    Description / Table of Contents: Zusammenfassung Bericht über 7 Patienten mit der Fehldiagnose eines inoperablen Gallenblasenoder Gallengangscarcinom, die 2mal endoskopisch und 5mal per Laparotomie gestellt wurde. In allen F←len handelte es sich nur um chronisch-entzündliche Veränderungen bei Gallensteinleiden (6mal), bei 1 Patient kam es zu einer Haemobilie, 1mal fand sich angiographisch eine „Infiltration” der A. hepatica. 6 Patienten konnten durch Cholecystektomie (4mal) oder Cholecystektomie mit en bloc-Resektion des D. hepatocholedochus (2) saniert werden. 1mal genügte die sekundäre Choledochus-Revision mit Entfernung einer Dauer-T-Drainage. Die intraoperative Diagnose eines inoperablen Gallenwegs-Carcinoms ist nur nach histologischer Sicherung erlaubt, da der makroskopische Befund allein leicht zu einer Fehldiagnose führen kann.
    Notes: Summary This is a report on seven patients who were falsely diagnosed as having inoperable biliary carcinoma, based on endoscopic retrograde cholangiography (2 patients) or intraoperative exploration (5). In fact, all cases had a benign origin, chronic inflammation for instance, due to gallstone disease (6), which was combined with hematobilia in one case. One patient even presented the characteristic arteriographic finding of malign infiltration to the hepatic artery. The operative procedures performed were cholecystectomy (4) and cholecystectomy with en bloc resection of the common bile duct (2). In one patient only was another choledochotomy with removal of the permanent T-stent necessary. Since macroscopic findings are easily misleading, definite intraoperative diagnosis of inoperable biliary carcinoma must be confirmed by tissue diagnosis.
