    Notes: Summary A 50-year-old male patient had three episodes of acrally located purpura within 12 years without any other symptoms. The first two episodes which lasted several months coincided with the intake of beta-blockers; the third episode developed after a respiratory infection. A cryofibrinogenemia was found without any evidence of an underlying primary disease, especially not of a neoplasm. The clinical, histological, immunohistological, and electron microscopical findings, as well as the results obtained by intravital fluorescence videomicroscopy are presented.

    Notes: 3H-thymidine incorporation into epidermal cells of four patients with morbus Darier was studied by an in vitro technique. The labeling index counted on 0.5 μ thick plastic sections was 13.7 ± 5.3, indicating increased epidermopoieses in morbus Darier.

    Notes: Biospy specimens from three patients with morbus Darier were studied by electron microscopy: Grains appeared in groups in the midst of dyskeratotic lesions and were recognized as being the result of more advanced acantholysis and precocious keratinization. Corps ronds were formed individually in the regions lateral to that of grains, where hypergranulosis was prominent in contrast to a mild dyskeratosis. Therefore, the formation of grains and corps ronds seem to be independent of each other. While in the literature acantholysis has mainly been ascribed to the separation of des-mosones into two segments, another initial process seems to be the primary disappearance of the attachment plaque along with the connecting tonofilaments. From the existence of the other contact complexes, it seems likely that a restoration of desmosomes between acantholytic cells should take place. In early stages of dyskeratotic cells, keratinosomes were increased in number and some of them persisted inside the plasma membrane without a release into the intercellular spaces, and thus they were present in grains and corps ronds. The dyskeratosis in morbus Darier is characterized by a premature, incomplete and individual keratinization as evidenced by the existence of keratohyalin granules and keratinosomes as early as at the level of the 3rd epidermal layer and the absence of a keratin pattern in the dyskeratotic horny cells.

    Notes: Cylindroma basement membranes (BMs) were studied by electron microscopy and immunofluorescence with antibodies to Types IV and VII collagen in order to correlate the ultrastructure with the biochemical characteristics. Type IV collagen served as a marker for lamina densa structures and Type VII collagen for the anchoring fibrils. Ultrastructurally, the cylindroma BMs were composed of a wide band of BM-like material in which numerous anchoring fibrils were embedded. The BMs surrounding the tumor cell clusters and nodules of BM-like material entrapped within the tumor cell islands stained positively for both Types IV and VII collagen. We conclude, that anchoring fibrils are a major component of cylindroma BM zone. A trichoepithelioma investigated as control showed a BM similar to the one found at the dermoepidermal junction, i.e. a BM distinctly different from the one of cylindroma. The trichoepithelioma BMs and nodules of BM-like material included within the tumor also stained positively for both Type IV and VII collagen.

    Notes: Fifty sets of twins (thirty monozygotic [MZ] and twenty dizygotic [DZ]), of whom at least one twin had a history of atopy, were investigated by means of skin tests, RAST and IgE determinations. The concordance of atopies for MZ was, with 56.7%, significantly higher as compared with the DZ with 20%. The mean values of the log serum IgE level differences for MZ and DZ sets of twins was not statistically significant but the percentage distribution of the log serum IgE level differences showed a closer correlation of the IgE levels among MZ. The MZ sets do not show a better concordance in the specific reagin production, as measured by RAST or skin tests, than the DZ sets. These findings seemed to indicate that although the tendency to IgE production is genetically determined its specificity is governed mainly by environmental influences.

    Notes: Nails from four patients, infected with dermatophytes, were investigated with the scanning electron microscope (SEM) to gain insight into the spatial arrangements of the dermatophytes within the nail. Fungal hyphae could he detected in nails of all four patients. A toenail from one patient infected with Trichophyton mentagrophytes was more extensively studied and the results are presented in this paper. Light microscopic observations with bright field illumination and Nomarski interference contrast confirmed the dermatophytic infection. Fungal hyphae found with the SEM distally on the ventral part of the toenail showed typical T. mentagrophytes structures and the comparison with cultured material clearly demonstrated the correspondence in morphology and size. Besides the fine structural morphology of the invasion of fungal hyphae into the nail plate between the horny cells and/or directly into corneocytes, “lunnel”-like holes were observed in paraffin embedded sections after removal of the paraffin. Scanning electron microscopy, with its enormous focal depth, yielded far more information than light microscopic techniques about the three dimensional behavior of dermatophytes in the nail.

    Description / Table of Contents: Zusammenfassung Anhand von 35 Exzisaten aus Kopf-, Rumpf- und Extremitätenbereich wurden die histopathologischen Veränderungen der melanotischen Präcancerose untersucht. Auf Grund dieser Analyse wird die morphodynamische Entwicklung der melanotischen Präcancerose zum malignen Melanom aufgezeigt. Auf der Basis verschiedener Haupt- und Nebenkriterien wird in Anlehnung an Duperrat ein Entwicklungsschema erstellt. Dies beinhaltet ein Primärstadium „stade éphélide”, welches in drei Fällen auch elektronenmikroskopisch untersucht wurde. Hierbei konnte festgestellt werden, daß die histologisch als banal imponierenden Veränderungen ultrastrukturell eine deutliche Atypie auf weisen. Das Sekundärstadium „stade lentigo” läßt sich in drei Entwicklungsstufen untergliedern. Besondere Schwierigkeiten bereitet histologisch der Übergang von einer melanotischen Präcancerose in ein malignes Melanom. Es wird daher vorgeschlagen, daß man solche Fälle weder als Präcancerose — diese Diagnose sollte nur dann gestellt werden, wenn der Prozeß mit Sicherheit präinvasiv intraepidermal verläuft — noch als melanotische Präcancerose mit Übergang in ein malignes Melanom bezeichnen sollte, sondern als Grenzfälle (Borderline cases).
    Notes: Summary The histopathological changes in lentigo maligna Hutchinson (precancerous melanosis of Dubreuilh) of the skin were examined in 35 biopsies on head, trunk, and extremities. The morphological dynamics of lentigo maligna is compared to malignant melanoma. A developmental system as suggested by Duperrat is put forward in accord with different major and minor criteria. This is composed of a primary stage called “stade éphélide”, which in three cases was also studied by electron microscopy. The banal histological changes showed a distinct atypical aspect on the ultrastructural level. The secondary stage called “stade lentigo” can be divided into three developmental stages. The transition of lentigo maligna to lentigo maligna melanoma is difficult to determine histologically. Therefore it is suggested that those cases be defined neither as lentigo maligna (premalignant melanosis) (this diagnosis should only be applied if the alterations are surely praeinvasive and restricted to the epidermis) nor as lentigo maligna with transition to lentigo maligna melanoma, but as borderline cases.
