Zusammenfassung Darstellung der Operationstaktik und Technik. Nach Fernsehbild und radiographischer Kontrolle der Gallengänge wird bei Cholecystolithiasis prinzipiell die Cholecystektomie vorgenommen; bei Choledocolithiasis müssen immer alle Konkremente aus den Hauptgallenwegen entfernt werden. Danach wird der Choledochus drainiert. Auch bei fehlender Lithiasis soll eine Choledochostomie erfolgen. Hiernach wird der Pankreas gründlich untersucht. Es ist unentbehrlich, Typ und Extension des Hämatoms sowie der Nekrose zu präzisieren. Sie werden maßgebend sein für die Wahl zwischen einfacher Drainage, Entfernung des nekrotischen Gewebes und Pankreasexhairese. Eine Drainage der Pankreasloge sowie der Bauchhöhle und eventuell der Flanken ist anzustreben. Eine Gastrostomie mit doppelläufiger Sonde, eventuell eine Y-förmige Jejunostomie, schließen den Eingriff ab. 23 frühzeitige Pankreasexhairesen wurden nach diesen Prinzipien durchgeführt mit 15 Heilungen und 8 Todesfällen.

Summary Surgical indications and techniques are described. First, the biliary tree is studied by peroperative cholangiography. When gallstones are present, a cholecystectomy is performed, if necessary combined with choledochostomy and careful exploration of the common bile duct. The biliary tree is always drained, even in the absence of gallstones. Next, the pancreas is explored. It is essential to determine the type and extent of haematoma and the amount of necrosis before deciding on treatment by drainage, excision of damaged tissues, or some form of pancreatectomy. A large drain in the region of the pancreas allows irrigation of the area in the postoperative period as well as drainage of the abdominal cavity. At the end of the operation a feeding gastrostomy or jejunostomy is performed. In our series of 23 patients who presented with acute fulminating pancreatitis and were treated by the methods described, 15 were cured and 8 died.
