    Description / Table of Contents: Zusammenfassung Bei drei Patientinnen am Ende der Gravidität (zwei Erstgebärende und eine Zweitgebärende) wurde eine stark erniedrigte Ausscheidung der Gesamtöstrogene im 24-Std-Harn festgestellt. Nach dem Dehydroepiandrosteronsulfatbelastungstest ließ sich die Östrogenausscheidung nicht verbessern. Der Plasmaspiegel des humanen placentaren Lactogens lag dagegen im Normbereich. Da die übrigen perinatalen Überwachungsparameter (USD, Cardiotokogramm) ebenfalls normale, der Gestationszeit entsprechende Befunde ergaben, wurde die Verdachtsdiagnose 3-Sulfatasemangel der Placenta gestellt. Die beiden Erstgebärenden mußten wegen einer totalen cervikalen Dystokie durch Kaiserschnitt entbunden werden. Alle Neugeborenen waren gesund und männlichen Geschlechts. Die in vitro-Inkubationsversuche des Placentagewebes (Homogenat oder Mikrosomenfraktion) mit radioaktiv markiertem Östronsulfat oder Dehydroepiandrosteronsulfat bestätigten den vermuteten Enzymdefekt gegenüber den Ergebnissen aus Inkubationen mit normalem Placentagewebe. Die placentaren Enzyme 17β-Hydroxysteroiddehydrogenase, Δ4/5-Isomerase und aromatisierendes System, wiesen dagegen eine normale Aktivität auf.
    Notes: Summary Three pregnancies (two I. parae and one II. para) with abnormally low urinary estrogen excretion of less than 5 mg estrogens per day have been investigated in this study. After intravenous injection of 100 mg DHA-S to the mothers no increase of estrogen production was observed. The level of human placental lactogen, was found within normal range. Ultrasonographic measurement of fetal size and monitoring of the fetal heart rate have demonstrated fetal benefit. At the end of pregnancy the cervix of both did not dilate after infusion of Syntocinon. Healthy male infants were delivered by Caesarean sections in both cases. The II. para, however, developed spontaneous labour and delivery at term. Clinical and laboratory tests have indicated the possibility of rare placental sulphatase deficiency. After delivery the placental steroid sulphatase activity was measured as the rate of hydrolysis of (6,7-3H) oestrone-sulphate or (4-14C) dehydroepiandrosterone-sulphate during in vitro incubation with the homogenate or the microsomal fraction from the pathological placentae. In all three cases the specific activity of 3-sulphatase was considerably (more than 80%) decreased when compared to three normal placentas, whereas the activity of the other enzymes (17β-hydroxysteroidoxidoreductase, Δ4/5-isomerase, aromatizing enzyme system) was found within the normal range.

    Notes: Summary We report on three independent cases with a partial deficiency of placental steroid sulfatase (E.C.3.1.6.2). Upon routine pregnancy monitoring these patients were detected on the basis of low estriol excretion and failing induction of labor. In all three cases a male was delivered and subsequently the diagnosis of partial deficiency of placental steroid sulfatase was confirmed enzymatically in placenta homogenates. In one case, fibroblast cultures were established from skin explants of mother and son. In fibroblasts of the child, as in placental tissue, the activity of steroid sulfatase was only 34% of normal. Similar values were obtained for arylsulfatase C, though this enzyme is clearly separable from steroid sulfatase by electrophoresis. In cells of the mother, enzyme activities were unremarkable.

    Description / Table of Contents: Zusammenfassung Das neue Aminoglycosidantibiotikum Tobramycin hat sich in einer prospektiven, nicht randomisierten, klinischen Studie bei schweren Systeminfektionen mit gram-negativen Keimen durch eine ausgesprochen starke antibakterielle Wirkung ausgezeichnet. Ototoxische Nebenwirkungen traten nur selten und dann in milder Form auf. Keiner von 18 behandelten Patienten zeigte signifikante Änderungen im Reintonaudiogramm vor und nach Behandlung sowie bei einer 9 Monate später vorgenommenen Kontrolle. Drei von 5 Patienten mit Niereninsuffizienz entwickelten leichtere vestibuläre Störungen, die durch elektronystagmografisch aufgezeichnete kalorische Prüfungen objektiviert wurden. Diese vestibulären Hyporeflexien erwiesen sich jedoch alle als reversibel wenige Wochen nach Unterbrechung der Medikation. Trotz der Seltenheit dieser Nebenwirkungen wird eine strikte kochleovestibuläre Überwachung während länger dauernden Tobramycinbehandlungen empfohlen, vor allem in Fällen mit Niereninsuffizienz.
    Notes: Summary In a clinical, prospective, non-randomized study, tobramycin has shown its high antibacterial activity in Gram-negative bacillary infections. The incidence of ototoxic side effects was very low: none of 18 patients studied before, at the end and 9 months after completion of tobramycin therapy showed any change in pure tone audiograms. Five of these 18 patients had renal impairment: 3 of them developed subjective and objective evidence of vestibular dysfunction, as demonstrated by caloric vestibulometry. However, these bilateral vestibular lesions all proved to be reversible a few weeks after cessation of therapy. Despite the low incidence and the reversibility of the ototoxic side effects, it seems advisable to monitor patients on long-duration tobramycin therapy for signs of vestibular dysfunction, particularly in cases of renal insufficiency.

    Notes: Summary The submucous resection of septum by Killian is not appropriate as a primary correcting treatment, especially in case of traumatical septum deformities. Due to the loss of cartilage often form and function of the nose deteriorates in a typical way. These complaints demand an operative correction under complicated circumstances. Based on the patient clientèle of the Univ.-HNO-Klinik Erlangen it is demonstrated how by secondary operations with the special technique of displacement of the remaining cartilage a satisfying result finally can be achieved.

    Description / Table of Contents: Zusammenfassung Die Kombination Massenspektrometer-Vielkanalanalysator wird zur quantitativen Aufnahme von Felddesorptions-Ionenströmen benutzt. Es wird gezeigt, daß die relativen Intensitäten von [M]+, [M + H]+ und [M + Na]+-Ionengruppen mit integrierender elektrischer Registrierung über eine Vielzahl von cyclischen Scans mit einem mittleren Fehler von 0,4% gemessen werden können. Anwendungen dieser direkten Isotopenanalyse zur Identifizierung von Verbindungen sowie zur Bestimmung des Markierungsgrades von stabilisotopen-markierten Substanzen werden vorgestellt und diskutiert.
    Notes: Summary The combination of a mass spectrometer with a time averaging computer is utilized for quantitative recording of field desorption ion currents. It is demonstrated that the relative intensities of [M]+, [M + H]+ and [M + Na]+ ion groups can be determined with an average error of 0.4% by intergrating electric recording of a large number of cyclic scans. Applications of this direct isotope analysis to the identification of compounds as well as to the determination of the label content of stable isotope labelled substances are introduced and discussed.
