Zusammenfassung Elektronenmikroskopische Untersuchungen an verschiedenen Typen hereditärer Ichthyosen und ein Vergleich ihrer abnormen Verhornung mit dem Keratinisierungsablauf normaler Haut zeigen, in welcher Weise genetische Faktoren die Keratinisierung beeinflussen können. Strukturell faßbare Defekte wurden bislang nur an autosomal dominanten Typen nachgewiesen (ADI, bullöse Erythrodermie, I.hystrix Typ Curth-Macklin); sie betreffen das Keratohyalin und die Tonofibrillen. Bei recessiv vererbten Ichthyosen wurden quantitative Störungen im Differenzierungs- und Reifungsprozeß der Keratinocyten gefunden. Genetische Faktoren beeinflussen die quantitativen Verhältnisse der Zellkomponenten (Keratinosomenverminderung bei XRI), die Proliferationsrate (I.congenita, bullöse Erythrodermie, I. h. Typ Curth-Macklin) und den Zellteilungsablauf (I. h. Typ Curth-Macklin). Schließlich ist eine Beeinträchtigung der Keratinisierung auch über Enzymdefekte des Stoffwechsels möglich (Lipidstoffwechselstörungen beim Refsum-Syndrom).

Summary The present paper summarizes and discusses recent electron microscopic findings in several common as well as rare types of inherited ichthyoses and their different keratinization patterns. On the whole, 28 cases of inherited ichthyoses have been investigated and are compared to normal keratinization in a material of 26 cases. It becomes evident that genetic factors may interfere with several consecutive steps of keratinization. They may influence structural components of the keratinocyte, disturbing their ultrastructural features (tonofilament defects in bullous erythroderma and ichthyosis hystrix type Curth-Macklin, defective keratohyalin in autosomal dominant ichthyosis vulgaris) or their frequencies (decreased keratinosome numbers in x-linked ichthyosis). They may further interfere with the proliferation rate of the epidermal cells (I. congenita, hystrix-like ichthyoses) or with the course of cell division (Ichthyosis hystrix type Curth-Macklin). Finally, they may indirectly impair keratinization by enzyme defects of the metabolism (disturbance of the lipid metabolism in Refsum's syndrome). Structural defects have yet been found only in types with autosomal dominant inheritance (autosomal dominant ichthyosis vulgaris, bullous erythroderma, ichthyosis hystrix type Curth-Macklin).
