    Description / Table of Contents: Zusammenfassung Wir berichten über unsere experimentellen Untersuchungen und klinischen Erfahrungen mit der Hüftsonographie bei Säuglingshüften. Aufgrund von über 3000 Sonogrammen konnten wir eine Typisierung der Dysplasieformen vom sonographischen Gesichtspunkt aus durchfiihren. Es ist möglich, Hüftreifungsstörungen bereits nach der Geburt zu erkennen und den weiteren Verlauf lückenlos zu überwachen. Neue sonographische Erkenntnisse und Details ermöglichen es uns, eine differenziertere Therapie als bisher einzuschlagen.
    Notes: Summary Our report relates to our experimental investigations and clinical experience gained with the sonography of infant hips. On the basis of over 3,000 sonograms we were able to carry out a standardization of dysplasia forms from the sonographic point of view. It is possible to recognize disturbed hip maturation already after birth and to continuously control the further development. New sonographic findings and details permit to take up a more differentiated therapy as applied so far.

    Notes: Summary The relationship between the pre-stimulus glucose level and immunoreactive insulin responses to a glucose challenge (20-g IV) was studied in normal subjects. When the steady-state pre-stimulus glucose concentration was lowered by a 0.33 mU· kg-1· min-1 insulin infusion or raised by a 900mg/ min glucose infusion, no effect on first phase insulin secretion (mean Δ 3–5 min insulin level) was observed. In contrast, the second phase response (10–60 min insulin area after glucose pulse) to intravenous glucose fell during insulin infusion and increased during the glucose infusion. Overall, a linear relationship was found between the change of pre-stimulus glucose level from the control to that during the insulin or glucose infusion and the change in second phase response (r=0.65, n = 14, p〈0.02). The effect of tolbutamide infusion (7 mg·m-2· min-1) when compared with saline control was to increase both first phase (+54 ±13 mU/1, n=8, p 〈0.001, mean±SEM) and second phase (+972±256mU · mur-1 · 1-1, P〈0.01) insulinsecretion. It is concluded that the first phase response to a glucose pulse is independent of the steady-state pre-stimulus glucose concentration and is directly enhanced by tolbutamide; in contrast, second phase is related to both the steady-state pre-stimulus glucose level and tolbutamide. These findings suggest that changes in basal or pre-stimulus plasma glucose during therapy with sulphonylurea drugs may be expected to influence the second phase insulin responses to glucose challenge.

    Notes: Summary In the islets of the rat pancreas, steroid diabetes induced by triamcinolon-acetonid leads to degranulation of the B cells and glycogen infiltration. The glycogen cannot be satisfactorily detected using methods like the chromic acid technique according to Bauer, staining with Best's carmine, or the usually applied periodic acid-Schiff (PAS) reaction. Glycogen detection is improved, however, when lead tetraacetate is used in place of periodic acid as oxidizing agent. When combining the carbohydrate detection method with the peroxidase — antiperoxidase (PAP) method used for immunocytochemical detection of the various pancreatic islet hormones, paraffin sections reveal that glycogen is primarily localized in granulated B cells; the degranulated B cells also contain glycogen, though in smaller amounts. In contrast, the islet cells containing somatostatin, glucagon and pancreatic polypeptide are nearly free of glycogen.

    Notes: Summary Morphologically different pancreatic islet cells of the rat reveal immunoreactivity under incubation with anti-ACTH 1-39. In normal rats, anti-ACTH reactivity is detectable predominantly in peripheral, polygonal cells. Paraffin serial section treated alternately with anti-ACTH and antiglucagon show that the major portion of the anti-ACTH reactivity is localized in A cells. A few A cells, however, lack anti-ACTH reactivity. Anti-ACTH reactivity was also detected in individual, round-to-oval, occasionally quite large and more centrally situated cells of pancreatic islets. In contrast, islets of steroid diabetic rats reveal a different distribution of anti-ACTH reactive cells. The number of peripheral reacting cells is greatly reduced; whereas there is an increase in the number of immunoreactive, large, round or polygonal cells, which are distributed throughout the islet. Preliminary investigations indicate that adsorption of ACTH 18-39 and glucagon onto anti-ACTH 1-39 reduces immunoreactivity in normal and steroid diabetic rats, whereas adsorption of ACTH 1-24 does not.

