    Notes: Abstract This paper highlights the fundamental role of MRI in the diagnosis of Neuro-Behçet disease. We present 4 cases examined between 1990 and 1991. Treatment in all the cases seems to have improved the patients' prognoses.

    Description / Table of Contents: Sommario Una donna di 77 anni fu ricoverata a distanza di un'ora dall'insorgenza di emiplegia destra con afasia. Una Risonanza Magnetica dell'encefalo risultò negativa, mentre una Angio-RM indicava l'occlusione dell'arteria cerebrale media di sinistra; fu quindi intrapresa la terapia con streptokinasi. Dopo due ore dall'infusione, le capacità motorie della paziente migliorano sensibilmente. Una Angio-RM effettuata sei ore più tardi evidenziò la parziale ricanalizzazione del vaso occluso. Questo caso dimostra la potenziale utilità del monitoraggio con Angio-RM nel corso della terapia trombolitica.
    Notes: Abstract A 77 year old woman was hospitalized one hour after the onset of right hemiplegia and aphasia. Magnetic resonance imaging (MRI) examination was negative and MRA indicated occlusion of the left middle cerebral artery. Treatment with streptokinase was instituted (a 60 minute intravenous infusion of 1.5 MU of streptokinase in 100 ml of saline solution). Two hours after infusion, the patient's motor function clearly improved. An MRA examination performed six hours later showed partial recanalization of the obstructed vessel. Our report demonstrates the potential usefulness of MRA monitoring during thrombolytic therapy.

    Description / Table of Contents: Sommario Abbiamo esaminato 21 pazienti affetti da patologia cerebro-vascolare ischemica con Angio-RM e Doppler Transcranico. In 17 pazienti (81%) i dati ottenuti con le due metodiche sono risultati concordanti. L'uso associato del Doppler Transcranico e dell'Angiografia a Risonanza Magnetica può permettere, a nostro avviso, una accurata valutazione diagnostica del paziente affetto da stroke ischemico consentendo l'individuazione della sede della eventuale patologia steno-occlusiva dei vasi cerebrali di medio e grosso calibro oltre alla possibilità di ottenere informazioni di tipo dinamico riguardanti la direzione del flusso ematico in presenza di eventuali circoli di compenso a livello del poligono di Willis.
    Notes: Abstract We examined 21 patients with ischemic cerebrovascular disease using magnetic resonance angiography and transcranial Doppler. In 17 patients (81%), the results obtained by the two methods were concordant. In our opinion, the associated use of transcranial Doppler and magnetic resonance angiography provides an accurate diagnostic evaluation of ischemic stroke patients, not only allowing the site of possible steno-occlusive disease of large and medium cerebral vessels to be identified, but also offering the possibility of obtaining dynamic information concerning the direction of blood flow in the presence of compensatory circulations at the level of Willis' circle.

    Description / Table of Contents: Zusammenfassung Bei einem 30jährigen Manne trat ein linksseitiger Herpes zoster ophthalmicus mit einer Hemiparese auf der rechten Körperseite auf. Eine linksseitige Carotisangiographie zeigte eine segmentale Einengung, die vereinbar war mit einer Arteriitis. Man erwog eine direkte Einwirkung des Virus, ausgehend vom intracraniellen Anteil des N. ophthalmicus. Im Computertomogramm des Schädels war eine ischämische Zone nachweisbar, die als sekundäre Auswirkung der Arteriitis angesehen wurde und das zentralnervöse Substrat der Hemiparese darstellte. Die Zunahme eines oligoklonalen IgGs und IgA im Liquor wies auf eine Immunantwort innerhalb der Blut-Liquor-Schranke hin.
    Notes: Summary Herpes zoster of the ophthalmic division of the left fifth cranial nerve with contralateral hemiparesis was observed in a 30-year-old man. Left carotid angiography showed segmental constrictions consistent with cerebral arteritis, possibly provoked by direct viral infection along the intracranial part of the ophthalmic nerve. An ischaemic lesion revealed by computed tomographic scan was considered secondary to arteritis and responsible for the hemiparesis. The presence of an immune response within the blood-CSF barrier was suggested by an increase of oligoclonal CSF IgG and IgA.

    Notes: Summary We have found numerous case reports, but no systematic study of the megadolichoverte-brobasilar anomaly (MDVBA). The purpose of this paper is to evaluate the relationships between arterial shifts of the vertebro-basilar system and neurological findings in the posterios fossa in our series of 132 cases. We found a high percentage (77.3%) of angiographic-clinical correlations having evaluated the arterial shifts, measured in mm, of the vertebro-basilar system in a frontal and a sagittal plane and concluded that the greater the degree of dislocation, the greater the number of positive cases. Nevertheless it is not possible to predetermine the presence of particular neurosymptomatology related to arterial dislocation degrees.

    Description / Table of Contents: Résumé L'auteur insiste sur le valeur pathogénétique des anomalies congénitales des arcs de l'atlas en cas de céphalées occipitales. Des 51 observations qui présentaient ces anomalies à l'examen radiographique du rachis cervical, le 50.9% accusait des céphalalgies. — Par contre, les autres malades se plaignaient d'une symptomatologie d'insuffisance vertébro-basilaire. Dans ces cas les anomalies de l'atlas sont responsables de troubles statiques et dynamiques du rachis cervical et d'une compression directe sur les artères du cou.
    Abstract: Zusammenfassung In dieser Arbeit wird die pathogenetische Bedeutung der leichten Entwicklungsanomalien verschiedener Segmente des Atlanten als Entstehungsursache eines typischen Kopfschmerzes hervorgehoben. Dieses Symptom war bei 50.9% der 51 Fälle vorhanden, die lediglich auf Grund roentgenologischer Befunde ausgesucht worden waren. Bei anderen Fällen verursacht die Knochenveränderung mittels statischer Störung der Gelenkdynamik der gesamten Halswirbelsäule, die sich auf die Arterien auswirkt, eine vertebro-basilare Insuffizienz.
    Notes: Summary The author emphasises the pathogenetic importance of slight anomalies of various elements of the first cervical vertebra in determining typical headache symptoms. — In 51 patients with such development anomalies chosen on the basis of X-ray findings this symptom is found present in 50.9 per cent of the cases. —In other cases the bone deformity, through changes in the articular statics and dynamics of the whole cervical spine, determines symptoms of vertebro-basilar insufficiency because of an effect on the vascular afferences.
