    Immunohistochemical study of p53 expression in endometrial carcinomas: correlation with markers of proliferating cells and clinicopathologic features (1993)

    Notes: Using anti-p53 (PAb1801 and PAb240), anti-DNA polymerase α and Ki-67 monoclonal antibodies, the expression of p53 was studied in 11 normal endometria, 14 endometrial hyperplasias and 27 endometrial carcinomas and its relationship to the proliferative activity of the tumors was examined. Normal endometria and simple hyperplasias were completely negative for p53. The PAb1801 indices of complex hyperplasias and complex atypical hyperplasias were 2.5±1.8% and 5.0±3.2%, respectively. The PAb1801 indices of grade 1, grade 2 and grade 3 endometrial carcinomas were 10.2±14.2%, 44.4±29/0% and 45.0±32.5%, respectively. These results indicate a progressively enhanced p53 expression in the sequence from normal endometrium, through hyperplasia to carcinoma. A significant correlation between p53 expression and labeling indices of Ki-67 and DNA polymerase α was observed in endometrial carcinomas. The endo-metrial carcinomas with p53 overexpression developed mainly in post-menopausal patients and were frequently high-grade tumors with deep myometrial invasion. These findings may indicate that overexpression of p53 protein contributes to the proliferative activity of the tumor cells.

    Notes: Four types of L-tryptophan lots produced by genetically modified B. myloliquefaciens strains II-V, which consisted of case-associated (11) with manifestations of EMS, control (16), and other lots (11), were compared by multivariate computer recognition programs (chemometrics) including HCA, PCA, KNN and SIMCA, as well as PLS. In HCA data analysis lots from strains III and V formed discrete clusters, and in PCA and SIMCA, both lots formed separated groups. Lots from strains II and IV were included in a group of lots from strain III. This indicated strains II and IV too were closely related to strain III. When strain vs case-associated control or other lots were compared by four methods, SIMCA provided adequate strain separation and strains were well grouped in case-associated or control lots.

    Notes: Abstract Cholangitis after biliary reconstruction for congenital biliary atresia is a troublesome postoperative complication. In order to clarify its mechanism and the changes in intestinal movement after biliary reconstruction by Roux-en-Y anastomosis, we performed electromyographic (EMG) studies of the motility of the reconstructed intestine in dogs. Monitoring the basic electrical rhythm (BER) and migrating myoelectric complexes (MMC), EMG analysis was carried out. As most of the Y-loop MMCs were propagated smoothly to the anal side, according to the continuity of the intestine, the intestinal contents were transported without stagnation. These intestinal movements appear to be useful as a biliary drainage route and to prevent ascending cholangitis. A comparison of short- and long-term intestinal motility after biliary reconstruction showed adaptation of the intestinal movement following the procedure. The outflow of bile appeared to accelerate intestinal motility because of prolongation of the MMC interval in the duodenum and oral jejunum without bile flow.

    Notes: Abstract Objective The detrimental effect of positive airway pressure on right ventricular (RV) performance is controversial and the aim of this study was to determine the effects of constant positive airway pressure without ventilatory fluctuation on RV performance with the aid of a pulmonary arterial catheter equipped with a rapid response thermistor for measuring RV ejection fraction (RVEF) and RV end-diastolic volume index (RVEDVI). Design A prospective, clinical study. Setting The central operating theatre of a university hospital. Patients Nine patients who had major surgery and required right heart catheterization for normal clinical management. Measurements and results Cold indicator was injected into the RV 4 or 5 times for each airway pressure (0, 10 or 20 cmH2O) which was maintained manually stable for 15 s, and 9 paired data were analyzed by repeated-measures analysis of variance. They are separated into two groups; RVEF at zero airway pressure greater (A group) or less (B group) than 0.4. In A group (7 patients), increasing airway pressures (0 vs 10 vs 20 cmH2O) did not affect RVEF (0.55±0.05 vs 0.54±0.06 vs 0.56±0.04), RVEDVI (69±36 vs 73±29 vs 58±20 ml·m−2), or stroke volume index (SVI: 38±18 vs 40±17 vs 33±13 ml·beat−1 ·m−2); however, in B (2 patients), RVEF (0.35 and 0.38 vs 0.31 and 0.28 vs 0.19 and 0.17) and SVI (35 and 28 vs 32 and 27 vs 27 and 23) decreased, while RVEDVI increased (99 and 73 vs 103 and 97 vs 146 and 132). Conclusions In most patients, the changes in RVEF, SVI, and RVEDVI did not occur under constant positive airway pressure, therefore the changes reported in mechanically ventilated patients may not attributable to the extent of positive airway pressure but rather to abrupt increases in airway pressure. There appears, however, to be patients whose RV function is so disturbed that they cannot cope with increased afterloads.

    Notes: Summary We performed MRI in 7 cases with adrenoleukodystrophy (ALD), including 3 cases with childhood ALD, 3 cases with adrenomyeloneuropathy (AMN) and one symptomatic heterozygote. The symptomatic heterozygote was the mother of the patient with AMN. High-field-strength (2.0T) MRI was used in two cases with childhood ALD. In all 3 cases of childhood ALD, MR showed widespread lesions in both cerebral hemispheres. Areas of low intensity in the diencephalon or striatum on heavily T2-weighted images at 2.0T were seen in 2 cases with childhood ALD. Cerebral lesions confined to the internal capsule, cerebral peduncle and optic radiation in the adult onset ALD including AMN and symptomatic heterozygote, have not been reported and this confinement may indicate an early stage of the disease.

    Notes: Summary Polysomnography, including electroencephalography, electromyography and electro-oculography was performed in three patients with palatal myoclonus (PM). The amplitude of the myoclonus decreased during sleep. The frequency did not change during non-REM sleep, but increased during REM sleep in two patients. Ocular myoclonus synchronized with PM disappeared during deep sleep stages in two patients and reappeared during REM sleep in one of them. In the other patient, ocular myoclonus was noted only in REM sleep, being absent even when the patient was awake. All patients showed episodic EEG activities synchronous with myoclonic jerks only in REM sleep. These episodes were noted 5–15 times throughout the night, and each episode lasting for 1–7 s. They were negative or positive waves of saw-tooth appearance which were distributed predominantly in the central region. During the episodes, the frequency of myoclonic jerks increased in two patients. Although it is known that REM sleep influences PM and ocular myoclonus, this is the first report demonstrating the electroencephalographic activity associated with PM.