    Description / Table of Contents: Zusammenfassung Das Ultraschallverfahren wurde in der Orthopädie wegen seiner schlechten Knochendurchdringung noch wenig eingesetzt. Da keine Strahlenbelastung vorliegt, bietet es sich jedoch besonders bei Säuglingen und Kindern als nichtinvasives Diagnoseverfahren an. In der Orthopädie ist die Diagnose der congenitalen Hüftluxation mit dem Ultraschall anzustreben. Wegen der Kleinheit des Objektes und der relativ geringen Auflösung stößt die Methode jedoch auf Schwierigkeiten. Es wird daher in der vorliegenden Arbeit im theoretisch-experimentellen I. Teil eine Abtasttechnik erarbeitet, mit deren Hilfe es unserer 2jährigen Erfahrung nach dem mit der Methode vertrauten und geübten Untersucher möglich ist, eine Diagnose zu stellen. Röntgenuntersuchungen könnten auf Zweifelsfälle und besonders ungünstige Fälle beschränkt werden. Im II. Teil werden erstmals praktische Ergebnisse aus den letzten 1 1/2 Jahren gezeigt und erläutert. Auf die Schwierigkeiten der Interpretation möchten wir besonders hinweisen. Es muß aber festgestellt werden, daß die vorliegenden Abbildungen bereits Reproduktionen sind. Die Bildqualität am Bildschirm ist erheblich besser, das Format größer. Wir sind überzeugt, daß es jedem Orthopäden nach relativ kurzer Zeit möglich ist, die vorgestellte Methode anzuwenden. Ein allgemeines Screening der Säuglingshüften liegt im Bereich der Möglichkeit.
    Notes: Summary Due to poor penetration of bone, ultrasonic techniques have not been popular in orthopedic procedures. Since they are non-invasive and are not associated with a radiation hazard, their application in the diagnosis of congenital hip-joint dislocations merits investigation. Methods are presented in Part I which should enable the experienced investigator to make the diagnosis and avoid falsh interpretations. In Part II, the practical results of these techniques over an 18 month period are presented and discussed. The small size of the object under examination and the, in general, poor resolving power of ultrasonoscopes constitute the major difficulties encountered when using ultrasonic diagnostic devices. The fact that these methods are harmless to the patient, economical, simple to perform, and noninvasive, make them attractive diagnostic tools for screening infant hip joints congenital luxations.

    Description / Table of Contents: Zusammenfassung Wir berichten über die Möglichkeit mit Hilfe des Ultraschalls die Feinstrukturen am Hüft pfannenerkerbereich bei Säuglingen abzuklären. Die Identifizierung des Erkerbereiches in Gelenkskapsel, Limbus und knorpelig präformierten Pfannenerker wird anhand experimenteller Untersuchungen erläutert. Bei der klinischen Anwendung ermöglicht der Einsatz des Ultraschalls ein Verfahren, um auf nicht schmerzhaftem nichtinvasivem, and vor allem nicht strahlenbelastendem Weg, der beliebig oft reproduzierbar ist, eine Diagnose zu stellen. Anhand von routinemäßigen Ultraschalluntersuchungen von Säuglingshüften, die wir zum Abschluß einer Hüftluxation durchführten, konnten erste Aussagen über die nicht im Röntgen sichtbaren oben erwähnten Strukturen gemacht werden. Dabei wird auf die prognostische Bedeutung des mit Hilfe des Ultra schalls dargestellten knorpeligen Pfannenerkers besonders hingewiesen.
    Notes: Summary We report about the possibilities to evaluate the fine structures of the area around the acetabular rim in infants using ultrasound. The identification of the rim within the joint capsule, limbus and cartilagineous preformed acetabular rim is explained by experimental investigations. The clinical use of ultrasound provides a diagnostic method which is painless, non invasive and especially avoids X-ray exposure. It also can be repeated as often as possible. First statements could be made about the above mentioned structures, not visible on the radiograph, according to routine ultrasonic examinations of in fantile hip joints, performed to exclude a luxation. We also point out the prognostic value of the cartilagineous acetabular rim demonstrated by ultrasound.

    Notes: Summary Unusual aggregations of ampullar dilated tubules (tubuloampullar structures, TAS) were observed in pancreatic B-cells of rats and in one case of human malignant insulinoma. The TAS were invariably associated with dilated rough endoplasmic reticulum, but lacked ribosomes. They were frequently seen to be closely associated with the Golgi apparatus. Furthermore, TAS were located mainly in degranulated areas of B-cells.
