstract Coexpression of cytokine genes together with antigen-encoding genes in DNA vaccination vectors can increase humoral and cellular immune responses and may steer them in a Th1 or Th2 direction. In this study, the modulatory effect of interleukin (IL)-2, IL-4, and interferon (IFN)-γ coexpressed with the 60-kDa heat shock protein (Hsp60) of Yersinia enterocolitica O:8 (Y-Hsp60) was studied. DNA vaccination with y-hsp60 evoked specific humoral and cellular immune responses as well as reduction of the splenic bacterial load upon challenge with Y. enterocolitica in a mouse infection model. Coexpression of IL-2 or IFN-γ enhanced Y. enterocolitica-specific total IgG (P 〈 0.05) and IgG2a antibody responses. Coexpression of IFN-γ also improved the proliferative T cell responses upon stimulation with Y-Hsp60. A reduction of the splenic bacterial load as compared with the plasmid encoding Y-Hsp60 only was found for the IFN-γ coexpressing vector. Thus, coexpression of cytokine genes such as IFN-γ in DNA vaccination vectors might improve immunity and help to overcome the side effects of standard adjuvants.

    Description / Table of Contents: Abstract Background and Objectives. Treatment of pruritus as observed in many dermatological and internal diseases may be very often disappointing. There are many reports describing the topical use of capsaicin, the pungent agent in red pepper. Long term administration of capsaicin depletes neuropeptides in unmyelinated, polymodal C-type and small myelinated Aδ-type cutaneous nerves that conduct pruritus and pain. Patients/Methods. The aim of this study was to evaluate the efficacy, safety and practicability of capsaicin in the treatment of pruritus and pain of different origin in a total of 40 patients. Results. Capsaicin suppressed completely itch in all patients. Conclusions. In addition to its excellent antipruritic and analgetic effect, capsaicin significantly contributed to healing of some dermatoses, such as prurigo nodularis and psoriasis.
    Notes: Zusammenfassung Hintergrund und Fragestellung. Juckreiz ist eine schwer therapierbare Begleiterscheinung zahlreicher Dermatosen und innerer Erkrankungen. In den letzten Jahren wurde Capsaicin, ein Alkaloid aus der Paprikapflanze, als alternative Behandlungsmöglichkeit empfohlen. Capsaicin blockiert bei topischer Applikation nach einer initialen Phase der neurogenen Entzündung unmyelinisierte, polymodale C- und Aδ-Nervenfasern und unterdrückt dadurch selektiv Juckreiz und Schmerz. Patienten/Methodik. Es wurden 40 Patienten mit verschiedenen juckenden und schmerzhaften Erkrankungen mit Capsaicin behandelt. Im Folgenden wird über Möglichkeiten, Nebenwirkungen und Grenzen der Capsaicintherapie berichtet. Ergebnisse. Die topische Capsaicintherapie führte in sämtlichen Fällen zum vollständigen Sistieren des Juckreizes. Schlussfolgerungen. Neben seiner ausgezeichneten symptomatischen antipruritischen und analgetischen Wirkung greift Capsaicin in pathophysiologische Mechanismen bestimmter Dermatosen, v.a. der Prurigo nodularis und Psoriasis ein und unterstützt ihre Abheilung.

    Notes: Laser spectroscopy, i.e., resonant two-photon ionization (R2PI), IR/R2PI ion depletion and hole burning spectroscopy have been applied in an experimental study of heterogenous clusters consisting of fluorobenzene (FB) or p-difluorobenzene (pDFB), respectively, microsolvated by up to three methanol molecules. Their infrared ion depletion spectra were taken in the region of the OH and CH stretches of methanol. In these complexes the methanol molecules form subclusters, which are weakly hydrogen bonded to either the aromatic π-system (πOH) or to the fluorine substituent (σF) and a CH group (σCH). In FB⋅(MeOH)1, pDFB⋅(MeOH)1, pDFB⋅(MeOH)2 and one isomer of FB⋅(MeOH)2 the methanol subunits exclusively exhibit σF and σCH H-bonds. A further isomer of FB⋅(MeOH)2 exhibits a πOH type interaction. For FB⋅(MeOH)3 and pDFB⋅(MeOH)3 (1:3) complexes the methanol subcluster may take on either a chainlike or a ringlike conformation. In the chainlike isomer of FB⋅(MeOH)3 the methanol trimer interacts with the chromophore via a πOH H-bond while with the corresponding pDFB cluster a σF H-bond is formed. High level ab initio calculations, still feasible for molecular clusters of this size, have also been carried out to supplement the experimental investigations. These calculations include complete geometry optimizations of FB⋅(MeOH)n (n=1–2) and pDFB⋅(MeOH)1 at the MP2/6-31+G* and B3LYP/6-31+G* levels of theory. The minimum energy structures of the microsolvates together with their harmonic vibrational spectra allow a reliable assignment of the experimentally observed spectra and the determination of structure of the complexes. © 2000 American Institute of Physics.

    Notes: This combined experimental and computational study is aimed at elucidating the structure and reactivity of heterogeneous molecular clusters. We report results for the system para-chlorofluorobenzene/methanol (pClFB/MeOH). Particularly, three different van der Waals (vdW) isomers of the neutral (1:1) aggregate (π,σF,σCl) have been assigned by comparison of experimental infrared frequencies in the O–H and C–H stretch regions measured by IR/R2PI depletion spectroscopy with calculated frequencies at MP2/6-31+G(d) and B3LYP/6-31+G(d) level. The isomers are weakly hydrogen-bonded complexes with methanol's OH group coordinated toward the aromatic π-electron cloud, the fluorine or the chlorine substituent, respectively. This assignment is corroborated by the UV and IR spectra of the corresponding monosubstituted benzene/methanol complexes. After resonant photoionization of pClFB⋅(MeOH)n, for n=1 besides vdW fragmentation, no chemical reactivity was observed for any isomer. The investigation of aggregates with n≥5 indicated a substitution reaction taking place to produce chloroanisole+, emphasizing the importance of neutral precursor cluster size over cluster structure for intracluster ion–molecule reactions. © 2000 American Institute of Physics.

    Notes: Summary:  An expression system for DNA vaccines is described, in which a fusion protein with an N-terminal, viral J-domain that captures heat-shock proteins (Hsps) is translated in-frame with C-terminal antigen-encoding sequences (of various lengths and origins). The system supports enhanced expression of chimeric antigens (of 〉800 residues in length) with an extended half life (〉8 h). When used as a DNA vaccine, it delivers antigen together with the intrinsic adjuvant activity provided by bound Hsps. We describe the design of vectors for DNA vaccination that support the expression of different immunogenic domains of different origins as large, Hsp-capturing chimeric fusion antigens. The immunogenicity of the antigens produced by this expression system (when it is built into DNA vaccines) has been characterized in detail, with particular emphasis on priming CD8+ T-cell responses. We also discuss areas of vaccine research to which the new technology may provide useful contributions.
