    Notes: Summary In the attempt to evaluate the role of Autologous and Allogeneic Bone Marrow Transplantation, we have retrospectively analyzed 159 patients with Acute Myeloid Leukemia in first complete remission treated in our Unit, most of whom were referred from other Institutions. High-dose therapy was uniform and consisted of cyclophosphamide 60 mg/kg/d on two consecutive days and TBI in a single dose (10 Gy) for ABMT patients and in fractionated doses (3.3 Gy × 3 days) for BMT patients. Eight years actuarial survival was similar in two groups (52% for BMT and 49% for ABMT). The actuarial risk of relapse for BMT and ABMT was 29% and 43%, respectively. Considering that none of ABMT patients was “purged” with in vitro technique, this review seems to confirm the importance of “in vivo” purging with postremission intensification, immediately before the harvesting. Of course, more patients and a longer follow-up are needed to drow final conclusions.

    Notes: Summary In the attempt to induce graft-vs-host disease (GVHD) in patients undergoing autologous bone marrow transplantation (ABMT) or blood stem cell transplantation (BSCT), 12 consecutive patients received cyclosporin A (CyA) post transplant. CyA was given at a dose of 1.5 mg/kg/day intravenously for 28 days, starting on the day of transplant. Histologically proven acute GVHD of the skin occurred in seven patients 9–14 days after ABMT and lasted 7–11 days. CyA-induced GVHD after ABMT resembles mild GVHD after allogeneic bone marrow transplantation (BMT). Although the preliminary data reported here show that it is possible to induce GVHD in patients undergoing ABMT, it is not yet known whether GVHD after ABMT will have an antitumor activity.

    Description / Table of Contents: Sommario Una donna di 77 anni fu ricoverata a distanza di un'ora dall'insorgenza di emiplegia destra con afasia. Una Risonanza Magnetica dell'encefalo risultò negativa, mentre una Angio-RM indicava l'occlusione dell'arteria cerebrale media di sinistra; fu quindi intrapresa la terapia con streptokinasi. Dopo due ore dall'infusione, le capacità motorie della paziente migliorano sensibilmente. Una Angio-RM effettuata sei ore più tardi evidenziò la parziale ricanalizzazione del vaso occluso. Questo caso dimostra la potenziale utilità del monitoraggio con Angio-RM nel corso della terapia trombolitica.
    Notes: Abstract A 77 year old woman was hospitalized one hour after the onset of right hemiplegia and aphasia. Magnetic resonance imaging (MRI) examination was negative and MRA indicated occlusion of the left middle cerebral artery. Treatment with streptokinase was instituted (a 60 minute intravenous infusion of 1.5 MU of streptokinase in 100 ml of saline solution). Two hours after infusion, the patient's motor function clearly improved. An MRA examination performed six hours later showed partial recanalization of the obstructed vessel. Our report demonstrates the potential usefulness of MRA monitoring during thrombolytic therapy.

    Description / Table of Contents: Sommario Abbiamo esaminato 21 pazienti affetti da patologia cerebro-vascolare ischemica con Angio-RM e Doppler Transcranico. In 17 pazienti (81%) i dati ottenuti con le due metodiche sono risultati concordanti. L'uso associato del Doppler Transcranico e dell'Angiografia a Risonanza Magnetica può permettere, a nostro avviso, una accurata valutazione diagnostica del paziente affetto da stroke ischemico consentendo l'individuazione della sede della eventuale patologia steno-occlusiva dei vasi cerebrali di medio e grosso calibro oltre alla possibilità di ottenere informazioni di tipo dinamico riguardanti la direzione del flusso ematico in presenza di eventuali circoli di compenso a livello del poligono di Willis.
    Notes: Abstract We examined 21 patients with ischemic cerebrovascular disease using magnetic resonance angiography and transcranial Doppler. In 17 patients (81%), the results obtained by the two methods were concordant. In our opinion, the associated use of transcranial Doppler and magnetic resonance angiography provides an accurate diagnostic evaluation of ischemic stroke patients, not only allowing the site of possible steno-occlusive disease of large and medium cerebral vessels to be identified, but also offering the possibility of obtaining dynamic information concerning the direction of blood flow in the presence of compensatory circulations at the level of Willis' circle.
