Notes: Summary We have found numerous case reports, but no systematic study of the megadolichoverte-brobasilar anomaly (MDVBA). The purpose of this paper is to evaluate the relationships between arterial shifts of the vertebro-basilar system and neurological findings in the posterios fossa in our series of 132 cases. We found a high percentage (77.3%) of angiographic-clinical correlations having evaluated the arterial shifts, measured in mm, of the vertebro-basilar system in a frontal and a sagittal plane and concluded that the greater the degree of dislocation, the greater the number of positive cases. Nevertheless it is not possible to predetermine the presence of particular neurosymptomatology related to arterial dislocation degrees.

Notes: Summary The author emphasises the pathogenetic importance of slight anomalies of various elements of the first cervical vertebra in determining typical headache symptoms. — In 51 patients with such development anomalies chosen on the basis of X-ray findings this symptom is found present in 50.9 per cent of the cases. —In other cases the bone deformity, through changes in the articular statics and dynamics of the whole cervical spine, determines symptoms of vertebro-basilar insufficiency because of an effect on the vascular afferences.

Notes: Summary The authors describe two cases of hemifacial spasm due to elongation of the terminal segment of the vertebral artery. One of the two patients had a bony malformation of the atlanto-occipital joint. This coincidence has indicated a vascular pathogenetic hypothesis in cases of hemifacial spasm with bone malformation of the atlanto-occipital joint.

Notes: Abstract We report a 13-year-old girl with leukaemia and Wernicke's encephalopathy induced by total parenteral nutrition. MRI showed unusual bilateral lesions of the caudate nuclei and cerebral cortex, as well as typical lesions surrounding the third ventricle and aqueduct. After intravenous thiamine, the patient improved, and the abnormalities on MRI disappeared.

Notes: Abstract The Klippel-Trenaunay syndrome (KTS) is a congenital disorder resulting from a mesodermal abnormality, characterised by cutaneous capillary haemangiomas, hypertrophy of bone and soft tissues and varicose veins. The presence of intracranial aneurysms has rarely been described, while oesophageal duplication has not been reported previously. We describe a patient with the KTS with both there additional abnormalities, which could be explained by a postulated mosaic gene abnormality.

Notes: Summary Measurement of the sagittal diameter of the atlas and axis revealed a significant reduction in many patients with slight anomalies of the atlas, in whom the most important symptom was cervico occipital headache.

Notes: Summary Nonionic water-soluble contrast medium, at first metrizamide and later Iopamidolo was used for lumbar myelography. Three serious neurological complications were noted: one case of total aphasia, one case of generalized seizures, and one of myoclonic jerking of the legs.
